Objective To examine plasma adrenomedullin(ADM) levels in patients with IgA nephropathy,and investigate the correlation of plasma ADM and urinary albumin.Methods Specific radioimmunoassay was used to measure the plasma concentration of ADM in 35 patients with IgA nephropathy and 22 controls,and to analyse correlation of plasma ADM level and urinary albumin. Objective To explore change and significance of serum and urinary transforming growth factor-? 1(TGF-? 1)in the patients with primary chronic glomerulonephropathy (PCG). Methods 65 patients with PCG were divided into four groups according to the degree of renal function damage: normal renal function,renal compensatory,renal decompensatory and renal function failure groups. Levels of serum and urinary TGF-? 1 were determined with ELISA, and the levels of urinary protein and renal function were examined as well. Results Levels of serum and urinary TGF-? 1 in patients with PCG were singificantly higher than those in control, and the levels of serum and urinary TGF-? 1 among four groups were significantly different. The improvement of hypoxia of tumor cells can effectively reduce their resistance to radiation and chemotherapy and im-prove the cure rate of the tumor. Recently, the compounds which are used to improve hypoxia of tumor cells and reduce radiation resist-ance are no longer just the original electrophilic radiosensitizers, and many potential targets as radiosensitizer for hypoxia also become research focus. Based on the mechanism of tumor hypoxia, the paper summarized the study progresses in four different radiosensitizers including new targeted agents, electrophilic radiotherapy sensitization agents, biological reductants and natural plant extracts.
4.Clinical observation of Medpor porous polyethylene implants in treatment of enophthalmos combined with orbital fracture

AIM: To observe the efficacy of Medpor porous polyethylene implants in treatment of enophthalmos combined with orbital fracture.
METHODS: Seventeen cases ( 17 eyes ) with enophthalmos caused by orbital fracture underwent surgical treatment with Medpor porous polyethylene implants. All accepted a 6-mo follow-up and the data of enophthalmos, eyeball movement and diplopia were collected.
RESULTS: The average difference of exophthalmos between damaged eyes and undamaged eyes was (3. 4±1. 5 ) mm preoperatively, two cases had residual 1mm enophthalmos 6mo after surgery, while other 15 cases were completely corrected. Seventeen cases suffered from eyeball movement restriction and diplopia preoperatively, 16 cases had normal eyeball movement without diplopia 6mo after surgery, 1 case with limitation of abduction and horizontal diplopia. There was no extrusion, rejection, infection or other complications occurred during follow-up.
CONCLUSION: Medpor porous polyethylene implants can effectively improve the orbit volume to repair enophthalmos caused by orbital fracture.
5.Comparative study of efficacy and stability of small incision lenticule extraction, FS-LASIK and LASIK for myopia with a follow-up of 6 months

AIM:To evaluate the efficacy and stability by comparing acuity and diopter of small incision lenticule extraction ( SMILE) , femtosecond laser in situ keratomileusis ( FS-LASIK ) and laser in situ keratomileusis ( LASIK ) in treating myopia with a follow-up of 6mo.
METHODS: A retrospective study, 42 cases ( 84 eyes ) received SMILE, 37 cases ( 74 eyes ) received FS-LASIK and 31cases (62 eyes) undergone LASIK in our hospital during Apr. 2014 to Jun. 2014 were involved. The follow-up data of 6mo was analyzed. The preoperative spherical equivalent was -5. 91±1. 83D, -5. 89±1. 96D, -5. 88±1. 68D in SMILE, FS-LASIK and LASIK group, respectively. The differences of preoperative best corrected visual acuity ( BCVA ) , pupil diameter ( PD ) and central corneal thickness ( CCT ) had no statistically significant between three groups. The postoperative uncorrected visual acuity ( UCVA) , BCVA and diopter were comparative analyzed at 1wk, 1, 3 and 6m after surgery.
RESULTS:1) No patients lost to follow-up of 1wk and 1mo. A total of 10 eyes (5 cases), 10 eyes (5 cases) and 8 eyes (4 cases) lost to follow-up of 3m in SMILE、FS-LASIK and LASIK group, respectively, and raised to 18 eyes (9 cases), 12 eyes (6 cases) and 14 eyes (7 cases) in follow-up of 6m. 2) At 1wk follow-up, the differences of UCVA between SMILE group, FS- LASIK group vs LASIK group was statistically significant respectively ( t=4. 098, P=0. 000;t=2. 493, P=0. 004). 3) In LASIK group, the differences of UCVA between 1wk vs 3, 6m follow-up was statistically significant respectively (t=3. 410, P=0. 001;t=3. 771, P=0. 000), the differences of UCVA between 1m and 6m follow-up was statistically significant (t=2. 283, P=0. 026). 4) The differences of diopter were not statistically significant among three groups at 1wk, 1, 3 and 6mo follow-up respectively (χ2=0. 119, P=0. 942;χ2=1. 504, P=0. 471;χ2=0. 949, P=0. 622; χ2=0. 277, P=0. 871). 5) the differences of eyes with UCVA≥5. 0 was statistically significant between SMILE group vs FS-LASIK group, LASIK group at 1wk follow-up (χ2=9. 249, P=0. 002<0. 05/3;χ2=12. 906, P=0. 000<0. 05/3), there was no significant statistical difference between FS-LASIK group and LASIK group (χ2=0. 500, P=0. 604). 6) there was no significant statistical difference of eyes with SE (±0. 50D) at any time post operation among three groups (χ2=0. 809, P=0. 697;χ2=1. 176, P=0. 634;χ2=0. 871, P=0. 736;χ2=0. 683, P=0. 770).CONCLUSION: All of SMILE, FS-LASIK and LASIK are effective and stable on treating myopia according to follow-up of 6mo. However, in this study, SMILE group shows more effective than FS-LASIK and LASIK at 1wk, which could enhance postoperative UCVA more rapidly.
6.Prevalence of refractive errors in middle school students in Lanzhou city

AIM: To assess the prevalence of refractive errors in middle school students in Lanzhou city and explore the risk factors for myopia.METHODS: A cross-sectional survey was conducted. A questionnaire assessed the students' socioeconomic background and visual tasks followed by visual acuity assessment and a full eye examination including slit lamp examination, fundus evaluation, retinoscopy, and subjective refraction.RESULTS: Among 2 256 enumerated students aged 15-19 years, 2 037 (90.3%) students had significant refractive errors. Myopia was the leading refractive error (1 951/2 256,86.5%), astigmatism was the second most common refractive error (921/2 256, 40.8%), but amblyopia (10/2 256, 0.4%),strabismus (5/2 256, 0.2%), hyperopia (4/2 256, 0.2%) and other treatable eye disorders were uncommon. Almost 95.3% of students with significant refractive errors wore spectacles before the survey. Age, sex, visual tasks, and a parental history of myopia were risk factors for myopia.CONCLUSION: The prevalence of refractive errors and the risk factors for myopia in schoolchildren in Lanzhou city are similar to those reported in other regions of China.Interventions of myopia progression should be performed to protect the visual acuity of school-aged students.

Objective To investigate the carotid atherosclerosis(AS) in hemodialysis(HD) patients.Methods Clinical index was measured and intma-media thickness(IMT) of extracranial common carotid artery and presence of atherosclerotic plaques were determined by high-resolution B-mode ultrasonography.Results The age,disease duration,HD duration,triglyceride(TG),low density lipoprotein(LDL),serum C-reactive protein(CRP),mean carotid artery IMT in HD with atherosclerotic plaque were significantly higher than HD without atherosclerotic plaque patients,however,the serum albumin(Alb) was significantly decreased.Mean carotid artery IMT is related to the disease duration,Alb,TG,LDL,CRP in HD patients.Conclusion The CRP,mean carotid artery IMT were increased in HD with atherosclerotic plaque.Mean carotid artery IMT is related to the disease duration,Alb,TG,LDL,CRP in HD patients.To improve malnutrition and decrease micro-inflammation,serum lipid might reduce the rate of AS in HD.
8.Effect of ω-3 polyunsaturated fattty acids-supplemented nutrition support on immune function and nutritional status of patients with gastrointestinal malignant tumor: a meta-analysis

Objective To systematically review the effect of ω-polyunsaturated fatty acids-supplemented nutrition support on the immune function and nutritional status of patients with gastrointestinal malignancies.Methods Databases of MEDLINE,EMBASE,Cochrance Central register of controlled trials,China National Knowledge Infrastructure (CNKI),China Biology Medicine disc (CBM) were searched.The search words were omega-3 polyunsaturated fatty acids,fish oil,cancer,turmor,neoplasms in English or Chinese.All the high quality randomized controlled trials were included after assessed with Jadad scale.RevMan 5.2 was used for statistical analysis.Results Altogether 12 studies were included.Meta-analysis results showed that CD4+% (I2 =0%,95% CI=3.44~6.93,Z=5.82,P<0.00001),CD8+% (I2 =0%,95% CI=2.44~7.13,Z=4.00,P<0.000 1),CD4+%/CD8+% (I2 =74%,95% CI=0.16~0.83,Z=2.87,P=0.004),IgA (I2=14%,95% CI=0.21 ~0.43,Z=5.84,P<0.00001),prealbumin (I2 =0%,95%CI =0.02 ~ 0.05,Z =3.94,P < 0.000 1) in the treatment group were higher than in the control group,C-reactive protein (I2 =76%,95% CI =-21.33 ~-5.03,Z =3.17,P =0.002) in the treatment group was lower than in the control group,the differences were all statistically significant.But there were no statistically significant differences in CD3 + % (I2 =0%,95% CI =-3.50 ~ 2.56,Z =0.31,P =0.76),IgG (I2 =92%,95% CI=-0.32 ~5.54,Z=1.74,P=0.08),IgM (I2 =99%,95% CI=-0.52~1.89,Z=1.11,P=0.27),total protein (I2 =0%,95%CI=-4.08 ~1.77,Z=0.78,P=0.44),albumin (I2 =29%,95% CI=-0.87 ~1.45,Z=0.49,P=0.63),transferrin (I2 =0%,95% CI=-0.23~0.19,Z =0.19,P =0.85) between treatment and control groups.Conclusion Omega-3 polyunsaturated fatty acids can improve the perioperative immune function of patients with gastrointestinal malignant tumor,reduce inflammatory and stress reactions,but have no obvious effect on the improvement of the nutritional status.
9.Concurrent chemoradiotherapy versus radiotherapy alone for T3-4N0-1M0 and T1-4N2-3M0 nasopharyngeal carcinoma after induction chemotherapy

Objective To compare the efficacy of concurrent chemoradiotherapy versus radiotherapy alone for T3-4 N0-1 M0 and T14 N2-3 M0 nasopharyngeal carcinoma (NPC) after induction chemotherapy.Methods From 2002 to 2005,400 patients with stage Ⅲ and Ⅳa NPC were randomly divided into 2 groups :induction chemotherapy followed by concurrent chemoradiotherapy group (IC/CCRT,201 patients),and induction chemotherapy followed by radiotherapy alone group (IC/RT, 199 patients).Subgroup analysis was conducted for 197 patients with stage T3-4N0-1M0 NPC and 203 with stage T1-4N2-3M0 NPC.Results The follow-up rate were 96.2%, with a median followg-up time of 3.9 years.For T3-4N0-1 M0 NPC patients in IC/CCRT group (104 patients) and IC/RT group (93 patients), the 3-year overall survival, disease-free survival, locoregional recurrence-free survival and distant metastasis-free survival rates were 84.0% and 85.9% (χ2=0.08,P =0.780) ,77.0% and 72.0% (χ2=0.44,P =0.510) ,89.5% and 92.3% (χ2=0.65 ,P = 0.420), and 84.9% and 77.0% (χ2= 1.59, P = 0.210), respectively; For T1-4 N2-3 M0 NPC patients in IC/CCRT group (97 patients) and IC/RT group (106 patients), the corresponding rates were 67.4% and 82.2% (χ2=3.48,P=0.060), 61.5% and 68.0% (χ2= 1.86,P=0.170), 86.2% and 87.0% (χ2=0.57 ,P =0.450) and 66.2% and 75.6% (χ2=2.07 ,P =0.150), respectively.Acute sideeffects were similar except more leucocytopenia in IC/CCRT group than IC/RT group.Conclusions Compared with IC/RT, IC/CCRT dose not improve the overall survival in patients with T3-4N0-1 M0 and T1-4 N2-3 M0 NPC.
