Objective To study the mechanism of insulin resistance in obesity, impaired glucose tolerance (IGT) and type 2 diabetes. Methods Insulin sensitivity index (SI), glucose effectiveness (S G) and insulin secretory function of ? islet cells were assessed by the reduced sample number of Bergman's minimal model method with frequently sampled intravenous glucose tolerance test in patients with obesity, IGT and type 2 diabetes as well as in the controls. Body mass index (BMI) and waist hip ratio (WHR) were determined in four groups. Results SI and S G were significantly higher in the controls than those in the IGT and type 2 diabetes patients. ObjectiveTo study the differentiation of adipose tissue-derived mesenchymal stem cells (ADMSCs) into cardiomyocytes in vitro. MethodsADMSCs were isolated and purified by the method of digesting and adhering to the culture plastis. The third generation of cells were determined with immunocytochemistry method and induced by 5-aza. On the 7th, 14th, 21st, 28th day after being induced, the surface antigen of myoblast cell and cardiac myocyte were determined, the expression of gene of GATA4 and Nkx2.5 were tested, and the content of atrial natriuretic polypeptide (ANP) assayed. ResultsImmunocytochemical staining showed CD44, CD13, CD105 positive, CD45, CD34, HLA-DR, factor Ⅷ negative. After being induced by 5-aza, the direction of cell arraying was gradually similar. The cells were stained positively for Desmin, α-Sarcomeric Actin, myosin heavy chain and Troponin T. The cells express GATA4 and Nkx2.5 and secrete ANP. ConclusionThere was mesenchymal stem cells in human adipose tissue. 5-aza may induce ADMSCs to differentiate into cardiomyocytes in vitro.

[Summary] A 61-year-old female patient was diagnosed as ACTH independent Cushing′ syndrome, right adrenal adenoma, right facial infection. The situation was well controlled with antibacterial agents, abscess incision drainage and ketoconazole therapy. The patient received partial adrenolectomy of her right adrenal gland 16 months later. This case indicated that anti-adrenal agents could be a reasonable choice when the Cushing′syndrome patients were acompanied with severe infection.

Objective To explore diagnosis,treatment and prognosis of intestinal atresia(IA) in neonates,so as to increase the healing rate.Methods Surgical treatment was given to 29 cases of congenital IA from Apr.2004 to Nov.2008.Comprehensive analysis was carried out on clinical manifestations and flat X-ray.All cases were with vomiting,15 cases were with normal fetal faeces.Stomach and intestinal atresia were observed in the upper abdomen in 19 cases,fluid levels in generous stomach and expended intestines were seen in 21 cases by X-ray.All children underwent operation,including ileum atresia(16 cases),duodenal atresia(7 cases),and jejunal atresia(6 cases).The types of atresia were type Ⅰ(9 cases),type Ⅱ(9 cases),type Ⅲa(9 cases),type Ⅲb(1 case) and type Ⅳ(1 case).Nine cases were treated with septum resection and then anastomosed longitudinally,3 cases underwent end-to-end anastomose and 17 underwent end-to-back anastomose.All cases were served in intravenous hyperalimentation.Results All cases lived postoperatively,and 1 child gave up for postoperatively treatment.Twenty-eight cases were cured.There was 1 case of obstruction of anastomatic,and was cured by non-operatively treatment.There was no anastomatic leakage and wound disruption.Twenty-six cases were followed up for 3 months and up to 4 years.These symptoms had no relapse.Conclusion Early diagnosis,appropriate surgical procedure and surrounding operation management are crucial for improving the cure rate.

Objective To evaluate the preoperative diagnosis and treatment of primary non-specific ureteritis,and to better understand this disease.Methods Three cases of primary non-specific ureteritis (1 man and 2 women)were reported.Their age was 28,38,68 years,respectively.One ease was found to have hydronephrosis on B-ultrasound at physical examination with no symptoms,and 2 had abdominal pain. The lesions were detected in lower part of the ureter in 2 cases,and in middle in I.The patients were diag- nosed preoperatively with combined use of MRU and ureteroscopy.Partial resection of the diseased part of the ureter was performed in all the cases.Results Pathological findings confirmed the diagnosis of non-spe- cific ureteritis.Microscopy showed necrotic tissue and inflammatory cell infiltration of ureteral mucosa,dilated and congested blood vessels,hypertrophic muscular layer,and proliferation of fibrous tissue.During the fol- low-up of 2 years,the patients had no symptoms and were free from hydronephrosis.Conclusions The eti- ology of primary non-specific ureteritis is unclear,and no typical symptoms can be found clinically.Combined use of MRU and ureteroscopy is helpful for definite diagnosis of the disease.Partial resection of the diseased part of the ureter has good results.
