2.Study of MAPKKK5 gene expression of brain tuissur in patients with cryptogenic refractory temporal lobe epilepsy

Objective To verify the variation of mitogen-activated protein kinase kinase kinase 5 (MAPKKK5) gene expression in patients with cryptogenic refractory temporal lobe epilepsy (TLE) and to evaluate the possible molecular pathogenesis of intractable TLE. Methods Reverse transcription polymerase chain reaction (RT-PCR) and Western-blot analysis were used to measure the expression alterations of MAPKKK5 mRNA as well as its protein product MAPKKK5 in temporal cortex samples from patients who had undergone temporal lobectomy for intractable epilepsy (n=10). Tissues from 10 subjects who did not have epilepsy served as controls. Objective: To establish the characteristic fingerprint of volatile oil of Pogostemon cablin (Blanco) Benth.Methods: The main components of 21 samples were determined by GC MS. Results: ? Patchoulene, ? Elemene, cis Caryophyllene, trans Caryophyllene, seychellene, ? Guaiene, ? Patchoulene, ? Guaiene, unidentification component A, patchoulic alcohol, pogostone were determined in all samples, and the relative content of 11 components add up to 80 percent. Compared with the samples produceds in Gaoyao and Hainan, Shipai samples were significantly differences.Conclusion: The retention time and relative content of 11 components bulidup the characteristic fingerprint of volatile oil of Pogostemon cablin (Blanco) Benth, it can be used for quality control and identification to Pogostemon cablin (Blanco) Benth.

5.Observation on therapeutic effect of acupuncture combined with medicine on mild cognition disorders in patients with post-stroke.

OBJECTIVETo explore the curative effect and safety of acupuncture for mild cognitive disorders after stroke.
METHODSRandomized controlled trial was adopted and one hundred cases conforming to criteria were divided into a combined acupuncture and medication group (group A) and a medication group (group B), 50 cases in each group. The basic treatment of cerebrovascular disease was applied in two groups. The group A treated was with acupuncture at Baihui (GV 20), Shenting (GV 24) and Qucha (BL 4) etc. and oral administration of Nimodipine. In group B, Nimodipine was taken orally. The treatment lasted for 3 months, the scores of Mini-Mental State Examination (MMSE), modified Hasegawa Dementia Scale (HDS-R) and Barthel Index (BI) were observed before and after treatment in two groups.
RESULTSThe total effective rate in group A was 66.7% (32/48), which was superior to 30.4% (14/46) in group B (P < 0.05). The scores of MMSE, HDS-R and BI after treatment were apparently improved as compared with those before treatment in two groups (all P < 0.01). The improvements in group A were superior to those in group B (all P < 0.05). There was no adverse event reported during the treatment in two groups.
CONCLUSIONAcupuncture can improve activity of daily living and has significant effect in treating mild cognition disorders after stroke without adverse reactions.

7.The feasibility study of ultrasound evaluation for brachial artery elasticity in patients with chronic renal failure

Objective To investigate the application of high-frequency ultrasound in pre-operative evaluation of brachial artery elasticity in chronic renal failure patients planning arteriovenous fistula formation.Methods A total of 65 patients with chronic renal failure undergoing radial artery-cephalic vein fistula (chronic renal failure group) and 40 healthy adults (control group) were selected.The elastic modulus (EM) of brachial artery was calculated by ultrasound.All subjects were divided into 5 subgroups according to age (≥50 years),hypertension,diabetes and chronic glomerulonephritis.Patients in subgroup 0 (28 cases),subgroup 1 (22 cases),subgroup 2 (24 cases),subgroup 3 (20 cases),subgroup 4 (11 cases) had 0,1,2,3 and 4 risk factors respectively.Results The levels of systolic blood pressure,pulse pressure,serum creatinine,and the EM of brachial artery in chronic renal failure group were higher than those in control group:(20.65 ± 1.32) kPa vs.(16.38 ± 1.08) kPa,(8.64 ±0.51) kPa vs.(7.12 ±0.35) kPa,(637.3 ± 116.5) μ mol/L vs.(84.7 ±31.1) μmol/L,(132.96 ± 11.78) kPa vs.(102.16 ± 10.10) kPa,and there were significant differences (P < 0.01 or < 0.05).The single factor correlation analysis showed that age in control group,serum creatinine in chronic renal failure group and systolic blood pressure in two groups were significantly positively correlated with the EM of brachial artery(r =0.636,0.548,0.703; P < 0.01).With the increase of risk factors,the EM of brachial artery increased gradually:(96.21 ±9.82),(104.45 ± 10.55),(114.87 ± 11.15),(127.26 ± 12.43),(143.54 ± 12.81) kPa in subgroup 0,subgroup 1,subgroup 2,subgroup 3,subgroup 4,and there were significant differences (F=17.43,P< 0.05).The EM of brachial artery increased significantly in subgroup 3 and 4 with multiple risk factors.Conclusions Brachial artery elasticity measured by high-frequency ultrasound can be used as an indicator of vascular function preoperative vascular assessment.Age,systolic blood pressure,pulse pressure and serum creatinine levels are the major risk factors for brachial artery elasticity.The coexistence of multiple risk factors of hypertension,diabetes or chronic nephritis could exacerbate the decrease of brachial artery elasticity.

? AIM: To observe the efficacy and safety of the phacoemulsification combined with goniosynechialysis and the phacoemulsification combined with trabeculectomy for angle-closure glaucoma ( ACG ) with different closure conditions accompanied with cataract.?METHODS:A total of 65 patients (70 eyes) with primary ACG accompanied with cataract were selected from those admitted in our hospital and were given phacoemulsification with goniosynechialysis ( group A, 30 patients with 33 eyes ) and the phacoemulsification with trabeculectomy ( group B, 35 patients with 37 eyes ) according to the conditions of the closed anterior angle, respectively. The two groups of patients were observed for preoperative and postoperative intraocular pressure ( IOP ) , vision, anterior chamber depth, and complications, and were followed up for 6. 5mo (6-8mo) on average.?RESULTS: Mean postoperative IOP in either group A or group B at 1mo after operation was of statistically significant difference, compared with mean preoperative IOP (P<0. 05). Postoperative 1-week vision in each group was of statistically significant difference, compared with preoperative vision (P<0. 05). Postoperative 1-month mean anterior chamber depth in each group was of statistically significant difference, compared with preoperative anterior chamber depth (P<0. 05). There were no significant differences between the two groups on postoperative 1-month mean anterior chamber depth ( P>0. 05) and postoperative complications (P>0. 05).?CONCLUSION: Clinical doctors can choose appropriate treatment according to different conditions of the closed anterior angle in patients with primary angle- closure glaucoma.
