1.Effect of Bushen Huatan Recipe on the Akt signal pathway in polycystic ovarian syndrome model rats with insulin resistance: an experimental research.

OBJECTIVETo observe the effect of Bushen Huatan Recipe (BHR) on the Akt signal pathway in polycystic ovarian syndrome (PCOS) model rats with insulin resistance (IR).
METHODSFifty Wistar female PCOS rats were randomly and equally divided into 5 groups, i.e., the control group, the model group, the low dose BHR group (5.406 g/kg), the medium dose BHR group (10.812 g/kg), and the high dose BHR group (21.624 g/kg), 10 in each group. Levels of fasting blood glucose (FBG) and insulin were detected to calculate homeostasis model assessment of insulin resistance (HOMA-IR). The expression of insulin receptor substrate 1 (IRS-1), glycogen synthetase kinase-3beta (GSK-3beta), glucose transporter 4 (GLUT-4), and peroxisome proliferator activated receptor (PPAR-gamma) mRNA were detected by RT-PCR. The expression of insulin signal transduction molecular kinase B (Akt) was detected by Western blot.
RESULTSCompared with the control group, HOMA-IR and the mRNA expression of PPAR-gamma mRNA significantly increased, the mRNA expression of GSK-3beta, GLUT-4, and IRS-1, protein expression of Akt and p-Akt significantly decreased in the model group (P < 0.05). Compared with the model group, HOMA-IR significantly decreased, the mRNA expression of GSK-3beta, GLUT-4, IRS-1, and Akt protein significantly increased in the high dose BHR group (P < 0.05, P < 0.01). The mRNA expression of p-Akt protein increased more obviously (P < 0.05, P < 0.01). mRNA expression of GSK- 3beta and GLUT-4 significantly increased (P < 0.05), while the mRNA expression of PPAR-gamma significantly decreased in the low and middle BHR groups (P < 0.05). The expression of p-Akt significantly increased in the low dose BHR group (P < 0.05).
CONCLUSIONSIR and abnormal insulin signal pathway existed in PCOS model rats. BHR could improve IR of PCOS rats, which was correlated with regulating protein expression of insulin signal transduction molecules.

2.Advancement in Intestinal Barrier Dysfunction of Severe Acute Pancreatitis

Objective To summarize the recent progress in pathogenetic,diagnostic and therapeutic researches on the intestinal barrier dysfunction(IBD) of severe acute pancreatitis(SAP).Methods The advancement of IBD in SAP,which was published recently at home and abroad,was collected and reviewed.Results The pathogenesis of IBD in patients with SAP was complex.Ischemia-reperfusion injury,endotoxin,inflammatory mediators and gastrointestinal hormone played an important role in the process of IBD.There were many ways to detect IBD,and the ratio of lactulose and mannitol,plasma diamine oxidase were relatively ideal markers.Medical therapies,such as treatment of SAP and maintaining the perfusion of intestines,were essential to cure IBD.On this basis,the propulsives,nutritional support and traditional Chinese drugs should be administered reasonably.Conclusions IBD is a sophisticated process of pathophysiology.In recent years,abundant of animal experiments and clinical researches have provided new clue for prevention and cure of IBD,but further researches are still needed on the mechanism of the cells and molecules implicated.
3.Analysis on the chapter of rehabilitation definition in the fourth edition of rehabilitation medicine textbook

The definition of rehabilitation should reflect the connotation and extension of rehabilitation medicine system,moreover,special attention should be paid to the process of translation,applications and promotion due to its derivation from Latin.The fourth edition of rehabilitation medicine textbook which published by People's Medical Publishing House described the definition of rehabilitation inaccurately,although integrated the foreign authority definitions.The definition of ‘ re-learning’has been described by the textbook without considering logical sequence,which caused difficulty in explaining and comprehending between teachers and students in class.Besides,application of another term of rehabilitation medicine in the textbook is imprudence.
4.Effect of Yuxingchangzhi Decoction on level of norepinephrine and 5-hydroxyptamin in serum of patients with post-stroke depression

ObjectiveTo evaluate the effect of Yuxingchangzhi Decoction on level of norepinephrine (NE) and 5-hydroxyptamin (5-HT) in serum of patients with post-stroke depression. Methods76 patients with post-stroke depression were treated with Yuxingchangzhi Decoction or placebo for 4 weeks. Before and after treatment, all the patients were evaluated with the Hamilton Rating Scale for Depression (HAMD),while the level of NE and 5-HT in serum of patients were measured.ResultsThe total score of HAMD of the patients of Yuxingchangzhi Decoction group was significantly different before and after treatment (P<0.01),while that of the placebo group was not significantly different (P>0.05). The NE and 5-HT level in serum of patients of the Yuxingchangzhi Decoction group were significantly different before and after treatment (P<0.01),but that of the placebo group was not significantly different (P>0.05). ConclusionYuxingchangzhi decoction can raise NE,5-HT level in patients with post-stroke depression and alleviate symptoms of depression significantly.
5.Protective effect of ATP on skeletal muscle satellite cells damaged by H2O 2.

This study investigated the protective effect of ATP on skeletal muscle satellite cells damaged by H2O2 in neonatal rats and the possible mechanism. The skeletal muscle satellite cells were randomly divided into four groups: normal group, model group (cells treated with 0.1 mmol/L H2O2 for 50 s), protection group (cells treated with 16, 8, 4, 2, 1, 0.5, or 0.25 mmol/L ATP for 24 h, and then with 0.1 mmol/L H2O2 for 50 s), proliferation group (cells treated with 16, 8, 4, 2, 1, 0.5, or 0.25 mmol/L ATP for 24 h). MTT assay, FITC+PI+DAPI fluorescent staining, Giemsa staining and immunofluorescence were performed to examine cell viability and apoptosis, and apoptosis-related proteins. The results showed that the survival rate of skeletal muscle satellite cells was decreased and the apoptosis rate was increased after H2O2 treatment (P<0.01). Different doses of ATP had different effects on skeletal muscle satellite cells damaged by H2O2: the survival rate of muscle satellite cells treated with ATP at 4, 2, or 1 mmol/L was increased. The protective effect was most profound on cells treated with 2 mmol/L ATP. Immunofluorescence showed that ATP could increase the number of Bcl-2-positive cells (P<0.01) and decrease the number of the Bax-positive cells (P<0.01). It was concluded that ATP could protect skeletal muscle satellite cells against H2O2 damage in neonatal rats, which may be attributed to the up-regulation of the expression of Bcl-2 and down-regulation of Bax, resulting in the suppression of apoptosis.

7.Relationship study of the expressions of Slit2 and Nogo-A in rats with cerebral hemorrhage

Objective To investigate the expressions of Slit2 and Nogo-A in the ipsilateral hippocampus after cerebral hemorrhage in rats.Methods A total of 64 adult Sprague-Dawley (SD) rats was randomly divided into control and model groups.Collagenase Ⅶ was used to induce cerebral hemorrhage model.Immunohistochemistry was used to detect the expressions of Slit2 and Nogo-A in hippocampus at the cerebral hemorrhage side at the time points (24 h,7 d,14 d,and 21 d).Results Compared to the control group,the expressions of Slit2 and Nogo-A were significantly enhanced in model group (P <0.01),with the highest level at the 7 d.Conclutions Cerebral hemorrhage can significantly enhance expressions of Slit2 and Nogo-A with a positive correlation of Slit2 and Nogo-A,which might have an important effect on the recovery of brain injury.
8.The planning target volume margins detected by cone-beam CT in head and neck cancer patients treated by image-guided intensity modulated radiotherapy

Objective To determine the planning target volume margins of head and neck cancers treated by image guided radiotherapy (IGRT).Methods 464 sets cone beam computed tomography (CBCT) images before setup correction and 126 sets CBCT images after correction were obtained from 51 head and neck cancer patients treated by IGRT in our department.The systematic and random errors were evaluated by either online or offline correction through registering the CBCT images to the planning CT.The data was divided into 3 groups according to the online correction times.Results The isocenter shift were 0.37 mm±2.37 mm, -0.43 mm±2.30 mm and 0.47 mm±2.65 mm in right-left (RL), anterior-posterior (AP) and superior-inferior (SI) directions respectively before correction, and it reduced to 0.08 mm±0.68 mm, -0.03 mm±0.74 mm and 0.03 mm±0.80 mm when evaluated by 126 sets corrected CBCT images.The planning target volume (PTV) margin from clinical target volume (CTV) before correction were:6.41 mm,6.15 mm and 7.10 mm based on two parameter model, and it reduced to 1.78 mm,1.80 mm and 1.97 mm after correction.The PTV margins were 3.8 mm,3.8 mm,4.0 mm;4.0 mm,4.0 mm,5.0 mm and 5.4 mm,5.2 mm,6.1 mm in RL, AP and SI respectively when online-correction times were more than 15 times, 11-15 times,5-10 times.Conclusions CBCT-based on online correction reduce the PTV margin for head and neck cancers treated by IGRT and ensure more precise dose delivery and less normal tissue complications.
9.Determination of energy expenditure in mechanically ventilated patients

Objective To investigate the energy expenditure of mechanically ventilated patients,compare the measured energy expenditure (MREE) with the energy expenditure expected from the Harris-Benedict equation adjusted with correction factors (PREE). Methods Twenty-four critically ill adult patients who were mechanically ventilated in the intensive care unit were enrolled in this study. Data during the 72 hours of mechanical ventilation were collected for computation of severity of illness. Resting energy expenditures were derived at 72hours after mechanical ventilation by indirect calorimetry. Predicted basal energy expenditure was obtained at the same time using the Harris-Benedict equation and predicted resting energy expenditure was calculated using the Harris-Benedict value adjusted with correction factors for illness. Results The Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ)scores and Marshall scores were 14 ± 5 and 6 ± 3, respectively. MREE and PREE were (6 793.64 ± 1 197.15) and (8 041.02 ± 1 971.54) kJ/day, respectively. There was no correlation between MREE and PREE (r2 = 0. 28, P = 0. 07), and the difference between MREE and PREE was statistically significant (t = 7.62, P = 0.04). No statistically significant correlations were observed between both MREE or PREE and APACHE Ⅱ score or Marshall score (r2 = 0. 14, P = 0. 08; r2= 0. 08, P = 0. 63; r2 = 0. 05, P =0. 65; r2 = 0.03, P = 0. 87, respectively). Conclusions In mechanically ventilated patients, the energy expenditure is not correlated with the severity of illness. The Harris-Benedict prediction modified with correction factors for severity of illness systematically overestimates the total energy expenditure.
10.Analysis on literature regarding acupuncture-moxibustion with high impact factor journal of SCI during the recent 5 years.

The status of acupuncture-moxibustion is more and more recognized by mainstream medicine in the world in recent years, and literature regarding acupuncture-moxibustion with high impact factor (IF) published in the worldwide mainstream medicine journals is also gradually growing by years. To understand the situation of related literature, literature regarding acupuncture-moxibustion with IF of more than 10 in Science Citation Index (SCI) during the recent 5 years was retrieved. The number, the types, the diseases involved, the publishing states of the acquired articles and the source, the citation, the IF of the publishing journals were analyzed and summarized. Additionally, some of the research foci, the new research tendencies and the deficiencies of research were discussed. The thoughts and suggestions are expected to be provided for further research of acupuncture.
