1.Clinical Significance of Fecal Smear Examination on Diagnosing Intestinal Flora Imbalance

Objective To study the clinical significance of fecal smear examination on diagnosing intestinal flora imbalance in infantile diarrhea.Methods A sterile cotton swab was used to spread a layer of fresh feces quantum satis from a sterile container on a clean slide;the smear was fixed and stained with Gram′s methods after it was air-dried,then the specimen was observed with a microscopy(field lens 100 ? eye lens 10) and recorded.Results In the acute diarrhea group(40 cases),the distribution feature of 3 floras on the fecal smears:≥50% Gram-negative bacilli in 6 cases(15%),≥50% Gram-positive cocci in 30 cases(75%) and ≥68% Gram-positive bacilli in 4 cases(10%).In the delayed and chronic diarrhea group(62 cases),the distribution feature of 3 floras on the fecal smears:≥50% Gram-negative bacilli in 7 cases(11.29%),≥50% Gram-positive cocci in 44 cases(70.97%) and ≥68% Gram-positive bacilli in 6 cases(9.68%).In the normal control group(32 cases),the distribution feature of 3 floras on the fecal smears:≥50% Gram-negative bacilli in 1 case(3.13%),≥50% Gram-positive cocci in 1 case(3.13%),and ≥68% Gram-positive bacilli in 17 cases(53.13%).For the distribution of 3 floras in the 3 groups,chi-squared test was performed,and the results showed that the difference was significant in Gram-positive cocci(?~2=47.76 P0.05).Conclusions Acute,delayed or chronic diarrhea can lead to flora imbalance.In order to timely and rapidly know the flora imbalance in children with diarrhea,clinically the simple,easily operated and practical smear staining method shall be widely applied.
2.Changes of biological behavioral of E. coli K1 after ppk1 gene deletion.

OBJECTIVETo study the changes in biological behaviors of meningitis E. coli K1 strain E44 after deletion of polyphosphate kinase 1 (ppk1) gene and explore the role of ppk1 in the pathogenesis of E. coli K1-induced meningitis.
METHODSThe wild-type strain E. coli K1 and ppk1 deletion mutant were exposed to heat at 56 degrees celsius; for 6 min, and their survival rates were determined. The adhesion and invasion of the bacteria to human brain microvascular endothelial cells (HBMECs) were observed using electron microscopy and quantitative tests. HBMECs were co-incubated with wild-type strain or ppk1 deletion mutant, and the cytoskeleton rearrangement was observed under laser scanning confocal microscope.
RESULTSThe survival rate of the ppk1 deletion mutant was significantly lower than that of the wild-type strain after heat exposure. The ppk1 deletion mutant also showed lowered cell adhesion and invasion abilities and weakened ability to induce cytoskeleton rearrangement in HBMECs.
CONCLUSIONSppk1 gene is important for E.coli K1 for heat resistance, cell adhesion and invasion, and for inducing cytoskeletal rearrangement in HBMECs.

4.Hhigh-frequency ultrasound research on the normal adult sciatic nerve.

OBJECTIVETo explore the application value of the high-frequency ultrasound examination of the normal adult sciatic nerve.
METHODSThe high frequency ultrasound was used to examine 60 cases ,a total of 120 normal adult sciatic nerve (28 males and 32 females ranging in age from 19 to 38 years old, averaged 23.8 years old). The age between men and women, height and weight showed no significant difference. The cross-sectional scanning and longitudinal scanning, if necessary, Panoramic Imaging were conducted. The cross section area of sciatic nerve was measured at the hip line, the middle of the thigh, the edge of the popliteal fossa, and the relationship betwen the sciatic nerve and surrounding muscle group was observed in each cross-section. Probe was rotated 90 degree to display the long axis of sciatic nerve, to get the longitudinal section along the long axis from top to bottom. So the sciatic nerve in the vertical axis and the surrouunding muscles was displayed by above method combined with panoramic imaging. The data of coss-sectional area of sciatic nerve were obtained at the central of hip line, the middle of the thigh, the edge of the popliteal fossa, and then were divided into different groups by gender and sides to do statistical analysis.
RESULTSThe applicatic nerve in thigh can be clearly displayed by high-frequency ultrasound in this group of 60 people with 120 nerves, but low display rate of sciatic nerve in hip, approximatley 31.4% (21/66). The longitudinal section of the sciatic nerve displayed as the banded isoechoic or slightly hyperechoic, parallel partial hyperechoic line could be seen in it; in the cross-section, it showed a clear edge of the sieve-like isoechoic or slightly hyperechnie, which could be oval ,trapezoidal, triangular and irregular in shape; the sciatic nerve bad visible passive movement and shape change when muscle contracted. There were statistically significant differences of cross-sectional area of sciatic nerves at the hip line ,the middle of thigh and edge of the popliteal fossa between 56 sciatic nerves in 28 males and 64 sciatic nerves in 32 females (P< 0.05); but there were no significant differences between left and right sides (P>0.05).
CONCLUSIONHigh frequency ultrasound can be used as the first choice of the sciatic nerve when an imaging method preferred.

5.Expression of Toll-like receptors in mononuclear cells from children with acute rotavirus diarrhea.

OBJECTIVERotavirus is the single most common cause of severe dehydrating diarrhea in young children worldwide, but the pathogenesis and immunity against this disease are not completely understood. A prospective study was conducted to assess gene expression of toll-like receptors (TLR) in children with acute rotavirus diarrhea.
METHODSSeventy-five children with acute rotavirus diarrhea and 38 control children were enrolled in this study from Sep. 2004 to Jan. 2005. All the 75 patients had detailed records of clinical characteristics. Rotavirus antigen was detected by ELISA from stools. Peripheral blood mononuclear cells (PBMC) were separated by Ficoll reagent and RNA was extracted by Trizol. The levels of mRNA for five TLRs in PBMC were examined by fluorescent quantitative RT-PCR.
RESULTSPatients with acute rotavirus infection had elevated mean levels of TLR 2, 3, 4, 7, 8 mRNA expressions in PBMC within 3 days since onset of the disease, P less than 0.05. But only TLR 2, 3, 8 mRNA levels remained increased in patients within 7 or 14 days since onset (P less than 0.05). Mean levels of mRNA for TLR 4 in PBMC was higher in patients with more severe diarrhea including longer duration of diarrhea, more episodes of diarrhea per day and higher severity scores (P less than 0.05).
CONCLUSIONManifold TLR may play roles in the start-up and regulation of immune responses in children with acute rotavirus diarrhea. These findings will be helpful to further recognize immune response in Chinese children with rotavirus diarrhea and, consequently, may provide directions and insights that could prove critical to the prevention or treatment of this important disease.

6.Reliability and Validity of Chinese Version of Illness Cognition Questionnaire in Women with Breast Cancer

Objective To test the reliability and validity of Chinese version of Illness Cognition Questionnaire(ICQ)in women with breast cancer. Methods From October,2016 to May,2017,297 women with breast cancer were investigated with the Chinese version of the ICQ and Brief Illness Perception Questionnaire(B-IPQ).The reliability and validity of the Chinese version of ICQ were tested. Results The Cronbach's α of three subscales of ICQ(helplessness,acceptance and perceive benefits)were 0.855,0.857, 0.878,and the test-retest reliability of three subscales were 0.946,0.963,0.901(P<0.001).A confirmatory factor analysis was tested,χ2/df=2.500,SRMR=0.063,CFI=0.920.The factor load capacity of each subscales was 0.37 to 0.84 and the correlation of helplessness,acceptance and perceive benefits was-0.49 to 0.81(P<0.001).The correlation coefficient of criterion-related validity of the helplessness subscale of ICP with B-IPQ was 0.727(P<0.001). Conclusion The Chinese version of ICQ has good reliability and validity,which is a valuable research tool in clinical studies of psychological cognition of patients with breast cancer.
7.Expression of plasma miRNA-21 and miRNA-200b in epithelial ovarian cancer and its clinical significance

Objective:To investigate the expression and clinical significance of plasma microRNA-21 (miRNA-21) and microRNA-200b (miRNA-200b) in epithelial ovarian cancer (EOC).Methods:The levels of plasma miRNA-200b,miRNA-21 and CA125 were detected by RT-PCR in 162 patients with EOC,120 patients with benign epithelial ovarian tumors(benign group) and 108 healthy women(control group),analyze the relation between miRNA-200b and miRNA-21 expression and clinicopathological features of EOC.The sensitivity and specificity of miRNA-200b,miRNA-21 and CA125 to EOC diagnosis were evaluated by ROC curve,and the re-lationship between three indexes and EOC was analyzed by multivariate Logistic regression model.Correlation analysis of plasma miRNA-200b and miRNA-21,CA125 in patients with EOC by Pearson correlation.Results:The levels of plasma miRNA-200b,miRNA-21 and CA125 in EOC group were significantly higher than those in benign group and control group[miRNA-200b(2-ΔΔCt):3.52±1.03 vs 1.26±0.37 and 1.15±0.34;miRNA-21(2-ΔΔCt):2.32±0.45 vs 1.18±0.32 and 1.04±0.28;CA125(U/ml):78.64±30.57 vs 26.27±11.36 and 21.53±9.45,all P<0.01].Plasma miRNA-200b,miRNA-21,CA125 and three combined diagnosis EOC of AUC (95% CI) were 0.896(0.834-0.958),0.792(0.731-0.847),0.908(0.841-0.973),0.947(0.883-0.995),the optimal cut-off values were 2.08,1.46,52.84 U/ml.Logistic regression analysis showed that elevated plasma levels of miRNA-200b,miRNA-21 and CA125 were independent risk factors for EOC[OR(95% CI)= 2.518(1.563-3.547),OR(95% CI)= 1.724(1.103-2.528),OR (95% CI)=2.316(1.347-3.419)].The correlation between plasma miRNA-200b and CA125 in patients with EOC was the best(r=0.702,P<0.01).Conclusion:Plasma miRNA-200b and miRNA-21 can be used as molecular markers for the early diagnosis of EOC, and their diagnostic efficacy is comparable to that of CA125.The combined use of the three methods is expected to improve the accuracy of early diagnosis of EOC.
8.Expression of Atrial Natriuretic Peptide mRNA during 2 Weeks of Head-down Suspension in Rats.

The aim of this study was to determine the adaptation of atrial natriuretic peptide(ANP) to simulated weightlessness. The level of immunoreactive plasma ANP level and the expression of right atrial ANP mRNA were assessed in male Sparague-Dawley rats during 2 weeks of head-down suspension(HDS) The rats were placed in a -45 degrees anti-orthostatic position. The plasma ANP level was investigated by radioimmunoassay and ANP mRNA was expressed by Northern blot analysis. The changes in daily water intake, body weight and arterial hematocrit did not show the statistical significances during HDS. The plasma ANP level slightly increased after 1 day of HDS, the response was transient and then decreased below the control levels. The expression of the ANP mRNA increased after 1 day, the peak value occurred at 3 days of HDS and thereafter ANP mRNA showed the similar patterns compared to the changes of plasma level. On the basis of these findings, we conclude that the initial adaptation of ANP to increased control blood volume in rats occurs between 3 and 7 days of HDS, and the ANP acts an important role during regulatory process to central hypervolemia at both synthetic and secretory levels within the early stage of HDS.

9.A study on K469E polymorphism of ICAM1 gene and ICAM1 plasma level in patients with coronary heart disease.

OBJECTIVETo study the linkage between K469E polymorphism of intercellular adhesion molecule 1(ICAM1) gene with ICAM1 plasma level and coronary heart disease (CHD) in Han population of China.
METHODSOne hundred and sixty-four controls without CHD and 160 patients with CHD were enrolled in our study. By nested PCR with allele-specific oligonucleotide primers, all patients and controls were genotyped for the ICAM1 polymorphism. And the ICAM1 plasma level was measured by ELISA.
RESULTSIn the patients with CHD, both K allele frequency and the plasma level of ICAM1 were higher than those in control (P<0.05). The individual with K allele had higher plasma level of ICAM1 than that without K allele (344.34+/-128.59 microg/L vs 303.54+/-108.74 microg/L, P=0.008). K allele enhanced the risk of CHD (P<0.01, OR=2.158, 95%CI: 1.250-3.727). There was the K allele cooperation with smoking in influencing the risk of CHD.
CONCLUSIONThere is the polymorphism of ICAM1 K469E gene in Han population of China, and the K allele may be a genetic factor influencing the risk of CHD.

10.Role of CD47 in hematologic malignancies.

CD47 is a ubiquitously expressed transmembrane glycoprotein on surface of many cells. Through its interaction with integrin, signal regulatory protein alpha (SIRPα) and thrombin sensitive protein-1 (TSP-1), it plays important roles in various immunological processes including inflammatory response, immune response and tumor immunity. Recently, it has been found that CD47 interacts with SIRPα expressed on phagocytic cells, which transfers a negative signal when being activated. By the mechanisms described above, CD47-SIRPα signal complex is involved in the pathogenesis of hematological diseases and might provide some informations for the therapy of patients. This review focuses on the structure and immunoregulatory functions of CD47, the mechanism of CD47 in tumor therapy, the CD47 and hematologic malignancies including acute leukemia, B-cell lymphoma and multiple myeloma, as well as CD47 and hematopoietic stem cell transplantation.
