Objective To evaluate the anatomic morphology of submandibular fossa. Methods Cone-beam computed tomography (CBCT) was used to analyze the anatomic morphology of submandibular fossa. Implants of various dimensions were virtually placed into the mandibular 1st and 2nd molar region. Spatial relationships among submandibular fossa , mandibular canal and implant were analyzed. Results The depth of the submandibular fossa was (1.0 ± 0.61)mm in the 1st molar region and (1.5 ± 0.61)mm in the 2nd molar region. There were no significant statistical difference in genders, sides, and tooth loss. The length of the implants was (19.7 ± 3.75)mm (d = 4 mm) and (18.6 ± 3.73)mm (d = 5 mm) in the 1st molar region and (16.2 ± 3.46) mm (d = 4 mm) and (15.2 ± 3.09)mm (d = 5 mm) in the 2nd molar region in patiens with teeth and lingual perforation; the length of the implants was (17.1 ± 3.77)mm (d = 4 mm) and (15.6 ± 3.85)mm (d = 5 mm), and (14.2 ± 4.05)mm (d = 4 mm) and (13.2 ± 4.27)mm (d = 5 mm) in patients with tooth loss, respectively. Conclusions Determination of spatial relationships among submandibular fossa , mandibular canal and implants by using CBCT has important significance in guiding dental implant surgery.

AIM:To observe the clinical effects of 23-gauge (23G) transconjunctival sutureless vitrectomy for idiopathic macular hole.
METHODS: In this retrospective study, 28 eyes of 28 consecutive patients who underwent 23 - gauge transconjunctival sutureless vitrectomy for idiopathic macular hole between January 2013 and October 2013 in our hospital were evaluated. The follow-up time was 3-12mo. The operation effects were analyzed.
RESULTS:The macular hole was closed in 27 eyes of 28 eyes which underwent 23G transconjunctival sutureless vitrectomy and not closed in 1 eye after surgery. Best-corrected visual acuity at postoperative 1, 3mo was significantly improved compared to pre-operation (χ2=8-65, P=0. 003;χ2=10. 33, P=0. 001). The macular hole was closed as shown by OCT. Intraoperative incision was sutured in 5 cases ( 18%) . There was no statistically significant difference in intraocular pressure between pre-operation and post - operation. No post - operative complications such as endophthalmitis, retinal detachment, vitreous hemorrhage came up.
CONCLUSION: 23G transconjunctival sutureless vitrectomy is observed to be safe and effective technique in the treatment of macular hole. It is therefore our preferred system for straightforward macular surgery.

Objective To investigate the relationship between C-peptide and artherosclerosis in lower ex-tremity in patients with type 2 diabetes. Methods Two hundred and fourteen type 2 diabetic patients were included in this retrospective study. They were divided into two groups:type 2 diabetes with artherosclerosis in lower extremi-ty in 123 cases and type 2 diabetes without artherosclerosis in lower extremity in 91 cases. Their medical history and the laboratory data were collected such as fasting serum C-peptide levels and postprandial serum C-peptide levels of 1 hour, 2 hours and 3 hours, glycated hemoglobin (HbA1c) and so on. Results Compared with artherosclerosis group in lower extremity, the age, duration of diabetes, total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), glycated hemoglobin and smoking rate were significantly higher in type 2 diabetes without artherosclero-sis in lower extremity (P < 0.05). However, the high density lipoprotein cholesterol (HDL-C) was much lower in the last group (P<0.05). There were no significant differences in the other indicators between the two groups (P>0.05). Logistic regression analysis showed that fasting and postprandial (1 hour, 2 hours and 3 hours)serum C-pep-tide levels were all negatively related to artherosclerosis in lower extremity (OR 0.524,P = 0; OR 0.440, P = 0;OR 0.688, P=0;OR 0.795, P=0). Conclusions Serum C-peptide level may be an independent associated fac-tor with artherosclerosis in lower extremity in type 2 diabetes.

Objective To assess the capability of monochromatic energy images of Gemstone spectral imaging(GSI) in reducing artifacts caused by metallic implants. Methods Twelve subjects with metallic implants underwent GSI (Discovery CT750 HD, GE Healthcare, Milwaukee ). The metallic orthopedic implants included 3 patients of dentures, 2 patients of cervical spinal vertebraplasty, one clavicle fracture fixation, one lumbar spinal vertehraplasty, 3 patients of artificial femoral head, one iliac fracture fixation and one tibial fracture fixation. GSI was performed by using a single source ultra-fast dual energy X-ray switching (80 kVp and 140 kVp). Following GSI scanning, thin slice images were reconstructed into 1.25 mm slice thickness. The monochromatic energy images were set to the same window width and level (window width 1500 HU,window level 500 HU). The artifact indexes (AI) at different kiloelectronvolts (keV) images were measured and compared. 3D reconstruction was performed using images with minimal AI. Result The artifacts index on monochromatic energy images varied with the change of keV. Of the images from 12 subjects, the maximal AI ranged between 145-225 at 40 keV, and minimal AI ranged between 15-90 at the 95-140 keV. The artifacts are clearly visible on polychromatic energy images and the artifacts are reduced markedly on the monochromatic energy images with minimal AI. Conclusion The artifacts caused by metallic implants can be reduced significantly by GSI with high keV monochromatic energy images.

BACKGROUND: There are plentful studies about bioreactor of tissue engineering of blood vessel, tendon, cartilage, heart valve, trachea, bladder and stern cell. OBJECTIVE: To construct small-sized tissue-engineerad blood vessels with human bone marrow stromal stem cells (hBMSCs) in improved bioreactor system.DESIGN, TIME AND SETTING: The single sample observation stuay was performed at the School of Mechanical and Power Engineering, East China University of Science&Technology, and Shanghai Tissue Engineering Research & Development Center from June 2005 to March 2008.MATERIALS: Vessel bioreactor was self-made by East China University of Science&Technology. hBMSCs were harvested from healthy volunteers. METHODS: A set of support bracket constructing tissue engineered blood vessels with the diameter of 2 mm was designed with the application of Finite Element Methods as an analysis method analyzing support bracket of tissue engineered small-sized blood vessel. Primary hBMSCs were first Collected and further cultivated exvivo. The third passage cultured cells were then seeded on the polyglycolic acid (PGA) to fabricate the cell-scaffold composite. Subsequently, this composite was subjected to dynamical culture in the blood vessel bioreactor. After cultured for 4 weeks, the composite was removed from the bioreactor.MAIN OUTCOME MEASURES: The following mentioned references were measured: composite growth; other correlation detection of the hBMSCs-PGA composite, RESULTS: Gross observation and scanning electron microscope were used at4 weeks after hBMSCs-PGA composite culture. It was observed that the tissue-engineered blood vessel had a bright color and certain elasticity. The blood vessel could rebound to its odginal shape after repeated press by the forceps. The secreted collagen matrix arrayed orderly around the cells and smooth muscle elastic acttn could also be detected in the formed tissues using immunohistochemistry. CONCLUSION: The in vivo mechanics conditions of blood vessels can be simulated using the current blood vessel bioreactor system. Using hBMSCs, the construction of tissue engineered small-sized blood vessels can be successfully achieved.
