1.Clinical analysis of posterior capsular rupture in small incision non -phacoemulsification cataract surgery

AIM:To analyze the cause of posterior capsular rupture in small incision non - phacoemulsification cataract surgery and investigate the treatment methods.
METHODS: The clinical data of 108 cases ( 121 eyes ) undergone small incision non - phacoemulsification cataract surgery combined with intraocular lenses ( IOL) implantation were retrospectively analyzed. The causes of posterior capsular rupture and treatment of 6 patients (6 eyes) after surgery were analyzed and summarized.
RESULTS: Six cases ( 6 eyes, 4. 96%) occurred posterior capsular rupture during the operation, of which 2 cases ( 2 eyes ) underwent posterior chamber IOL implantation intraoperative, 2 cases (2 eyes) receivedIIstage IOL implantation in the ciliary sulcus at 1wk postoperative, 2 cases ( 2 eyes ) were given anterior chamber IOL implantation. One case ( 1 eye ) whose vision acuity <0. 1 was macular degeneration; One case ( 1 eye ) whose vision acuity = 0. 3 was diabetic retinopathy;0. 4~0. 6 in 2 cases (2 eyes), 0. 6~0. 8 in 2 cases (2 eyes) after 1 ~3mo postoperatively. No severe syndrome was found after surgery.
CONCLUSION:Posterior capsular rupture which occurs in different stages of the operation is the commonest intraoperative complication in small lincision non -phacoemulsification cataract surgery. With improving operative techniques, and continuously accumulating experience, incidence of posterior capsular rupture can be effectively reduced. Even posterior capsular rupture occurs intraoperative, patients can still get a good outcome after timely and proper treatment.

Nuclear magnetic resonance (NMR) spectroscopy can be used to both identify and quantify chemicals from complex mixtures. Over the last several decades, significant technical and experimental advances have made quantitative nuclear magnetic resonance (qNMR) a valuable analytical tool for quantitative measurements of a wide variety of samples. This particular approach is now being exploited to characterize the metabolomes of many different biological samples and is called quantitative metabolomics or targeted metabolic profiling. In this review, some of the strengths, limitations of NMR-based quantitative metabolomics will be discussed as well as the practical considerations necessary for acquisition with an emphasis on their use for bioanalysis. Recent examples of the application of this particular approach to metabolomics studies will be also presented.

4.Early results in 26 patients with long donor ischemia time in orthotopic heart transplantation

Objective To introduce the donor heart preservation techniques and study the clini- cal results in 26 patients undergoing orthotopic heart transplantations with long donor ischemia time (DIT) in our center.Methods From Sept.2004 to Aug.2006,26 patients underwent orthotopic heart transplantation at our center with DIT longer than 360 min.Crystalloid cardioplegia was infused through aortic root after aorta was cross clamped.UW solution was used when donor heart was removed.All patients underwent bicaval anastomotic cardiac transplantation.NO was inhaled through ventilator during and after operation in patients with pulmonary hypertension.Results DIT was 360- 560 (mean 401?43) min.There was no operative death in this group.Echocardiography and X-ray examinations in one month were normal.One case of abnormal coronary orifice was subjected to re- anastomosis of aorta.One case of renal failure post-operation was subjected to blood dialysis.Conclu- sion With proper donor heart procurement and preservation,recipient selection,anastomotic technique,efficient postoperative surveillance and pulmonary hypertension treatment,transplanted patients with long DIT can obtain the satisfactory short-term outcome.
5.The effects of Tianqijiangtang capsule combined with Metformin on the levels of interleukin-6,tumor necrosis factorαand C-reactive protein in patients with type 2 diabetes mellitus

Objective To explore the effects of Tianqijiangtang capsule combined with Metformin on the levels of IL‐6 ,TNF‐α and C‐RP protein in patients with T2DM. Methods 80 T2DM were randomized into control group (treatment with Metformin ,n= 40) and treatment group (treatment with Tianqijiangtang in combination with Metformin ,n=40). The changes of levels of IL‐6 ,TNF‐α and C‐RP were observed before and after the treatment. Results Compared to the pretreatment in the same group and after treatment in control group ,IL‐6 ,TNF‐αand C‐RP of treatment group were reduced significantly [(6.79 ± 0.60) ,(7.15 ± 0.62) vs (5.08 ± 0.43)pg/ml;(8.94 ± 0.55) ,(8.61 ± 0.66) vs (5.18 ± 0.45) pg/ml;(6.79 ± 0.60) ,(6.58 ± 0.59) vs (5.56 ± 0.30) mg/L ,P<0.01]. Conclusion Tianqijiangtang capsule combined with Metformin can decrease the levels of IL‐6 ,TNF‐αand C‐RP in T2DM patients.
6.Effects of imatinib mesylate on rat hepatic fibrogenesis and the expression of transforming growth factor-?1

Objective To investigate the anti-fibrogenesis property of imatinib mesylate in a rat model of liver fibrosis induced by carbon tetrachloride/olive oil and its effect on the expression of trans- forming growth factor(TGF)-?1.Methods Rat liver fibrosis was induced by intraperitoneal administra- tion of carbon tetrachloride and olive oil mixture twice a week for eight weeks.Imatinib mesylate was given 20 mg/kg daily by oral lavage.The control rats received saline by oral iavage.Liver collagen depo- sition was evaluated by immunohistochemistry with Masson staining.The activation of hepatic stellate cells was detemined by the immunohistoehemistry staining of?-smooth muscle actin.The mRNA expres- sions of TGF-?1,c-Abl and TIMP-1 were measured by RT-PCR.While protein expressions of TGF-?1, phosphorylated platelet-derived growth factor receptor and c-Abl were detected by Western blot and im- munohistochemical staining.Hepatic hydroxyproline content was also quantified.Results The collagen deposition[(16.23?1.01)%vs(25.61?0.92)%]and the number of activated HSCs(10.52?1.33vs 13.10?1.21)were reduced in the imatinib mesylate treatment group compared with the control group by 35% and 20%,respectively(P
7.Comparison of costs of combined intravenous and inhalation general anesthesia and total intravenous anesthesia

ObjectiveTo compare costs of combined intravenous and inhalation anesthesia (CIIA) and total intravenous anesthesia (TIA) when lower limbs orthopedics (LLO) performed on cerebral palsy children (CPC).MethodsTo analyze data of 49 CPC who received LLO under CIIA and TIA,and compare costs of two anesthetic styles.ResultsTIA had a significantly lower cost than CIIA (P<0.001),and showed a lower incidence of post-operation nausea and vomiting.ConclusionTIA can decrease cost of anesthesia and post-operation nursing compared with CIIA.
