OBJECTIVETo investigate the clinical application of pedicled superficial circumflex iliac artery (SCIA) flaps in Burns and Plastic Surgery.
METHODS19 cases with skin and soft tissue defects were repaired with the Superficial circumflex iliac artery flaps. The defects located in the hand and forearm, the lower abdomen and perineal area, the radicle area of thigh, etc. Before the SCIA flaps were transfered to the recipient areas, 15 cases with different kinds of wounds were debrided thoroughly, 3 cases with scar were removed directly. The size of the flaps ranged from 3.0 cm x 4.5 cm to 26.0 cm x 22.0 cm, and the pedical was 5 cm to 7 cm in length.
RESULTSThe flaps in the 18 cases survived completely. Skin necrosis in the distal end of the flap appeared in 1 case, and the wound healed after the second repair. The follow-up period rang from 3 to 18 months. The apperance and function of the hand or foot was satisfactory.
CONCLUSIONSBecause the pedicled SCIA flaps can be obtain conveniently and contain sufficient blood-supply, so the flap is easy to survive and the flap can be designed in a large size. We believe it is an ideal method to use this flap to repair skin and soft tissue defects located in hands, forearms, the lower abdomen and perineal areas, the radicle area of thigh and so on.

Objective To explore the occurrence of diaphragmatic muscle fatigue(DMF) in children with pneumonia and its relationship with respiratory failure. Methods One hundred and twenty-six cases of pneumonia(pneumonia group) and 60 healthy children(healthy control group) were selected, and 2K-V1 type intelligence electric impedance respirograph was used to measure diaphragmatic function of the children in both groups to find if the ribcage-abdomen motion was synchronic and to calculate M levels. The abdomen impedance values as X-axis were calculated, which were regarded, and the chest impedance values as Y-axis were calcaulated, which described two-dimensional chart of ribcage-abdomen motion curve and the ? angle which was between connection of maximum X value to the smallest X value and the X-axis were calculated.According to the changes of chest-abdominal curve in one-dimensional chart and the changes of M value and ? angle, the degree of the diaphragmatic muscle fatigue was judged. Results There were 98 cases of DMF in the 126 children with pneumonia, and 68 cases of which were type Ⅰ DMF,and 30 cases were type Ⅱ DMF.One-dimensional chart showed contradictory chest-abdominal respiratory motional curve, and two-dimensional chart moved in a clockwise direction.M value was(46.1?8.4)%,? angle was(136.7?12.0) degrees in children with type Ⅰ DMF;One-dimensional chart showed that the peak value of chest-abdominal respiratory motional curve dislocates, and two-dimensional chart moved in a counter-clockwise direction.M value was(17.2 ? 3.2)%,? angle was(48.2 ? 9.5) degrees in children with type Ⅱ DMF;the control group had M value(4.3 ? 1.0)%,? angle was(31.7 ? 5.2) degree. There were significant differences in M value and ? angle between type Ⅰ DMF children, type Ⅱ DMF children and healthy children(Pa

OBJECTIVETo evaluate the effects of prenatal stress on neurological functions after middle cerebral artery occlusion (MCAO) in adult offspring rats.
METHODSPregnant rats were randomly assigned to prenatal stress treatment, which was exposed to restraint three times daily in the last week of pregnancy, and no prenatal stress treatment. Adult male offspring rats were subjected to transient focal cerebral ischemia by MCAO. They were randomly divided into four groups: sham group, prenatal stress + sham group, MCAO group and prenatal stress + MCAO group (n = 10). After 24 hours of reperfusion, the neurological deficits were evaluated. The infarct size, cell apoptosis and expression of Caspase 3, cleaved Caspase 3 and Bcl-2 were detected.
RESULTSCompared with MCAO group, the neurological deficits, infarct size and apoptotic cells in prenatal stress + MCAO group were increased significantly (all P < 0.05). The expressions of Caspase 3 and cleaved Caspase 3 were much greater in prenatal stress + MCAO group than those of MCAO group, while the expression of Bcl-2 was significantly decreased in prenatal stress + MCAO group compared with MCAO group (all P < 0.05).
CONCLUSIONPrenatal stress might exacerbate neuroloeical deficits in the offspring rats after MCAO by increasing cell apoptosis.

Objective To investigate the efficacy and safety of catheter radiofrequency ablation of paroxysmal supraventricular tachycardia. Methods From Jul 2003 to Jan 2011,1106 cases with narrow QRS complex tachycardia who were treated by catheter radiofrequency ablation were recruited from our center and followed up for the rates of successful treatment, rcurrence and complications. Results There were in total 1106 patients (atrioventricular reentrant tachycardia:588 ; atrioventricular nodal reentrant tachycardia:477; atrial tachycardia:41 ),with a sex proportion of 1∶1.Successful ablation rate was 98.3 % (1087/1106).Of the 1087 successful ablation cases,43 (3.9% )were warranted repeated ablation. The recurrent rates for trioventricular nodal reentrant tachycardia, atrioventricular reentrant tachycardia, left accessory pathway and right accessory pathway were 1.5%,5.6%,3.9%,and 9.1% respectively.Complication rate was 1.5%.The major complications included pheumothorax ( 6 cases ),pulmonary embolism ( 1 case ),transient third-degree atrioventricular block ( 2 cases ),first-degree atrioventricular block ( 3 cases ),and persistent third-degree atrioventricular block(2 cases)with pacemaker implantation. There was one case of cardiogenic sudden death 5 days after the treatment procedure. The cause of his death was chronic stroke-related but not related to the operation procedure. Conclusion Catheter ablation has high efficacy and low complication rate in long-term follow-up,and is a promising treatment for paroxysmal supraventricular tachycardia.

This study aims to investigate the current situation of the enrollment , training model and career planning of medical students in the targeted admission program .After analyzing the implementation process of this targeted admission policy in medical education , we raised some feasible strategies to increase the attraction and reten-tion of health professionals in rural China .The study covered 4 medical universities in central and western China .A cohort of 308 medical graduates from this targeted admission program were recruited in June 2015 .A baseline survey , including questionnaire survey , in-depth interviews and the focus group discussions , were conducted right before their graduation .The first follow-up online survey was done 6 months after their graduation .he findings of this investiga-tion show that 72.9%of the investigated students in the program used to live in rural areas before 15 years old.The average annual household income was 28,200 Yuan. The top three reasons of choosing this program were guaranteed job after graduation , low performance in the college entrance examination , and wavier of tuition fee .There were sig-nificant differences of implementation of this policy between different provinces , especially the contract-signing and job deployment .Income and working environment were the main influencing factors of the medical graduates 'job-see-king intention .Based on the findings of this study , it can be concluded that the targeted admission program is rather successful in attracting medical graduates for rural health services .However , extra efforts are needed to improve the implementation of the policy and to retain the medical graduates in rural area .
