1.Application of focus group interview to explore oncology nurse communication difficulties

Objective To explore the reason and influence of communication difficulties between oncology nurses and patients or families of patients,and to obtain some relevant advice about communication difficulties.Methods This study used focus group to interview 24 oncology nurses who were selected from the First Affiliated Hospital of Chongqing Medical University by homogeneous sampling method,and were randomly divided into three groups (8 in each group).The data were processed and analyzed by content analysis.Results The reasons why oncology nurses had communication difficulties with patients and their families were as follows:putting forward negative problems about their disease,ineffective communication and conflicts with patients,families.The communication difficulties showed negative effect on oncology nurses.Oncology nurses thought that the main reason of communication difficulties lied in the deficiency of institution and propounded the suggestion to resolve it.Conclusions oncology nurses were tend to have difficulty in communicating with patients and their families in the long-term work,especially in the period of crisis:early diagnosis of tumor stage,relapse,advanced cancer and some special dates (birthday,parties etc.).Therefore,oncology nurses,patients and their families need to be supported.To solve the difficulties of communication could reduce the negative emotions of patients and their family,such as anxiety,frustration,helplessness,so as to improve the quality of care and life of patients.

4.Hair Removal by Intensive Pulsed Light: A Preliminary Clinical Report

Objective To explore the clinical efficacy of intensive pulsed light(IPL) on body hair removal.(Methods A) total of 172 patients who suffered from heavy body hairs were treated by IPL since April 2002.It took 3 to 5 procedures,with the interval of one month. Results After treatment,80 patients(47%) were cured,42(24%) improved,and 50(29%) had no effect. Conclusion The intensive pulsed light system is safe and efficient in hair removal.
5. The cross-section study on warfarin anticoagulation effects with amiodarone's combination in different CYP2C9 and VKORC1 status

OBJECTIVE: To analyse the effect of amiodarone on the warfarin anticoagulation effects in different CYP2C9 and VORCORC1 status. METHODS: From January 2010 to November 2012, patients on warfarin therapy with or without amiodarone medication were included whose blood samples were collected and tested in VKORC1 and CYP2C9 genotypes and whose clinical data were collected. The clinical characterics, warfarin medication related data were compared between the groups with and without amiodarone therapy and the genotype status were used in the following subgroup analysis. RESULTS: There are no statistically difference of warfarin dosage or INR(international ratio of PT) value between the groups with and without amiodarone medication and no statistically difference in the following analysis of genotype subgroup. CONCLUSION: The patients on short-duration usage of amiodarone has no obviously effect on warfarin stable dosage and INR value during stable dosage.
6. A rat model of multiple organ dysfunction syndrome induced by subarachnoid hemorrhage

Objective: To establish a rat model of multiple organ dysfunction syndrome (MODS) by subarachnoid hemorrhage (SAH), and to explore the pathogenesis of MODS occured in acute cerebrovascular diseases (ACVD). Methods: Model of SAH was induced in rats by injecting arterial blood around the Willis' circle. 48 Wister rats were randomly divided into three groups: normal control group (n = 6), sham-operative group (n = 6) and SAH group (n = 36). The SAH group was further divided into 6 groups according to the time points of 4h, 12h, 24h, 36h, 48h and 72h after SAH, each group has six rats. Changes of temperature, heart rate and respiratory rate of the rats in each group were recorded, the amount of white blood cell (WBC) and biochemical criteria were assayed. Pathological changes of lung, liver, intestines and kidney were observed under optical microscope. Systemic inflammatory response syndrome (SIRS) and MODS were diagnosed by their criteria. Results: 1 There were no significant differences of temperature, heart rate, respiratory rate and the levels of WBC, ALT, AST, BUN, Cr and CK between normal control group and sham-operative group (P > 0.05). The rats of SAH group had obviously higher temperature, heart rate, respiratory rate, WBC, ALT, AST, BUN, Cr and CK than that of the normal control group and sham-operative group (P < 0.01), the changes were most obvious at 24-36 hour point. 2 The vital organs of the rats showed inflammatory injuries in varying degree at each time point after SAH, and peaked on 24-36 h, alleviated after 48 h, and last to 72 h after SAH. 3 The incidences of SIRS, MODS and mortality were 100% (36/36), 69.4% (25/36) and 38.9% (14/36) respectively in the SAH group. Conclusion: 1 An experimental animal model of MODS can be established successfully by injecting arterial blood around the Willis' circle in rats. 2 SAH can cause inflammatory changes in lung, liver, intestines, kidney as well as SIRS, and SIRS may be the pathological basal of MODS after SAH.
7.Speech therapy and occupational therapy on Broca aphasia with bucco-facial-apraxia following hemiplegia after stroke

ObjectiveTo study the method about language rehabilitation for Broca aphasis with bucco-facial apraxia in hemiplebies after stroke.Methods55 patients in hemiplegies after stroke who were diagnoised Broca aphasia with bucco-facial-apraxia by Chinese standard language test of aphasia and apraxia test,were randomly divided into two groups:treatment group(30 cases) and control group(25 cases). The trainning about language rehabilitation and occupational therapy(OT)to bucco-facial-apraxia were given in the treatment group, while in the control group the language rehabilitation training were given only. Evaluation was done in pre-treatment and post-treatment respectively.ResultsThe improvement of bucco-facial-apraxia and language expression function(repetition,speech and speech-reading) of treatment group were significant than that of control group (P<0.05).Conclusions The OT for bucco-facial-apraxia may obviously improved bucco-facial-apraxia and language expression function on training of language rehabilitation for Broca aphasia with bucco-facial-apraxia.

9.Operation Timing and Technique Selection of Floating Knee Injury in Children

Objective To explore the operation timing and technique selection of floating knee injury in children.Methods According to Letts types,36 cases were divided into 5 types.The function outcome in every group was compared by Karlstrorm criterion according operation timing or technique selection respectively.Results Thirty-six cases were followed up for an average of 25 months(19 to 84 months).The rate of excellent and good results accounted for 94.12%(16/17) in one stage operation,78.95%(15/19)in stages,60%(6/10)in 2 stages,respectively.Conclusions It may be recommended to treat children with floating knee injury with open reduction intenal fixation or external fixation.The best operation time shall be urgent treatment of both femur and tibia fracture.
