1.Development strategies of hospitals under the market economic system

Only by working out and implementing scientific and rational developmnent strategies can hospitals contend for the initiative and have a leading edge in the fierce market competition. At present, it is imperative for hospitals to develop the new century-oriented market competition strategy in view of their respective current conditions and circumstances, the talent developnent strategy based on the reform of the personnel system, the strategy of scientific and technological development, and the strategy of the socialization of rear services. Hospital managers ought to promote the construction of their hospitals in light of the above strategies. Nevertheless, in implementing hospital development strategies, it is important to guarantee the continuity and stability of the overall development strategies and not to make unpredictable changes. Only by doing so can the development of hospitals be pushed forward via working out hospital development strategies.

The standardized prevalences of CDS suggestiondiagnosed metabolic syndrome(MS) and its components were 11.46% in MS,5.17% in diabetes,16.35% in hypertension,25.59% in high triglyceride level,4.72% in low HDL-C level,18.84% in overweight or obesity in 2512 randomized official employees in Fushan city of Guangdong province in 2003.The MS prevalence was increased with ageing.
3.The estimation of the rationality of lower cut-point (5.6 mmol/L) of FPG for the diagnosis of impaired fasting glucose

Screening the 2512 employees aged 20~94 years in Fushan city by 75 g OGTT showed that 5.51 mmol/L of FPG was the optimal cut-point for diagnosis of diabetes with a sensitivity of 80% and a specificity of 80%; 5.15 mmol/L of FPG was the optimal cut-point for the diagnosis of IGT.It is reasonable to lower FPG cut-point for IFG.
4.Combined traditional Chinese and western medicine treatment of early postoperation inflammatory small bowel obstruction

Objective To investigate the principles of combined traditional Chinese and western medicine treatment of early postoperation inflammatory small bowel obstruction(EPISBO).Methods 93 cases of EPISBO were randomly divided into two groups including group A,47 cases received combined traditional Chinese and west- ern medicine treatment,and group B 46 received only western medicine treatment.Results 93 patients were all cured through nonsurgical treatment.The combined traditional Chinese and western medicine treatment was better in symtom improvement and reduce of expense.Conclusion The principle treatment of EPISBO is mainly nonsurgical. The traditional Chinese medicine and somatostatin are very important.
5.Clinical study on human lamellar keratoplasty for fungal corneal ulcers with porcine acellular corneal stroma

AIM:To observe the transplantation of acellular porcine corneal stroma on the treatment of superficial keratitis by drug-resistant fungal.METHODS:We performed a retrospective analysis of 16 cases of fungal keratitis received the transplantation of acellular porcine corneal matrix from June 2015 to March 2016 with a follow-up of 6mo.We analyzed on items as postoperative visual acuity, corneal graft status, postoperative recurrence and postoperative complications.RESULTS:We observed a healing time of corneal epithelium in 7 to 10d postoperatively generally and the absence of corneal edema in 1mo, while the cornea gradually returned transparent in the 16 cases.Two cases required medication for an epithelial recovery and 3 cases received intervention for decreasing intraocular pressure to a certain level.During the follow-up we observed no cases of cornea degeneration, recurrence of infection or rejection.The vision acuity showed 1.27±0.22, 1.11±0.13, 0.79±0.22 in 1, 3 and 6mo after operation respectively.There was no statistical difference between vision in 1mo and the vision before surgery (P=0.06);while we found a statistical difference when comparing the vision of 3 and 6mo with vision before surgery (P=0.01,0.001).The vision in 6mo increased with a statistic difference to the vision at 1 and 3mo (P<0.001) while no statistic difference was observed between 1 and 3mo(P=0.11).CONCLUSION:Transplantation of acellular porcine corneal matrix is a safe and efficient treatment for fungal keratitis.
6.Expression of estrogen and progesterone receptor subtypes in human endometrial carcinoma tissues

Objective To investigate the expressions of estrogen receptor (ER) and progesterone receptor (PR) subtypes in human endometrial carcinoma tissues. Methods Thirty samples of normal endometrial tissue and 66 endometrial carcinomas were investigated using western blot assays and reverse-transcription polymerase chain reaction. Results (1)Both ER? and ER? mRNA were significantly increased in endometrial carcinoma compared with that in normal endometrium tissues. The relative value of ER? and ER? mRNA in endometrial carcinoma was 8.00?7.77, 3.84?2.57 and that in normal endometrium tissues was 4.15?3.55, 0.41?0.29, respectively. 7.Changes of the biological behavior of dermal fibroblasts in Ⅲ skin burns wound in rats using chitosan

Aim To study the changes of the biological behavior of dermal fibroblasts in Ⅲ skin burns wound in rats using chitosan.Methods Wistar rats were randomly divided into 5 groups as follows:1% chitosan(W/V)group,2% chitosan(W/V)group,4% chitosan(W/V)group,bFGF(basic fibroblast growth factor) group and the control group.Rats were made for Ⅲ skin burns.The wound healing time was recorded,and the wound healing rate was calculated.Then the cell cycle and apoptotic dermal fibroblasts were determined and the amount of Hydroxyproline(HOP) in the skin tissue was analyzed.Results The wound healing rate of 4% chitosan(W/V) group was higher and the wound healing time of 4% chitosan(W/V) group was shorter than that of the control group.On the 7th,14th day post-injury,the content of protein of 4% chitosan(W/V) group was higher than that of the control group.The content of HOP of 2% and 4% chitosan(W/V) group was highest on the 7th day post-injury. Compared with that in control group,the percentage of cells of S stage in 4% chitosan(W/V) group was aboundant,and was reduced in apoptotic dermal fibroblasts.Conclusion The changes of cellular biological behaviors might be one of the mechanisms of that Chitosan could promote the wound healing of Ⅲ skin burns in rats significantly.
8.Deep Fungal Infection in Patients:Clinical and Pathogenic Analysis

OBJECTIVE To investigate the postoperative morbility of deep fungal infections and the source and composition of the pathogenic fungi.METHODS Clinical data of 816 patients with post-operative deep fungal infections from Jul 2006 to Jun 2008 were reviewed and analyzed retrospectively.RESULTS The detectable rate of post-operative deep fungal infections was 24.82%,among which Candida albicans was the most common(65.69%),followed by C.tropicalis(10.57%);the rate of broad-spectrum antibiotic application in peri-operation was 96.45%;the infection site in the descending order was cardio-thorax,gastrointestinal tract,urinary tract,female reproductive system,blood and skeleton.CONCLUSIONS Operative trauma is an important factor that causes deep fungal infections in hospital,and is closely related to broad-spectrum antibiotic application.Positive prevention,timely diagnosis and effective treatment should be highly emphasized when dealing with post-operative deep fungal infections.
9.The volume of residual urine correlates with bladder outlet obstruction and detrusor contractility in patients with benign prostatic hyperplasia.

OBJECTIVETo identify the correlation of the volume of residual urine (VRU) with the severity of bladder outlet obstruction (BOO) and detrusor contractility in patients with benign prostatic hyperplasia (BPH).
METHODSA total of 152 patients with clinically diagnosed BPH underwent ultrasonography for measurement of the prostate volume and RVU, free uroflowmetry, and urodynamic examination for the severity of BOO and detrusor contractility. Using the software SPSS20. 0, we analyzed the correlation between the ultrasonographic results and urodynamic parameters and compared the two sample means by the t-test.
RESULTSThe prostate volume was correlated positively with BOO severity (r = 0.432, P < 0.01) and detrusor contractility (r = 0.343 , P < 0.01) while Qmax negatively with BOO severity (r = 0.327, P < 0.01) but not significantly with detrusor contractility (r = 0.123, P > 0.05). VRU showed a significantly negative correlation with detrusor contractility when > 150 ml (r = -0.490, P < 0.01), even more significantly when > 300 ml (r = -0.717, P < 0.01), but exhibited no significant correlation with it when ≤ 150 ml (r = 0.041, P > 0.05).
CONCLUSIONVRU can somehow predict the detrusor function. For patients with VRU > 150 ml, especially for those with VRU > 300 ml, the detrusor function should be evaluated and urodynamic examination is recommended for exact assessment of BOO severity and detrusor contractility.

10.Finite element method analysis of anteflexion traction on various angles for the treatment of cervical spine.

OBJECTIVETo analyze the data of angle variation on traction based on a finite element model of complete cervical spine with straight physiological curvature, and try to give experimental reference and suggestion in treating cervical spondylosis.
METHODSA 43-year-old female patient with straight cervical spine was chosen and the CT scan data were collected. By using specially designed modeling system, a high quality finite element model of complete cervical spine with straight physiological curvature is generated,which included ligament and muscle according to anatomy. After the model was confirmed, traction was loaded with angle 0 degree, anterior 5 degrees, 10 degrees, 15 degrees, 20 degrees, 25 degrees, to observe the data of distance change on between adjacent intervertebral foramen, processus articularis, uncovertedral joint, intervertebral discs, and stress of anulus fibrosus and nucleus pulposus.
RESULTSWhen the angle was 0 degrees-15 degrees, the distance between intervertebral foramen, Luschka joint and processus articularis posterioris was enlarged, the tensile stress was adequate and compressive stress was small. It met the clinical requests.
CONCLUSION0 degree-15 degrees anterior position is suggested for the treatment of cervical spondylosis.
