1.Pre and Postoperative Assessment of Depression and Cognitive Dysfunction Aged 65 Years or Older in Department of Surgery.

BACKGROUNDS: As the lifetime is prolonged, the chances of operation of elderly people are increased concomitantly. In general, elderly patients have more frequent and severe psychiatric problems as depression and cognitive dysfunction than young patients. But, the prevalence rate of depression and cognitive dysfunction pre and postoperatively is known a little. Also, the delirium which makes the surgeon embarrassed is high in dementia and elderly patients even diagnosed normally preoperatively. We tried to know how many elderly people has depression and cognitive dysfunction pre and postoperatively, also how many elderly patients become delirious postoperatively. METHOD: We obtained questionnaires from 100 elderly patients(mean age 75.0) undergoing operation pre and postoperatively. We used Short Portable Mental Status Questionnaire(SPMSQ) which calculated the wrong answer for the fact to evaluate the cognitive dysfunction and short version of Geriatric Depression Scale(GDS) which calculated the wrong answer defined previously to study the depression. The depression was suggestted if GDS score was above 6 and cognitive dysfunction was suggested if SPMSQ score was above 3. The dementia and postoperative delirium patients were diagnosed and managed by the psychiatrist and the operator. RESULTS: Pre and postoperative mean score of GDS was 10.1 and 9.5 respectively. Pre and postopera- tive incidence of depression was 65% and 67% respectively. Pre and postoperative mean errors of SPMSQ was 6.5 and 6.7 respectively. Pre and postoperative incidence of cognitive dysfunction was 30% and 31% respectively. The dementia patients were ten. The postoperative delirium patients were six. Three of them were dementia patients diagnosed preoperatively and the others had non-cognitive dysfunction preoperatively. CONCLUSION: The incidence of depression and cognitive dysfunction in elderly patients is relatively high. And the postoperative delirium is high in dementia patients. Even the patients diagnosed non- cognitive dysfunction preoperatively have high incidence rate than young people. So, we must do critical care with elderly patients pre and postoperatively and know the appropriate management of depression and cognitive dysfunction to prevent prolonged hospitalization, poor quality of life and conflict with patient`s family.

2.Gross and microscopic findings of dermatophytes cultured on potato dextross agar.

To ease the convinience of indentification of common dermatophytes in Korea and to check the possibility of diseovering new species not yet reported in Korea, several species of dermatophytes were cultured on potato dextrose agar(PDA) at the room temperature. They were 46 strains isolated from Korea and 6 domestic strains from a foreign country. Gross and microscopic findings of their colonies were examined. The colonial morphology and microscopic appearance were compared with those of the same strains on Sabouraud dextrose agar(SDA) and corn meal agar(CMA). The results were as follows Typical T. rubrum and its varisnts showed characteristic wine red pigmentation which was rapid, diffuse and uniform, typically arrsnged microconidia were better on PDA. T. mentagrophytes showed yellow-brown pigmentation which were more rapid and darker in the center with gradual fading to the periphery. The mscroconidia, grape-like clusters of microconidia and spiral hyphae were seen better on SDA. T. tonsurans showed characteristic mahogsny colored pigmentation, ballooned microconidia and macroconidia which were seen better on PDA, but diffusible pigmentation of agar were present on SDA only. The gross morphology of T. verracosum were similar on PDA and SDA, but macroconidia were seen better on PDA and CMA, and tortuous hyphae with antler-like branching were seen better on SDA. T. rnegninii showed green-brown pigmentation which appeared rapidty and prominently on PDA. T. rubrum-like microconidia were seen better on PDA and CMA, but in SDA only hyphae were seen. The gross morpholgy of M. canis was similar on PDA and SDA, but spindle shaped macroconidia were seen more typically on PDA. The gross morphology and microscopic appearance of M. gypseum were very similar in all three media. M. ferruginenm showed characteristic rust colored pigmentation and bamboo hyphae which were seen more typically on PDA. The gross morphalogy of E floceosum were similar on PDA and SDA, but cluster formation of club shaped macroconidia were seen more prominently on PDA and CMA. These findings suggest that PDA is a mare convincing media t6Cifferentiate among dermatophytes belonging to genus Trichopkyton.

Partial trisomy 10q syndrome is a rare chromosome anomaly characterized by severe mental and growth retardation, craniofacial dysmorphia with prominent forehead, fine arched eyebrows, deep set small eyes and micrognathia, In addition, other physical manifestations have been reported as skeletal anomaly, congenital heart disease, inguinal hernia, and so on. We report a case of partial trisomy 10q syndrorne with certain stigmata which confirmed by chromosome analysis.

7.The Treatment of Hip Fracture

Because most of the hip fracture is endemic to an elderly population, high mortality and morbidity has been encountered due to the fatal complications such as pneumonia, thromboembolism, and decubitus ulceration. So, it is essential to achieve early ambulation with complete reduction by strong interoal fixation divices in order to reduce those complications. As early as 25 years ago McCarroll remarked that this fracture must be considered “unsolved until the incidence of aseptic necrosis and nonunion could be diminished or abolished. Since that time improved reduction method and many new strong fixation divices such as Jewett nail-plate, Massie nail, and compression hip screw have been developed to achieve more accurate reduction and rigid immobilization. The following clinical results were shown by analysis of 117 cases of hip fractures experienced in the Department of Orthopedic Surgery, Seoul National University in the past 5 years from Jan. 1974 to Dec. 1978. 1. Femoral neck fractures were 67 cases and intertrochanteric fractures were 36 cases. 2. The most common cause of hip fractures was slip or fall (70.1%). 3. Incidence of femoral neck fracture was most frequent in the 6th decade, and that of the intertrochanteric fracture was in the 7th decade that is compared with 7th and 8th decade of Caucasian respectively. 4. Old hip fracture more than 3 weeks after trauma was in the 53.6%, and especially that of neck fracture was about 57%. 5. In the classification of the hip fractures, transcervical neck fracture was most frequent (70.1%), and unstable type (79.5%) in the intertrochanteric fracture. 6. Knowles pin was usually used in the stable neck fracture, while compression hip screw used in the unstable neck fracture. 7. Jewett nail-plate was usually used after Dimon-Hughston reduction, and compression hip screw after anatomical reduction in the unstable intertrochanteric fracture. But in the stable fracture either Jewett nail-plate or compression hip screw was usually used after anatomical reduction. 8. Endoprosthesis (26 cases), and total hip replacement arthroplasty (8 cases) were usually carried out in the old femur neck fracture treated by total hip arthroplasty was better than that by endoprosthesis. 9. Avascular necrosis was 13.3%, and nonunion 20% in the femur neck fracture. Nonunion was 12.5%, and delayed union 4.2% in the intertrochanteric fracture. With the above mentioned results, the following conclusion is obtained: 1. It is recommended that open reduction or arthriplasty of the hip should be performed if the closed reduction of the fracture of femoral neck is not adequate. 2. Graden alignment index is a good criteria for adequate reduction of femoral neck fracture. 3. Malunion induces the fate of femoral head to be avascular in the femoral neck fracture. 4. Compression hip screw is a good stabilizer for unstable intertrochanteric fracture. 5. The position of femoral endoprosthesis for delayed or inadequately reduced femoral neck fracture may be replaced by total hip replacement. The clinical result of total hip replacement is far better than that of femoral endoprosthesis In the cases of arthroplasty indication.
