1.The correlation between vaginal intraepithelial neoplasia and cervical intraepithelial neoplasia, and their risk factors

Objective To investigate the correlation and risk factors between vaginal intraepithelial neoplasia (VAIN) and cervical intraepithelial neoplasia (CIN).Methods A total of 121 cases of VAIN patients was retrospectively analyzed with examination results such as clinical manifestations,liquid based cytology (TCT),human papilloma virus infection,virus infection type,and clinical data of treatment and follow up.Results VAIN Ⅰ,VAIN],and VAIN Ⅲ] were accounted for 33.9% (41/121),38.0% (46/121),and 24.4% (34/121) ; for 40 or less years old patients,VAIN Ⅲ was accounted for 10.7% (3/28) ; for more than 40 years old patients,VAIN Ⅲ was accounted for 33.3% (31 /93).Among 121 VAIN patients,77.7% (94/121) patients had CIN history,15.7% of which (19/121) had a history of cervical cancer; 27.3% (33/121) with hysterectomy history,of which 75.8% (25/33) was due to cervical intraepithelial neoplasia and cervical cancer resection of the uterus.Papilloma viral load in patients with VAIN Ⅰ was detected with a (HPV-HC2) positive rate of 87.8% (36/41),while in VAIN Ⅱ and VAIN [patients,the positive rate was 100%,the infection of high risk type of HPV ratio was 95.9% (116/ 121).of which the mixed infection was accounted for 56.9% (66/116).There was 94.6% in oatients with VAIN (114/121),and liquid-based cytology test results suggest abnormality; 88.4% (107/121) in patients with VAIN lesions occurred in upper 1/3 of vaginal.After diagnosed cervical disease,the onset time of VAIN was (3.45 ± 2.02) years,non-total-hysterectomy was (3.24 ± 1.97) years,after hysterectomy to pathological diagnosis time for VAIN was (5.01 ± 3.95) years:for the diagnosis of VAIN disease after resection of cervical original time was (3.96 ± 2.99) years,and rather than non cervical disease resection was an average of (9.94 ±5.37) years.In the course of treatment of the 12l cases of CIN patients,6 cases of patients were progressed to invasive carcinoma.Conclusions The clinical manifestation of VAIN is similar to CIN,the principle of diagnosis and treatment of CIN can also apply to VAIN,high risk HPV infection virus,cervical intraepithelial neoplasia are the risk factors for VAIN.

5.Mechanism of the reversibility of chronic cyclosporine nephrotoxicity after drug withdrawal

Objective To explore the mechanism of the reversibility of chronic cyclosporine (CsA) nephrotoxicity in rats after long-term drug withdrawal. Methods Chronic CsA nephrotoxicity was induced in Sprague-Dawley rats by administering CsA(15 mg . kg-1. d-1) for 5 weeks,and then the effect of drug withdraw for 5 and 10 weeks was observed. Body weight,systolic blood pressure,and renal function were monitored. In addition,renal histopathology(arteriolopathy,ED-1-positive cells,and tubulointerstitial fibrosis) and expression of osteopontin(OPN) and transforming growth factor(TGF) -?1 mRNA was examined. Results Compared with the control rats,CsA-treated rats showed loss of body weight,deterioration in renal function and development of typical histopathology. All of these above parameters were significantly reversed,meanwhile,the upregulation of OPN and TGF-?1 mRNA expression decreased significantly at 5th and 10th week after CsA withdrawal. Of note,the decreased OPN and TGF-?1 mRNA expression was positively correlated with the reduction in the tubulointerstitial fibrosis score. Conclusion The established chronic CsA nephrotoxicity is reversible after long-term CsA discontinuation,and the mechanism may be associated with the down-regulation of OPN and TGF-?1 mRNA expression.
6.RP-HPLC in determination of dextromethorphan and dextrophan in human urine:phenotype analysis of CYP2D6

Objective To establish a RP-HPLC method for determination of the concentrations of dextromethorphan and its metabolites dextrorphan in human urine.Methods Phenacetine was used as internal standard,and the urine sample was hydrolyzed by enzyme,alkalified and extracted with hexane-butanol(91).The separation was carried out on DiamonsilTM C18 column(250 mm?4.6 mm,5 ?m) with mobile phase of acetonitrile-1% triethylamine buffer solution(pH adjusted to 2.2 with H3PO4).Gradient elution was done for 0-15 min(20%-35% A).The flow rate was 1.0 ml/min.The detection wavelength was set at 280 nm and the column temperature was 40℃.Results The linear ranges of dextromethorphan and dextrorphan were 0.05-2.0 ?g/ml(r=0.999 9,n=5) and 0.5-20.0 ?g/ml(r=0.999 9,n=5),respectively,and their lowest detecting concentrations were 0.04 ?g/ml and 0.4 ?g/ml,respectively.The intra-day and inter-day precision were both less than 10%.The low,middle and high extraction recoveries were between 94%-108%.Conclusion Our method is accurate and sensitive,and is suitable for the CYP2D6 phenotype analysis and pharmacokinetic studies of dextromethorphan and its metabolites in human.
7.Effect of intracoronary stenting for bifurcational lesions on the side branches

Objective To study the effect of intracoronary stenting for bifurcational lesions on the related side branches. Methods One hundren and twenty patients with 132 coronary bifurcational lesions were enrolled in the study. Intracornary stents were deployed in the lesions. The incidence of side branch oppression was analysed. Results Twenty-four side branches were oppressed during PCI (14.28%), especially in type Ⅰ and type Ⅳ bifurcation lesions. Conclusion The details of PCI and morphotogical characteristics of bifurcation lesions were the important factors leading the side branch oppression during PCI in bifurcation lesions.
8.The Drawbacks of Applying the "Equitable Responsibility" Principle to Settle the Medical Accident and the Measure to Deal with it

Objective To discuss the medical treatment responsibility caused by unpredictable harm.Method In medical treatment,the court usually applies the "Equitable Responsibility" principle to settle the medical conflicts caused by unpredictable harm,this poses certain limitations.Result The fact that hospitals being liable for compensation or reparation ignores the mutual risk-sharing responsibility of hospitals and patients.Conclusion To establish the system of medical accident insurance is one of the best measure to solve the medical conflicts caused by unpredictable harm.
9.Research in application value of outpatient mobile infusion system in the department of infectious outpatient

Objective To observe the effect of the outpatient mobile infusion system in the department of infectious outpatient night infusion.Methods From July to December in 2012,520 patients in department of infectious outpatient infusion in our hospital were selected and were divided into the observation group (using outpatient mobile infusion system)and the control group (adopting conventional infusion) with 260 cases in each group.The infusion effect was compared between two groups.Results The average occupancy per patient care time in the observation group was less than the control group,the difference was statistically significant.The satisfaction degree in the observation group was higher than that of the control group,the difference was statistically significant.There was no nursing error occurred in both of the observation group or the control group.Conclusion The use of outpatient mobile infusion system can significantly improve the nurses work efficiency,ensure nursing safety,improve quality of care and patients' satisfaction degree.
