2.Pre and Postoperative Assessment of Depression and Cognitive Dysfunction Aged 65 Years or Older in Department of Surgery.

BACKGROUNDS: As the lifetime is prolonged, the chances of operation of elderly people are increased concomitantly. In general, elderly patients have more frequent and severe psychiatric problems as depression and cognitive dysfunction than young patients. But, the prevalence rate of depression and cognitive dysfunction pre and postoperatively is known a little. Also, the delirium which makes the surgeon embarrassed is high in dementia and elderly patients even diagnosed normally preoperatively. We tried to know how many elderly people has depression and cognitive dysfunction pre and postoperatively, also how many elderly patients become delirious postoperatively. METHOD: We obtained questionnaires from 100 elderly patients(mean age 75.0) undergoing operation pre and postoperatively. We used Short Portable Mental Status Questionnaire(SPMSQ) which calculated the wrong answer for the fact to evaluate the cognitive dysfunction and short version of Geriatric Depression Scale(GDS) which calculated the wrong answer defined previously to study the depression. The depression was suggestted if GDS score was above 6 and cognitive dysfunction was suggested if SPMSQ score was above 3. The dementia and postoperative delirium patients were diagnosed and managed by the psychiatrist and the operator. RESULTS: Pre and postoperative mean score of GDS was 10.1 and 9.5 respectively. Pre and postopera- tive incidence of depression was 65% and 67% respectively. Pre and postoperative mean errors of SPMSQ was 6.5 and 6.7 respectively. Pre and postoperative incidence of cognitive dysfunction was 30% and 31% respectively. The dementia patients were ten. The postoperative delirium patients were six. Three of them were dementia patients diagnosed preoperatively and the others had non-cognitive dysfunction preoperatively. CONCLUSION: The incidence of depression and cognitive dysfunction in elderly patients is relatively high. And the postoperative delirium is high in dementia patients. Even the patients diagnosed non- cognitive dysfunction preoperatively have high incidence rate than young people. So, we must do critical care with elderly patients pre and postoperatively and know the appropriate management of depression and cognitive dysfunction to prevent prolonged hospitalization, poor quality of life and conflict with patient`s family.

3.Correlation of serum hyaluronidase with Ang Ⅱin hepatitis B patients

Objective To study on the correlation of hepatic fibrosis with angiotensin Ⅱ (Ang Ⅱ ) in the hepatitis B patients . Methods The electrochemical luminescence and the rate method were adopted to detect hyaluronidase (HA) and Ang Ⅱ levels in 92 cases of acute hepatitis B (acute group) ,108 cases of chronic hepatitis B(chronic group) and 50 healthy people(control group) , and the detected results were performed the correlation analysis .Results Compared with the acute group and the control group ,the HA and Ang Ⅱ levels in the chronic group were increased significantly (P<0 .05) .The HA and Ang Ⅱ levels had statistical differ-ence between the acute group and the control group (P<0 .05) .Further analysis showed that there was a positive correlation be-tween HA and Ang Ⅱ levels(r=0 .629 ,P<0 .05) .Conclusion There the HA level is closely related with the Ang Ⅱ level ,and both of them participate in the process of liver fibrosis ,their levels have the guiding significance in evaluation of severity of liver fi-brosis in the patients with hepatitis B .
8.Clinicopathologic analysis of 3 patients with metaplastic thymoma

Objective To investigate the clinicopathologic characteristics of metaplastic thymoma.Methods The histological features and immunophenotype of three cases with metaplastic thymoma were studied, and the literatures were reviewed.Results There were 2 females and 1 male of 3 patients with 45, 52 and 40 years old, respectively.All tumors were characterized by a biphasic pattern composed of epithelial components and spindle cells.The epithelial areas consisted of anastomosing cords and islands or broad trabeculae of cohesive neoplasia cells.Some epithelial cells showed mild cytologic atypia, the presence of prominent nuclear grooves or pseudoinclusion body, and very low mitotic activity.The cells appeared bland and a fine nuclear chromatin pattern, and the mitoses were not found.The epithelial cells were diffusely and strongly positive for AE1/AE3.The spindle cells were diffusely positive for Vimentin.The Ki-67 fraction was low (<5 %) in both components.Conclusions Metaplastic thymoma is a rare primary thymic epithelial tumor, and its biologic behavior is likely benign or lowly malignant process.Occasionally, the tumor invades adjacent tissue or recurrence.With complete tumor resection, the patients have good prognosis.The diagnosis of metaplastic thymoma mainly relies on its morphologic features and immunohistochemical profiles.
9.Gastric cancer chemotherapy related genes

The expression of genes have an important effect to gastric cancer chemotherapy.Some genes are related the therapeutic efficacy of drugs,some genes are related the untoward effects of drug,and some genes are related the sensitivity of the tumor to drug.Chemotherapy related genes may play an important guiding role to improve the curative effect of chemotherapy patients.In recent years,the gastric cancer chemotherapy related genes have made great progress,especially in platinum,fluorouracil,paclitaxel and targeted medicine related genes.
