1.Clinical analysis of surgical treatment of 79 patients with primary cardiac tumors

Objective To analyze the clinical features in patients with primary cardiac tumors and surgical treatment of primary cardiac tumors.Methods A retrospective analysis of primary cardiac tumors patients from June 2002 to September 2012 was performed.79 patients were confirmed using cardiac echocardiography.Primary cardiac tumors were excised using general anesthesia during cardiopulmonary bypass.Results Only 5 patients relapsed after the follow-up for 6 months to 5 years.No tumor recurrence and distant metastases were found in the other patients.Conclusion Left atrial myxoma should be removed by surgical resection as soon as possible once there is a clear diagnosis.The surgical effect of left atrial myxoma is good but it may relapse.The malignant cardiac tumors progress rapidly and should be removed by early surgery combined with comprehensive treatment to improve the prognosis.
2.Injectable small intestinal submucosa is co-cultured with adipose-derived mesenchymal stem cells in vitro

BACKGROUND:The decel ularized porcine smal intestinal submucosa is a kind of bioactive extracel ular matrix, which is mainly composed of col agen, glycoprotein, proteoglycan and rich in col agen, glycosaminoglycan and various growth factors, and these components play an important role in promoting the differentiation and proliferation of tissue cel s. OBJECTIVE:To prepare the injectable smal intestinal submucosa and to investigate its co-culture with rat adipose-derived mesenchymal stem cel s in vitro. METHODS:The injectable smal intestinal submucosa and rat adipose-derived stem cel s were prepared. Cel counting kit-8 test for cel proliferation:Passage 3 adipose-derived stem cel s were seeded onto the injectable smal intestinal submucosa (experimental group) and cel s cultured under normal condition as control group. The cel proliferation was observed at 1, 3, 5 and 7 days of incubation. Live/dead staining test for the survival of cel s:Passage 3 adipose-derived stem cel s were respectively cultured in the injectable smal intestinal submucosa extracts (experimental group) and complete culture medium (control group). Cel survival was determined at 1, 3, 5 and 7 days of culture. RESULTS AND CONCLUSION:Scanning electron microscope oval and strip adipose-derived stem cel s adhered onto the material. The absorbance values in the experimental group were higher than those in the control group at 1 and 5 days of incubation (P<0.05). Cel survival:The number of cel s appeared to be in a rising trend with time in both two groups;after 1-day co-culture, al cel s in the two groups survived. Then dead cel s appeared in both two groups, showing no significant difference. These results show that the injectable smal intestinal submucosa exhibits a good cytocompatibility.
3.Risk of stroke recurrence and its predictors in young patients with ischemic stroke or transient ischemic attack

Objective Toinvestigatetheriskoflong-termrecurrenceofstrokeanditspredictorsin youngpatientswithischemicstroke/transientischemicattack(TIA).Methods Theclinicaldataofthe consecutive young patients (18-45 years)with ischemic stroke/TIA (within 2 weeks after onset)admitted to the department of neurology in the First Affiliated Hospital of Sun Yat-Sen University between August 2008 and July 2013 were enrolled prospectively. All patients were regularly followed up for a long time (The patients were followed up at the 1 st,6 th,and 12 th month after onset;then they were followed up once for every 6 months)in order to investigate stroke recurrence. The Kaplan-Meier curves were used to analyze the cumulative stroke recurrence rate of all patients. The last contact time for patients lost to follow was used as censored data to be enrolled in the analysis. The univariate analysis of the related risk factors for stroke recurrence using Log-rank test. Multivariate Cox proportional hazard regression was used to detect the related risk factors associated with stroke recurrence (adjusting for age and sex). The variables of the results of Log-ranktestP≤0.1wasselectedandenrolledinthemultivariateregressionanalysis.Results Atotalof 312 patients were enrolled in the analysis,including 294 with ischemic stroke and 18 with TIA. Their mean follow-up time was 34 ± 19 months. Thirty-four patients had recurrent stroke,including 23 with ischemic stroke,7 with TIA,and 4 with cerebral hemorrhage. The cumulative recurrence rates of stroke at 1 ,3 , and 6 years after onset were 6. 2%,10. 3%,and 16. 4%,respectively. The results of multivariate Cox proportional hazards regression analysis showed that hypertension (risk ratio [RR]2. 159;95% confidence interval [CI]1. 048-4. 447,P=0. 037)and cardioembolism (RR,2. 869;95%CI 1. 119-7. 357,P=0.028)weretheindependentpredictorsforstrokerecurrence.Conclusion Theoverall6-yearriskof recurrent stroke is not high in the Chinese young patients with ischemic stroke/TIA,but the risk of stroke recurrence is relatively higher in the first year. Hypertension and cardioembolism are the potential predictors of stroke recurrence;therefore,attention should be paid in clinical practice.
4.Risk factors, diagnostic evaluation, etiology and treatments in young patients with ischemic stroke:a prospective single center study

Objective To investigate the risk factors, diagnostic evaluation, etiology, and treatment in young patients with ischemic stroke. Methods The clinical data of young patients (age range 18 - 45 years) with ischemic stroke (within 2 weeks of stroke onset) admitted to the Stroke Center, the First Affiliated Hospital of Sun Yat-sen University from August 2008 to July 2013 were registered prospectively. Results A total of 300 patients were enroled, their age was 37. 8 ± 6. 8 years. There were 227 males (75. 7% ). The age of 84 patients (28. 0% ) was ≤35 years. The most common risk factors were smoking (43. 3% ), hypertension (38. 7% ), and hypercholesterolemia (38. 0% ). The positive result detection rates were lower in the antinuclear antibody (8/278), anticardiolipin antibody (34/250), 24-h Holter monitoring (2/60 ), and transthoracic echocardiography (38/232). According to the MRI findings, the detection rate of the white matter changes, old infarcts, single acute infarcts, and multiple acute cerebral infarcts were 40/282, 77/282, 145/282, and 137/282, respectively. The stroke subtypes of TOAST: large artery atherosclerosis (26. 7% ), smal artery occlusion (18. 0% ), cardioembolism (10. 0% ), other determined etiology (23. 0% ), and undetermined etiology (22. 3% ). Large artery atherosclerosis was mainly involved in anterior circulation (69/80) and intracranial arteries (75/80). In other definite causes, cerebral artery dissection was most common (36/69), and other causes included moyamoya disease (7/69) and infectious vasculitis (7/69), etc. The average length of hospital stay was 15. 4 d. The mean National Institutes of Health Stroke Scale score on admission was 7. 0, and the mean modified Rankin scale score at discharge was 2. 5. The complication rate during hospitalization was 9. 7% . 80. 3% and 48. 3% of patients received anti-platelet drugs and statins therapy during hospitalization.Conclusions This study used a prospective single-center method. It conducted a comprehensive analysis of risk factors, diagnostic evaluation, treatment, and etiology in current Chinese young patients with ischemic stroke. Its research data wil provide useful information for establishing a diagnostic strategy of high performance cost ratio, in-depth understanding of its pathophysiological mechanisms, and improving treatment strategies in Chinese young patients with ischemic stroke.
5.Preliminary study on the value of ratio of serum luteinizing hormone/follicle-stimulating hormone in diagnosis of polycystic ovarian syndrome among women with polycystic ovary

Objective To investigate the value of ratio of luteinizing hormone (LH) to folliclestimulating hormone (FSH) in diagnosis of polycystic ovarian syndrome (PCOS) among women with ploycystic ovary (PCO) and to compare the difference of the diagnostic criteria between the Rotterdam Consensus and the Committee for Reproductive and Endocrine in Japan Society of Obstetrics and Gynecology.Methods By means of transvaginal Doppler ultrasound, 195 women with PCO were diagnosed in Nanfang Hospital of Reproductive Medicine Center and compare difference of multiple clinical indexes according to Rotterdam consensus and Japan consensus respectively. In the mean time, the ratio of LH/FSH, the level of LH, testosterone (T) and recevier operating characteristic (ROC) curve were explored to on the value of diagnosis of PCOS. Results By Rotterdam consensus, 144 women were diagnosed with PCOS and 51 women were non-PCOS, while 111 were identified as PCOS and 84 were non-PCOS according to Japan consensus. LH/FSH in PCOS and non-PCOS were 1.59 ±0. 84 and 0. 85 ±0. 47 respectively when based on Rotterdam consensus, and this ratio were 1.87 ± 0. 76 in PCOS and 0. 78 ± 0. 39 in non-PCOS based on Japan consensus. When using LH/FSH to diagnosis PCOS by Rotterdam consensus and Japan consensus,areas under ROC curve are 0. 786 and 0. 942, respectively. Conclusions The ratio of LH/FSH ≥ 1 provide the significant value in the diagnosis of PCOS. The criteria of the Committee for Reproductive and Endocrine in Japan Society of Obstetrics and Gynecology is more suitable for Chinese patients.
6.Quality of life for drug abusers accepting methadone maintenance treatment.

OBJECTIVE:
To explore the change and influence factors of quality of life QOL for drug abusers who accepted methadone maintenance treatment (MMT).
METHODS:
The questionnaire of QOL-BRIEF was used to measure the level of QQL when patients just entered the study and 6 months after the treatment respectively. The effect of therapy and characteristic of patients on change of QOL was analyzed.
RESULTS:
The scores in physiological domain, psychological domain, and the total score obviously increased after the treatment. There was no significant difference in the change of QOL between the patients of different dose groups. The QOLs of males and patients with long-time drug use improved significantly after the treatment.
CONCLUSION
MMT can raise the QOL of drug abusers. Gender and time of drug use may be the risk factors of QOL improvement.

8.PMP22 mutation of an infant-onset Charcot-Marie-Tooth disease family.

OBJECTIVETo study the mutation of PMP22 gene of an early-onset family with Charcot-Marie-Tooth disease (CMT) and the genetic features of the disease.
METHODSTwo patients with CMT, fifteen unaffected members in the family and 20 healthy controls were enrolled. STR-PCR and gene scanning were used to detect PMP22 duplication mutation.
RESULTSThe mutations of PMP22 were found in the two patients and other five unaffected members in the family. The mutations were located in the STR locus D17S921 in 5 cases and in the STR locus D17S4A in 2 cases. The other members in the family and 20 healthy controls did not show the mutations of PMP22.
CONCLUSIONSThe gene causing CMT in the family is found in the 17p11.2-p12 region containing PMP22 gene duplication mutation, resulting in the subtype CMT1A.

9.Effects of intracerebroventricular administration of brain-derived neurotrophic factor at different time intervals on cerebral ischemia-reperfusion injuries in rats

Objective To observe the effects of intracerebroventricular administration of exogenous brain-derived neurotrophie factor (BDNF) at different intervals on cerebral ischemia-reperfusion(I/R)injuries in rats. Methods A total of 70 adult male Wistar rats,weighring 250-300g,were randomly divided into normal control group(group C,n=10),ischemia-reperfusion group(group I/P,n=10)and BDNF-treatment groups([12 or 6 h before ischemia,at just the moment performing ischemia,60r12 h after ischemia][group A1,A2,A3,A4 and A5],n=10).Focal cerebral I/R model was established in rats by reversible left middle cerebral artery occlusion with filament.BDNF(0.5μg)was microinjected into the left lateral ventricular of the rats in the BDNF-treatment groups.The activities of superoxide dismutase(SOD)in the brain tissue were measured by xanthine oxidase method and the contents of malondialdehyde(MDA)in the brain tissue were determined by thiobarbituric acid method.Neuron apoptosis in the brain cortex was identified by TUNEL.One left brain cortex(1 mmx×1mm)was removed randomly in each group to observe the ultrastructure changes of cerebral tissue under electron microscope. Results Compared with those in the group I/R,the activity of SOD was increased significantly but the content of MDA was decreased significantly in ischemia brain tissue in every BDNF-treatmnent group (P<0.05).The index of apoptotic neuron cells in the BDNF-treatment groups was decreased significantly as compared with that in the group I/R(P<0.05).The activity of SOD in the brain tissue(25.02±2.77,24.01±1.03)was higher and the content of MDA(10.35±1.23,12.29±0.92)and the index of neuron apoptosis(21.77±3.56.23.84±2.63)were lower in the BDNF-treatment groups than those in the other 2 groups at 12 or 6 h before ischemia (P<0.05).The ultrastructure of the cerebral tissue in each BDNF treatment group was improved to different degree,but that in I/R group was seriously damaged. Conclusion Intracerebroventricular administration of BDNF at different time intervals, significantly decreasing the oxidative stress and the neuron apoptosis rate,has obvious protective effect in a time-dependent manner on brain injury and intracerebroventricular microinjection of BDNF before ischemia has a far better effect.
10.Evidence of waveform information in arterial blood gas by beat-by-beat sampling method in patients with normal heart function.

OBJECTIVESince 2011 EB-APS conference, we hypotheses that phase switching of inspiration-expiration is dominantly initiated by oscillatory information PaO2, PaCO2 and [H+] via fast peripheral chemical receptors. However, the evidence of the waveform of ABG is lack.
METHODSSix surgery patients with normal heart function and negative Allen test, had been placed the arterial catheterization directly connected to 3 x 1 000 mm pre-heparin plastic pipe for continuous collecting arterial blood. We counted the number of heart beat for the blood collecting time, and separated the blood pipe into the heart beat numbers' short pieces using haemostatic forceps, then put pipe into iced water at once fir analyzing PaO2, PaCO2, pH and SaO2 as soon as possible. We selected two breaths cycles of waveform from each patient for data calculations of magnitudes and time interval.
RESULTSThe heart beat numbers for filling blood into pipe were 16 ± 2, and all covered more than 2 breathing cycles. Each breathing cycle is cover 5 ± 0.6 heart beat. There were significant changes of PaO2, PaCO2, [H+] a and SaO2 (i.e. the highest high values compare to the next lowest values, P < 0.05). The time interval of changing PaO2, PaCO2, [H+]a and SaO2 magnitudes were 11.28 ± 1.13 mmHg, 1.77 ± 0.89 mmHg, 1.14 ± 0.35 nmol/L and 0.52% ± 0.44% respectively.
CONCLUSIONThis simple continuous beat-by-beat arterial blood sampling and ABG analyzing method is new and practicable. We obtain a clear evidence of periodic parameters ABG waveform, which following breathing cycle.
