Objective Objective To explore the nursing experiments of using health education for spontaneous pneumatothorax patients who have treated by video-assisted thoracoscopic surgery(VATS).Methods Divided 100 patients spontaneous pneumatothorax into 2 groups randomly,one was observation group and accepted health education,the other was control group and no health education.Follow up 2 groups by telephone and compare the satisfactory rate of nursing. ObjectiveTo explore clinical effect and safety ofShaoyao-Gancao decoction combined withSiwu decoction for the patients with diabetic peripheral neuropathy (DPN).MethodsA total of 76 patients with DPN in our hospital from 2013 July to October 2015 were divided into control group and observation group by the random number method, 38 patients in each group. The control group were treated with conventional treatment combined with oryzanol injection, and the observation group were treated with Shaoyao-Gancaodecoction combined withSiwu decoction on the basis of control group. Both groups were treated for one month. The level of FPG, 2 hPG, HbAlc, TC, TG, LDL-C and HDL-C was detected respectively before and after treatment. The whole blood viscosity and plasma viscosity was detected with LBY-N6A self-cleaning rotary viscosimeter. The red blood cells deposited and platelet aggregation rate were tested with Winchester points vascular method and turbidimetric method, respectively.The leg nerve and sural nerve of nerve conduction velocity (nerve conduction velocity, NCV) were determined with electromyography, at the same time the clinical curative effect was evaluted.ResultsTotal effect rate of observation group was 89.5% (34/38), which was significantly higher than the control group 71.1% (27/38), and the difference was statistically significant (χ2=4.070,P=0.044); After treatment, the levels of FPG (6.12 ± 0.38 mmol/Lvs.6.58 ± 0.52 mmol/L, t=4.403), 2 hPG (7.83 ± 0.82 mmol/Lvs. 8.41 ± 0.93 mmol/L,t=2.884), HbAlc (6.27 ± 0.52%vs. 6.82 ± 0.64%, t=4.112) in the observation group were lower than those in the control group (P<0.05). The levels of TC (4.73 ± 0.83 mmol/L vs. 5.11 ± 0.64 mmol/L,t=52.235), TG (1.83 ± 0.35 mmol/Lvs. 2.03 ± 0.41 mmol/L, t=2.287) and LDL-C (2.91 ± 0.54 mmol/Lvs. 3.25 ± 0.58 mmol/L,t=2.645) in the observation group were lower than those in the control group (P<0.05), and the HDL-C (1.47 ± 0.33 mmol/Lvs. 1.26 ± 0.31 mmol/L,t=-2.859) in the observation group was higher than than that in the control group(P<0.05). The level of whole blood specific viscosity (10.16 ± 2.12 mPa?svs. 11.33 ± 2.51 mPa?s,t=2.195), plasma viscosity (1.24 ± 0.25 mPa?svs. 1.62 ± 0.37 mPa?s,t=5.246), red blood cell volume (0.31 ± 0.16L/Lvs. 0.42 ± 0.08 L/L,t=3.791) and platelet aggregation rate (50.21% ± 8.03%vs. 54.16% ± 7.82%,t=2.172) in the observation group were significantly lower than those in control group (P<0.05). The tibial nerve velocity (45.22 ± 3.20 m/svs. 38.26 ± 5.19 m/s,t=-7.037) and the common nerve velocity (43.22 ± 6.34 m/svs. 36.23 ± 4.81,t=-5.415) in the observation group were as significantly higher than those in control group (P<0.05).ConclusionsThe Shaoyao-Gancaodecoction combined withSiwu decoction could improve the state of blood sugar, blood lipid and blood rheology of patients with DPN, and improve nerve transfer speed of leg nerve and sural nerve, the curative effect is better than conventional western medicine therapy alone.

Objective To discuss the effects and methods of the surgical treatment of complex humeral nonunion.Methods Ten cases of scleroid humeral nonunion were treated by compressive interlocking medullary nailing and autografting of iliac bone.Fifteen cases of humeral nonunion combined with osteomyelitis or osteoporosis were treated with grafting of free vascularized fibular flaps.Results All the cases were followed up for six months to six years.They all obtained bone union.The average time of bone union was 3.1 months for the free vascularized fibular grafting group and 3.8 months for the interlocking medullary nail group.Conclusions Compressive interlocking medullary nailing and autografting of iliac bone is effective for scleroid humeral nonunion,while grafting of free vascu- larized fibular flaps is effective for humeral nonunion combined with osteomyelitis or osteoporosis because the free vas- cularized fibular grafting can well reconstruct the defective humerus bone with osteoporosis or osteomyelitis.

Objective To study the value of lung window and soft tissue window in measuring colon diseases with experimental CTVC examination.Methods Twenty-four artificial polypoid diseases were placed with glue in a thoroughly cleaned porcine colon(overall length,150 era)that was distented with air and submerged in a water phantom and scanned 10 times on a 64-MSCT(GE Light speed)with identical scanning parameters.The data were reconstructed every 0.625 mm and reviewed using 2D images, navigation,MPR and 3D volume-rendered images on a GE AW 4.2 workstation.The maximum diameters of the artificial polypoids were measured by two radiologists with more than 10 years working experience in lung window(W = 1000,L =-700)and soft tissue window(W =400,L =40),respectively.The average value of maximum diameters of the artificial polypoids were measured for 10 times and were compared with the actual diameter.Results Twenty-four artificial polypoid diseases and their shape,size,loeation and relation of diseases with bowel wall were all showed excellently.The maximum diameter measured was close to the actual diameter.The average value was mostly less than actual diameter and a few of them more than the actual diameter.The maximum diameter measured in lung window was close to the actual diameter,and there was no difference between the two data sets(t =0.431,P =0.669).There was difference between the maximum diameter measured in soft tissue window and the virtual diameter(t = 2.691,P = 0.010). Conclusion In screening the colon diseases,CTVC is a good method of choice with no insult or less insult, and higher repeatability.The lung window should be chosen in measuring colon diseases in CTVC examination.

Objective Callus induction and plantlets regeneration of Pogostemon cablin(Blanco)Benth.are studied to establish a high-efficiency regeneration system for tissue culture.Methods The leaf segments,nodular stem segments,stem segments and root tips from Pogostemon cablin(Blanco)Benth.were cultured in vitro.The factors which affect the induction,multiplication and differentiation of callus were studied.Results Benzylaminopurine(BA) or thidiazuron(TDZ) combined with ?-naphthylacetic acid(NAA) was helpful to induce callus.The optimal concentrations of BA were 0.2～0.5mg/L.Nodular stem segments and stem segments were the most suitable explants for callus induction.The medium containing BA and NAA promoted the multiplication of callus.The multiplication rate and differentiation rate of 30-day callus were much higher than those of 60-day callus.BA was helpful to induce callus differentiation.The callus from leaf segments exerted a higher differentiation rate.Conclusion A high-efficiency regeneration system of callus induction and plant regeneration from Pogostemon cablin(Blanco)Benth.has been established.

OBJECTIVE:To investigate the risk,advantages and disadvantages and countermeasures of new drugs,generic drugs and imported drugs in different transfer opportunities,and to provide basis for improvement of development strategy for phar-maceutical enterprises. METHODS:The analysis was done in accordance with relevant regulations on transferable projects in the process of applying for registrations of new drugs,generic drugs and imported drugs. The transfer period and risk were explored and countermeasures were put forward. RESULTS & CONCLUSIONS:Transferable projects included intellectual property rights (patents,patent application,technical secrets,application information,non-disclosed data,etc.)and ownership rights(clinical tri-al approvals,new drug certificates,drug approval number,pharmaceutical product registration certificates,imported product regis-tration certificates,etc.)in the process of applying for registrations. There are 4 opportunities for drug technology transfer,opportu-nity 1 is before applying clinical trial approvals after the completion of non-clinical research such as pharmacology,toxicology;op-portunity 2 is ahead of clinical trial after the acquirement of clinical trial approvals;opportunity 3 is new drug technology transfer;opportunity 4 is production technology transfer. The new drugs have 4 transfer opportunities,generic drugs and imported drugs can transfer in opportunity 1,2,4. Different transfer opportunities present different risks and profits. The risk gradually decreases with the further promotion of drug registration process,while the innovation decreases at the same time. Pharmaceutical enterprises should combine with the policy,market and their own features to select a suitable transfer period.

Objective To explore the incidence and clinical treatment of related complications caused by implantable venous access port(IVAP) in patients with breast cancer during chemotherapy.Methods The data of 755 patients with breast cancer recieved chemotherapy by which caused some related complications in our hospital from January 2014 to March 2016 were retrospectively analyzed.Results 753 patients IVAPs were implanted succussfully.The total placement time of implantable venous access port was from 110 days to 940 days,with median placement 147.33 days.The related complications of IVAP were catheter malposition(0.79％,6/755),catheter-related thrombosis(27.81％,210/755),catheter fracture(0.13％,1/755),port exposure(0.93％,7/755) and IVAP-related bloodstream infection(0.13％,1/755).The IVAP-related complications and thrombosis rate were significant higher when IVAPs implanted in the left internal jugular veincompared with that in right internal jugular vein(34.88％ vs.25.74％,33.10％ vs.24.68％).Conclusion Application of IVAP in patients with breast cancer during chemotherapy is a safe and effective operation.The most common complication is asymptomatic mural thrombus formation around the catheter,which should be paid attention to.

Objective·To investigate the incidence, risk factors and treatment of the catheter-related thrombosis (CRT) in breast cancer patients after implantation of totally implantable venous access port (TIVAP) in chemotherapy. Methods·A total of 190 cases after implantation of TIVAP were investigated. Color Doppler ultrasound was used to monitor the neck blood vessels to find whether there was CRT before chemotherapy and before taking out the port. The incidence of CRT, occurrence time, risk factors and treatment efficacy were observed. Results·There were 112 (58.9%) cases with CRT and 108 (56.8%) patients with asymptomatic thrombosis, and only 4 cases had symptomatic thrombosis, the incidence of which was 2.1%. Most thrombosis developed on the 21th day after catheterization, and the patients over the age of 60, with clinical stage Ⅲ - Ⅳ and chemotherapy regimens TEC (docetaxel combined pirubicin and cyclophosphamide) were the risk factors for thrombosis. All the patients with asymptomatic thrombosis accepted anticoagulant treatment with low molecular heparin, earthworms enzyme or aspirin, respectively, but there was no significant difference in efficacy in the three groups (P=0.743). Conclusion·Port catheter related symptomatic thrombosis incidence is low but the incidence of symptomatic thrombosis is high in the breast cancer patients after chemotherapy. Age, tumor stage and TEC chemotherapy regimens are the risk factors for catheter-related thrombosis.
