Tracheopathia osteoplastica is a rare, benign pathologic condition chahacterized the occurrence of numerous cartilage and bony nodules within the tissue of mucosa. We report a case of tracheopathia osteoplastica that involved both larynx and trachea. The patient was 73 years old male and had hoarseness. The neck CT showed thickened and irregular wall of the trachea extending to the level of the carina with multiple calcified nodules and plaques of bony density projecting into the lumen.

4.A Case of Chronic Lymphocytic Leukemia with Trisomy 12 detected by Fluorescence in situ Hybridization (FISH).

We report a case of B-cell chronic lymphocytic leukemia (CLL) with trisomy 12 detected by FISH using chromosome 12 alpha-satellite Probe (Oncor , USA) in uncultured interphase cells. Chromosome studies did not produce an analyzable metaphase by standard short term culture and revealed only normal female karyotype by B cell mitogen (phorbol 12-myristate 13-acetate) stimulated 96 hr culture. The patient, a 59-year-old female, did not have hepatomegaly, splenomegaly, lymphadenopathy and any other symptoms. The peripheral blood of the patient showed marked lymphocytosis (WBC : 28,300/microL, Lymphocyte: 80%) and the diagnosis by immunophenotyping was B cell CLL:CD5, CDl9, CD2O, SmIg, HLA-DR positive.

6.The causes of Unsatisfactory Unaided visual Acuity after Exximer Laser PRK for Myopia.

we analyzed data from 102 patients(152 eyes) who underwent photorefractive keratectomy to investigate the postoperative causes of unsatisfactory unaided visual acuity. A VISX 20/20 exicimer laser(VISX, Inc, Santa Clara, DA USA, version 4. 01) was used in all cases by one surgeon(J.W.H). The patients who showed unsatisfactory unaided vision, less than 20/40 uncorrected visual acuity, with minimal follow-up periods of 6 months were included. A decreased unaided visual acuity was detected in 20 eyes(13%). Range of preoperative myopia was -2.50 to -14.00 diopters. Mean postoperative uncorrected visual acuity(geometric mean)was 0.33+/-0.12 and mean corrected visual acuity was 0.83 +/-0.25. Multiple factors contributed to unsatisfactory unaided vision; myopic regression in 8 eyes, irregular astigmatism in 4 eyes, overcorrection in 1 eye, severe corneal opacity in 1 eyes(2.6%) and the causes were as follows; irregular astigmatism, decentration combined with central island and severe corneal opacity. In conclusion, refractive results after photorefractive keratectomy were reasonably predictable and stable, but some sight threatening complications occured in a minority eyes. Improved techniques such as centering procedures and improved excimer laser program will be essential to prevent unsatisfactory unaided vision.

8.A Comparison of Cryotherapy Versus Transscleral Diode Laser Photocoagulation for the Treatment of Retinopathy of Prematurity.

PURPOSE: To determine whether transscleral diode laser photocoagulation is as effective as cryotherapy for the treatment of retinopathy of prematurity (ROP). METHODS: Thirty-six eyes of 18 infants with acute ROP were treated with cryotherapy and 32 eyes of another 16 infants with acute ROP were treated with transscleral diode laser photocoagulation. RESULTS: The transscleral diode laser photocoagulation group had 94% favorable outcome, whereas the cryotherapy group had 78% favorable outcome. CONCLUSIONS: The results suggest that transscleral diode laser photocoagulation is as effective as cryotherapy for the treatment of retinopathy of prematurity.

9.A Randomized Double - Blind Clinical Trial of 2,000cc vs. 4,000cc Preparation with Balanced Lavage Solution (Golytely) for Colonoscopy.

To determine the most effective and tolerable method of preparation with Golytely for colonoscopy, 141 consecutive cases were randomly allocated to drink either 2,000cc(74 cases, Group I) or 4,000cc(67 cases, Group 2), with infusion rate of 1,000cc/h. The reaults were as follows. 1) The rates of effective colonic clearance in group 1 and group 2 was 94.6% and 94.0%, respectively. There is no significance difference between two groups. 2) There was no significant difference in the ingested volume until the first bowel movement and the time required for rectal efflunt to be clear between two groups. 3) Patient acceptance was higher in group 1 than in group 2. 4) Side effects were less noticed in group 1 than group 2, and incraased in body weight (average 0.8 kg) after ingestion of Golytely was observed only in group 2.

10.Endoscopic Prosthesis in Malignant Stricture.

Non-operative palliative treatment for malignant colonic obstruction can sometimes be accomplished by the insertion of anorectal tube, endoscopic balloon dilation or endoscopic laser therapy. But these methods have some disadvantages, such as limitaation of activity, need of repetitive treatment and high-risk of perforation. Endoscopic prosthesis is generally accepted as a safe, effective palliative treatment for malignant esophageal stricture, because this method has no above disadvantages. Neverthless, there is only a few experence with endoecopic prosthesis in malignant colorectal stricture over the world. We report two cases which were safely, effectively performed endoscopic prosthesis in palliative treatment for their malignant rectal strictures.
