Tracheopathia osteoplastica is a rare, benign pathologic condition chahacterized the occurrence of numerous cartilage and bony nodules within the tissue of mucosa. We report a case of tracheopathia osteoplastica that involved both larynx and trachea. The patient was 73 years old male and had hoarseness. The neck CT showed thickened and irregular wall of the trachea extending to the level of the carina with multiple calcified nodules and plaques of bony density projecting into the lumen.

4.A Comparison of Cryotherapy Versus Transscleral Diode Laser Photocoagulation for the Treatment of Retinopathy of Prematurity.

PURPOSE: To determine whether transscleral diode laser photocoagulation is as effective as cryotherapy for the treatment of retinopathy of prematurity (ROP). METHODS: Thirty-six eyes of 18 infants with acute ROP were treated with cryotherapy and 32 eyes of another 16 infants with acute ROP were treated with transscleral diode laser photocoagulation. RESULTS: The transscleral diode laser photocoagulation group had 94% favorable outcome, whereas the cryotherapy group had 78% favorable outcome. CONCLUSIONS: The results suggest that transscleral diode laser photocoagulation is as effective as cryotherapy for the treatment of retinopathy of prematurity.

5.Endoscopic Enucleation of a Gastric Lipoma by Strip Biopsy.

Gastric lipomas are rare benign submucosal tumors which can present the diagnostic and therapeutic problems. The preferred treattment is observation or local excision. Since preoperative diagnosis and differentation from malignant tumors can be difficult by use of the conventional diagnostic modalities such as X-ray or endoscopic examination, patients are sometimes subjected to more extensive surgical procedures than warranted. As to the diagnosis of submucosal tumors of the upper digestive tract, a newly developed diagnostic modality of endoscopic ultrasonography(EUS) allows us to visuialize the structures underlying the gastrointestinal wall in a noninvasive maneuver, and can contribute to make differential diagnosis and decision of management. Strip biopsy is an endascopic tissue resection technique which permits resection of both mucosal and submucosal tissue regardless of the morphological type of the lesion, because the submucosal saline injections during the procedures make the flat or depressed lesions to be elevated. Now, we report a 56-yr-old female which had a Yamada type I polypoid gastric submucosal lipoma in the antrum, which could be performed the different diagnosis and successful endoscopic removal by endoscopic ultrasonography and strip biopsy.

A new method for combining ultrasonography snd endoscopy, endoscopic ultrasonography (EUS), was developed to improve the diagnostic accuracy of ultrasound by direct image of the target lesion via the gastrointestinal lumen. EUS has the unique ability to provide clear sonographic images of the wall structure of the stomach. This capability can be ultilized in assessing the depth of cancer invasion of the stomach by identifying which layer the ultrasonographic change extends to. However, the depressed types of early gastric cancer(EGC) are often accompanied by ulcerative change in the tumor foci. And the ultrasonographic picture is also influenced by the ulcerative change. 7.A Case of Chronic Lymphocytic Leukemia with Trisomy 12 detected by Fluorescence in situ Hybridization (FISH).

We report a case of B-cell chronic lymphocytic leukemia (CLL) with trisomy 12 detected by FISH using chromosome 12 alpha-satellite Probe (Oncor , USA) in uncultured interphase cells. Chromosome studies did not produce an analyzable metaphase by standard short term culture and revealed only normal female karyotype by B cell mitogen (phorbol 12-myristate 13-acetate) stimulated 96 hr culture. The patient, a 59-year-old female, did not have hepatomegaly, splenomegaly, lymphadenopathy and any other symptoms. The peripheral blood of the patient showed marked lymphocytosis (WBC : 28,300/microL, Lymphocyte: 80%) and the diagnosis by immunophenotyping was B cell CLL:CD5, CDl9, CD2O, SmIg, HLA-DR positive.

9.Familial Exudative Vitreoretinopathy in Three Families.

PURPOSE: Familial exudative vitreoretinopathy (FEVR) is a vitreoretinal dystrophy which has multiple modes of inheritance and variable clinical expressions. Thus it is important to screen the whole familial members carefully. We have observed 7 patients of 3 unrelated families. METHODS: To rule out other diseases, whole members were examined by indirect ophthalmoscopy with scleral depression or slit lamp biomicroscopy with Goldmann three mirror. When possible, fluorescein angiography was done. RESULTS: All 7 patients had no history of prematuriry, low birth weight and oxygen supplementation. They exhibited various clinical expressions of the disease process. Two patients showed fibrovascular masses with dragged disc and tractional retinal detachment. The others showed only avascular retina in temporal periphery. It was difficult to identify the mode of inheritance because we could not examine enough members.

10.A Randomized Double - Blind Clinical Trial of 2,000cc vs. 4,000cc Preparation with Balanced Lavage Solution (Golytely) for Colonoscopy.

To determine the most effective and tolerable method of preparation with Golytely for colonoscopy, 141 consecutive cases were randomly allocated to drink either 2,000cc(74 cases, Group I) or 4,000cc(67 cases, Group 2), with infusion rate of 1,000cc/h. The reaults were as follows. 1) The rates of effective colonic clearance in group 1 and group 2 was 94.6% and 94.0%, respectively. There is no significance difference between two groups. 2) There was no significant difference in the ingested volume until the first bowel movement and the time required for rectal efflunt to be clear between two groups. 3) Patient acceptance was higher in group 1 than in group 2. 4) Side effects were less noticed in group 1 than group 2, and incraased in body weight (average 0.8 kg) after ingestion of Golytely was observed only in group 2.
