2.The Expression of L-type Calcium Channel mRNA by the Concentrations of Glucose on the Cell Proliferation in Cultured OLETF Rat Aortic Vascular Smooth Muscle Cells.

BACKGROUND: The proliferation of vascular smooth muscle cell(VSMC) is a part of the major pathogenic mechanism for atheroscle- rosis. It has been reported that L-type calcium channel plays a role in the VSMC proliferation in diabetic rats. But there is a little study results about the association between L-type calcium channel and VSMC proliferation by glucose concentrations in culture media. So we examined the association between voltage-dependent L-type calcium channel of VSMCs and the proliferative activity of vascular smooth muscle cells. METHODS: Rat aortic VSMCs were isolated from the aorta of OLETF rat by enzyme method. VSMCs were cultured in various concentrations of glucose(5.5, 25 mM). The VSMCs(1x104 cells in 24-well plates) were incubated in the presence of Bay K 8644 (10-6M) with/without verapamil(10-6M) for 48 hours. Then the proliferation was assessed by MTT(methylthiazole tetrazolium) assay and expression of L-type calcium channel mRNA was measured by RT-PCR. RESULTS: The proliferative ability and the expression of L-type calcium channel of cultured VSMCs were increased dose-dependently by the glucose concentrations(p<0.05). Bay K 8644 enhanced the proliferation of VSMC and verapamil blocked the incremental effects induced by Bay K 8644. CONCLUSION: These results suggest that L-type calcium channel may play a role in VSMC proliferation of OLETF rat.

3.Left-Sided Appendicitis: A Case Report.

Frequent anomalies causing a left-sided appendix are situs inversus and malrotation. The ectopic position of the appendix frequently causes a delay or error in diagnosis because of atypical symptoms. A physical examination is of limited value unless dextrocardia is present. A left-sided appendix is a rare anomaly, but when other diseases are ruled out, it must be considered especially at the emergency department.

5.Barriers to dietary practice adherence among the elderly diabetes.

BACKGROUND: Some research viewed that effective dietary therapy was enough to control proper blood glucose level, but in the most patient, dietary therapy was not practiced and the most difficult part of managing their diabetes. The purpose of this research study was to investigate dietary practice adherence and perceived barriers among the elderly diabetes. METHODS: The survey was mailed to 852 persons with diabetes member via diabetic educator of 156 hospitals or clinics and 24 health centers. Questionnaire had background information of patients, meal regularity, food intake as a dietary practice adherence, barriers of 36 items which have 3 areas such as motive/attitude, knowledge, authority/resource. We asked the person with diabetes to rate barrier to dietary practice adherence. 432 questionnaire were returned the response. we selected 69 persons who were over 65 year old. RESULTS: 1) meal regularity was more satisfactory than food intake. 2) deficit of meal regularity were evening snack and resonable spacing between evening meal and evening snack. 3) deficit of food intake was serving of milk. 4) main barrier to dietary practice adherence was that of knowledge. CONCLUSION: For practice effectively dietary therapy to elderly diabetes, individual consultation or small group education must be pursued and more easily educational skills should be required.

6.Acquired Tufted Angioma (A Case of Acquired Tufted Angioma).

Acquired tufted angioma is a benign, slowly progressive angioma with a characteristic histologic pattern, that was first described by Wilson Jones in 1979. We present herein a case of this rare disease developed in a 43 year old healthy woman. Routine biopsy specimens and an immunoenzymatic staining of factor 7IE showed compatible findings with those cases reported previously. The lesions were surgically excised, and during the two months of follow up, there was no sign of recurrence.

Primary malignant melanornn of the vagina is rare, with only about 130 reported cases worldwide. They presumably arise from melanocytes that are present in the vagina in 3% of normal wornen. Vaginal melanoma accounts for less than 1% of melanomas and less than 3% of primary malignant tumors of the vagina. Most of patients complain vaginal bleeding, vaginal discharge, foreign body sensation in order. The best treatment of vaginal melanoma rernains eni.gmatic, Many authors indicate that radical surgery may be the pcferred approach and the number of reported cases treated with radical surgery is increasing. But other author comment that no significant change in survival is apparent with radical surgery. Overall prognosis is poor as most patients have deeply penetrating lesions at the time of diagnosis. Recently, not only the effort to increase the survival rate but the quality of life including sexual function after treatment is becoming an important issue about treatment ot cancer patients. We experienced a case of primary malignant melanoma of the vagina treated with radical surgery and restored the sexual function by vulvovaginal reconstruction using gracilis myocutaneous flap. So we report this case with brief literature review.

8.The Diagnostic Efficacy of Abdominal Ultrasonography for Evaluation of Children with Urinary Tract Infection.

For the evaluation of children with urinary tract infection, the effective diagnostic approach with appropriate imaging studies may be one of the most important process. But there is no definitely standardized method to evaluate these children. The author analysed the radiologic findings on 88 consecutive children with recurrent urinary tract infection to investigate the diagnostic efficacy of abdominal ultrasonography compared to conventional excretory urography and determine logical sequence of uroradiologic study in children with urinary tract infection. The results were as follows: 1 Of all the children studied, there were 37 cases of vesicoureteral reflux (VUR) with or without another urologic abnormalities such as posterior urethral valve and double collecting system, 32 cases of ureteropelvic junction obstruction (UPJ obstruction) and 1 case of multicystic kidney. 2. All cases with UPJ obstruction and multicystic kidney were diagnosed with abdominal ultrasonography. But excretory urography did not detect 2 cases of mild UPJ obstruction due to intermittent hydronephrosis 3. Of 37 cases with VUR, 3 cases were not detected with abdominal ultrasonography and 5 cases were not detected with excretory urography 4. Of all the cases studied, 18 cases did not have any anatomical abnormalities. In these cases, abnormal findings on excretory urography and ultrasonography were detected in 2 and 3 cases, respectively 5. In the evaluation of anatomical abnormalities in the patient of recurrent UTI, combined radiologic study of ultrasonography and voiding cystourethrography has the same efficacy as excretory urography and voiding cystourethrography. These results suggest that abdominal ultrasonography may be more effective than excretory urography in the evaluation of children with urinary tract infection. And both ultrasonography and excretory urography are relatively insensitive for detecting reflux and its sequel. So in the evaluation of children with urinary tract infection, voiding cystourethrography and ultrasonography should be performed initially. And excretory urography could be performed for selected cases.

9.A Case of Fox-Fordyce Disease.

Fox-Fordyce disease is a chronic pruritic papular eiruption, usually appearing in women and presenting a strict localization to the areas bearing apocrine sweat glands. More than 90g, of the cases were seen in women, the majority being in the age group of 13 to 35 years. The present report deals with unusual case of 14-year-old boy who developed Fox-Fordyce disease 6 years ago. To our knowledge, it is extremely rare case in Korea.
