1.Occurrence of Anthracnose on English Ivy Caused by Colletotrichum trichellum in Korea.

Anthracnose symptoms severely occurred up to 50% on leaves of English ivy growing in greenhouses in Cheongwon area of Korea during disease survey in April, 2000. The symptoms developed as concentric spots with dot-like acervuli on leaves of the plant. A total of 24 isolates of Colletotrichum sp. were obtained from the spotted lesions and identified as Colletotrichum trichellum based on the morphological and cultural characteristics. Leaf spots similar to the original anthracnose symptoms were induced on the host leaves by artificial inoculation with the isolates of the fungus. This is the first record of C. trichellum causing anthracnose of English ivy in Korea.

3.Esophagus, Stomach & Intestine; Evaluation of Endoscopic Ultrasonography in Gastric Carcinoma.

BACKGROUND: Endoscopic Ultrasonography(EUS), a combination of endoscopy and ultrasonography is one of the most useful tools for diagnosis of digestive system diseases. EUS is useful in staging the primary tumor invasion and regional lymph node metastasis because of its ability to image the gut wall and adjacent structures in unique detail. We performed a study preoperatively for assessing the accuracy and limitation of EUS in gastric carcinoma. METHODS: In 75 patients(Male 54, Female 21, Mean age 62.3 years) with a gastric carcinoma confirmed by endoscopic biopsy, we performed EUS preoperatively. The results were compared with the postoperative histopathologic staging. RESULTS: 1) In assessing the depth of tumor invasion, EUS accuracy on depth of invasion was 76.0% and overstaging and understaging were 14.7% and 9.3%, respectively. 2) In assessing the lymph node metastasis, the accuracy rate of EUS was 65.3%. A rate of false-positive diagnosis was 27.3%, and a rate of false-negative diagnosis was 24.5%.3) In assessing the incidence of lymph node metastasis for each T stage, the involve rate was 10.5% in pT1, 36.4% in pT2, 95.7% in pT3, 100% in pT4. CONCLUSIONS: EUS is the most accurate diagnostic methods presently available to determine tumor infiltration depth and lymph node metastasis.

4.Personal Privacy Protection of Electronic Medical Record in Health Information System.

OBJECTIVE: The consolidation of health information has led to the creation of a national network of medical institutions. In this wireless and hard-line communication web, patients' medical information, which is extremely private and must be kept in secret, may leak out, and the risk of dangers such as privacy infringement and misuse of information has increased. METHODS: For this dissertation, a patient information protection algorithm was designed for a hospital information system in order to improve the untraceability and anonymity of patients' individual information in a ubiquitous computing environment. RESULTS: Additionally, analyzing the computing load showed that the computing load of the entire system did not increase, and since 57 percent of the sensor node's computing load was transferred to the secure server, the sensor node's load was minimized. Thus, the system can also be applied to low performance sensor networks. CONCLUSION: In conclusion, improving the security level of identification information transmissions while minimizing the computing and communication load of sensor nodes in a ubiquitous computing environment did not lead to an increase in the system's overall computing or communication load. Thus, a system with improved security features for identification information and communication information was developed and established.

Cancer of the vulva accounts for approximately 0.5% of all gynecologic malignancies. At diagnosis, one-third of these cases is detected in an advanced stage (FIGO stages III, IV), and local extension of primary vulvar cancer may involve adjacent midline structures such as the clitoris, urethra, vagina, and anus. Initial surgical therapy of such locally advanced primary cancers may compromise the functional integrity of midline structures, necessitating ultraradical surgery including pelvic exenteration. In view of the relatively elderly age of the patients and the morbidity of this ultraradical dissection, concomitant chemoradiation therapy - that the efficacy had been proven in head and neck cancer, anal cancer has approached for patients with locally advanced vulvar cancer. We experienced a case of stage III vulvar cancer patient, who underwent concomitant chemoradiation therapy with 5-fluorouracil(FU) and cisplatin and who showed complete response. So, we report this case with brief review of the literatures.

10.The Effect of Neoadjuvant PI(Cisplatin, Ifosfamide) and PAIB(Cisplatin, Adriamycin, Ifosfamide, Bleomycin) in the Cervical Cancer.

For cervical cancer, the combination chemotherapy could increase the trial of radical hysterectomy due to reduction in tumor volume even in advanced stage and help to decrease corrected dose of radiotherapy. It also reduces postoperative radiotherapy to avoid the side effect and therefore make the quality of life better. Cisplatin chemotherapy alone was known to result in objective response rates of 20%. Todays animal experiment studies stated that Ifosfamide could accelerate the efficacy of Cisplatin. Thus in this study, neoadjuvant chemotherapy was applied to those cervical cancer patients in the use of the combination chemotherapy including Ifosfamide, PI(Cisplatin+ Ifosfamide) and PAIB(Cisplatin+ Ifosfamide+ Adriamycin+ Bleomycin). A total of 43 patients were entered in this trial, 23(53%) achieved at least a 50% reduction in tumor volume, 20(47%) showed no change. After completion of combination chemotherapy, 30(70%) of the 43 were done radical hysterectomy, 13(30%) given to radiotherapy. 9(30%) of the 30 patients were applied postoperative radiation therapy. Nausea and vomiting occurred in 35 of the 43 patients(81%) and alopecia 15(34%) for toxicity. Severe CNS toxicity(Grade 4) was detected in one patient. This study suggest that Ifosfamide combination chemotherapy is effective in tumor reduction and minimal toxity and can performed radical operation in cases of large tumor volume.
