1.The Incidental Polyorchidism and Treatment: Report of 2 cases .

Herein, polyorchidism possessing of more than the usual number of testicles, which was found in two patients, is reported. Both patients had three testes, and one presented with painful swelling of the right scrotum caused by torsion of the testis and the other revealed a non-palpable testis in the right hemiscrotum. Both patients were managed surgically with orchiopexy and an orchiectomy, respectively, followed by the insertion of an artificial testis.

2.The Effect of Buffered Versus Non - Buffered Lidocain as an Additive to Triamcinolone Acetonide Suspension on Intralesional Injection for Alopecia Areata.

BACKGROUND: Intralesional injection of corticosteroids has widely been used for the t,reatment of alopecia areata. The pair produced during the repeated multiple injection procedures is an actual therapeutic obstacle in both adult and child patients. Buffering of the local anesthetics is known to reduce the pain. OBJECTIVE: The purpos; of this study was to evaluate the degree of pain reduction and hair regrowing effect by the use of buffered lidocain-triamcinolone acetonide suspension(BLT) compared with nonbuffered lidocairi triamcinolone acetonide suspension(LT) in the intralesional injection of alopecia areata. METHODS: We evaluat d the degree of pain reduction by the use of BLT compared with LT in the intralesional injection in 40 cases of alopecia areata. The pain was ineasured by patients verbal pain score from zerc to ten and simultaneously observed if there were be any differences in hair regrowing bet,ween the two groups. RESULTS: 1. Remarkable reduction of the pain du~ring the inject,ion was noted by the buffering agent and the pain was further reduced by the vertical insertion of the needle. The pain score of BLT was 1.5 by vertically inserted injections and 2.4 by ~30 degree angle injections while the pain score of LT was 4.4 by the vertical and 5.6 by t.he 30 degree, Z. The mean time for appearance of regrowing hairs was 3.6 weeks in the BLT area and 4.0 weeks in the LT area. The use of BLT area showed a statistically faster response than the LT area. CONCLUSION: The use ot BLT is recommended not only by it,s remarkable effect on pain reduction but also by its sigriificantly accelerated hair regrowing effect in the trea~tment of alopecia areata.

6.A Study on the Importance and Performance of Foodservice Tasks between Dietitian from Hospitals and Contract Managed Foodservice Companies.

This study compared he importance of food service, views to the performance, management objectives, and recognition gap about the contract contents towards the dietetic departments of hospitals that manage the patients' food service and managers of contract managed companies, and dieticians. It conducted questionnaires and survey towards the vice director and dieticians of 17 hospitals, over 500 beds, and the persons responsible for contract managed companies and dietitians who were in charge of food service. The hospitals showed significantly(p<0.05) higher the importance scores in menu planning, the distribution of meal, sanitation management, and leadership than those of the contract managed companies. In the difference of hospitals and contract managed companies about performance, it appeared high in the contract managed companies. In the importance of the foodservice management objective, the hospital had significantly(p<0.05) higher scores in the management of client's satisfaction and quality improvement element through management innovation than those of the contract managed companies. In the importance of contract term, the contract method, expense, and payment condition of commission fee were significantly(p<0.05) high scores in the contract managed companies, compared to the hospital.

7.Extraperitoneal Bladder Rupture following a Normal Spontaneous Vaginal Delivery.

The urinary bladder is seldom injured by external forces, as it is located in the pelvic cavity, behind the symphysis pubis, and protected by the bony pelvis. Gynecological procedures are a major cause of bladder injury. Herein, a case of extraperitoneal bladder rupture, identified 6 days after vaginal delivery in a 27-year-old woman, with no history of cesarean section or pelvic surgery, and who was cured of the bladder rupture using conservative treatment with the indwelling of a urethral Foley catheter, is reported.

8.Biofeedback and Functional Electrical Stimulation Therapy for Patients with Intractable Chronic Pelvic Pain Syndrome.

PURPOSE: Chronic pelvic pain syndrome(CPPS) is a debilitating condition characterized by pelvic pain and voiding symptoms. Pelvic floor tension myalgia is hypothesized to be a contributing factor in CPPS. The therapeutic effect of biofeedback and electrical stimulation for CPPS was evaluated prospectively. MATERIALS AND METHODS: Thirteen patients with CPPS intractable to conventional medication were enrolled. The patients were treated by domestic KontinenceTM equipment 2 times a week for 6 weeks. Patients completed the National Institute of Health Chronic Prostatitis Symptom Index(NIH-CPSI) before and after treatment. RESULTS: The average score for all 3 domains in NIH-CPSI significantly decreased. Pain domain decreased from 12.9+/-3.4 to 9.8+/-1.7(p=0.002), voiding symptoms from 4.5+/-1.6 to 3.2+/-0.9(p=0.006), and impact plus quality of life from 7.3+/-2.2 to 4.7+/-1.5(p=0.003). CONCLUSIONS: Biofeedback with electrical stimulation therapy can provide significant improvement in CPPS intractable to medication. Large, randomized trials are needed to confirm its efficacy.

9.Laparoscopic Management of Complicated Urachal Remnant in a Child.

A few cases of laparoscopic surgery for urachal remnant in children has been reported in English literature. With recent developments in minimal invasive surgery, laparoscopic approach for urachal remnant in adulthood is recommended by some laparoscopic surgeons because of its technical feasibility and safety as well as cosmesis. Recently we experienced a case of complicated urachal remnant in a 14-month-old girl, who was managed by laparoscopic approach. At presentation, she complained of high fever and lower urinary tract symptoms. After 6 weeks of antibiotics therapy, laparoscopic surgery was performed transperitoneally via 3 ports. Our experience suggests that laparoscopic excision of urachal remnant can be performed easily and safely in children.

10.The Clinical Significance of Serum C-reactive Protein in Patients with Acute Uncomplicated Pyelonephritis.

PURPOSE: To evaluate the predicted results of treatment and to decide the time of using parenteral antibiotics to oral medication in Acute Uncomplicated Pyelonephritis, the serum C-reactive protein (CRP), WBC counts and body temperature (BT) were compared. MATERIALS AND METHODS: Between May 2003 and August 2004, 59 female patients with acute uncomplicated pyelonephritis were enrolled. The serum CRP, WBC counts and BT were measured on the first, third, fifth and seventh day after admission, and on the seventh day following discharge. The durations and patterns of the serum CRP, WBC counts and BT were statistically assessed after the use of parenteral antibiotics. RESULTS: The serum CRP, WBC counts and BT were increased in all patients on the first hospital day, but only the serum CRP was statistically correlated with the clinical course, with an average level of 11.83mg/dl. After the use of parenteral antibiotics, the mean serum CRP decreased to 8.38, 4.00 and 1.92mg/dl on the third, fifth and seventh day after admission, respectively. There were no cases increased serum CRP level or recurrence of inflammation during the follow-up period. The initial serum CRP level was also correlated with the disease severity when over 15mg/dl, the longer use of parenteral antibiotics and the longer stay in hospital were needed. CONCLUSIONS: Clinically, the level of serum CRP was a better indicator in determining the therapeutic effect of parenteral antibiotics use in patients with acute uncomplicated pyelonephritis than either the WBC counts or BT. Especially, in patients whose serum CRP level was decreased below 2.00mg/dl during the early days of the hospital stay, a decreased use of parenteral antibiotics and decreased hospital stay were observed. The initial serum CRP level was also correlated with disease severity.
