OBJECTIVE:
To explore the clinical application value of using needle electrode in transurethral plasmakinetic resection of bladder tumor around ureteral orifice.
METHODS:
Retrospective analysis was performed on the clinical data of 16 cases who had bladder tumors around ureteral orifice and underwent transurethral resection using plasmakinetic needle electrode in Department of Urology, Peking University International Hospital from June 2015 to December 2019. There were nine cases with the tumor of one to two centimeters from the ureteral orifice. The rest of the seven cases had tumor that was within one centimeter from the ureteral orifice, including two cases whose ureteral orifice was invaded by the tumor. All the patients studied were diagnosed before surgery and contraindications were excluded. The plasmakinetic needle electrode was used to treat the tumor with en bloc resection, and all the excised tissue was sent for pathological examination. Intravesical chemotherapy and postoperative follow-ups were performed. Statistical analysis was performed on the operation time, the incidence of obturator nerve reflex, the peri-operative bleeding, the parameters of indwelling ureteral catheter or double-J stent, the incidence of postoperative hydronephrosis, the clinical stage of tumor, and the recurrence rate.
RESULTS:
The operation was successfully completed for all the sixteen cases. The operation time was 16 to 57 minutes, with an average of (32.6±11.8) minutes. No obvious obturator nerve reflex and perioperative bleeding occurred in all the patients. Ureteral catheters were indwelled prior to the operation of tumor resection in seven cases. Four of the seven cases had the ureteral catheters remained while the rest three were replaced by double-J stent after surgery. Postoperative pathological analysis showed that all the tumors were urothelial carcinoma, including 9 cases of low grade and 7 cases of high grade. Pathological staging: 10 cases were in Ta stage, 5 cases in T1 stage, and 1 case in T2a stage. All tumor bases and lateral margins were negative. All the patients received 3-56 months, with an average of (26.0±18.1) months of follow-up. There was no case of upper urinary tract hydronephrosis or tumor recurrence.
CONCLUSION
The transurethral plasmakinetic resection of bladder tumor using needle electrode can realize en bloc tumor resection without obturator nerve reflex and reduce the risk of ureteral orifice injury. It is a safe and effective surgical method for treating bladder tumors around the ureteral orifice.

Objective To investigate the effect of diazepam on blood-brain barrier ( BBB ) after radiation-induced brain injury. Methods Two hundred and forty healthy male SD rats weighing 240-270 g were randomly divided into 4 group with 60 animals in each group: Ⅰ D-R group received intraperitoneal diazepam 5 mg?kg-1 30 min before irradiation; Ⅱ N-R group received no diazepam before irradiation; Ⅲ D-SR group received intraperitoneal diazepam 5 mg? kg-1 30 min before sham irradiation and Ⅳ N-SR group received no diazepam before sham irradiation. The animals were placed in a prone position awake without any anesthetics. The whole brain was irradiated by 4 MeV electron beam at a single dose of 30 Gy. The changes in BBB were assessed by (1) magnetic resonance imaging (MRI) with the T1-weighted images, T2-weighted images and MRI enhancement images induced by Gd-DTPA; (2) Evans blue (EB) assay; 2% EB 3 ml?kg-1 was injected via femoral vein and 3 h later the animals were decapitated and EB content in the brain tissue was determined by spectrophotometry; (3) The expression of vascular endothelial growth factor (VEGF) in the brain tissue determined by immuno-histochemistry before irradiation ( T0 ) ( baseline) at 6 h (T1 ), 1 day ( T2 ) , 1 week ( T3 ) and 1 month ( T4 ) after irradiation. Results In group Ⅱ (no diazepam before irradiation) the signal intensity of T1-weighted images significantly decreased while the signal intensity of T2-weighted images and the enhancement rate significantly increased at T4. However in group Ⅰ (diazepam was given before irradiation) the changes in the MRI were significantly attenuated. The EB content at T1, T3 , T4 and the expression of VEGF at T1 , T, were significantly higher in group Ⅱ than in group Ⅰ . Conclusion Diazepam given before irradiation can protect BBB against irradiation injury by decreasing the expression of VEGF in the brain tissue and BBB permeability.

Objective To observe the Leu72Met single nucleotide polymorphism (SNP)of ghrelin gene and the relationship with essential hypertension (EH).Methods Polymerase chain reaction -restriction fragment length polymorphism (PCR -RFLP)was used to detect the Leu72Met SNP of ghrelin gene in 210 EH patients and 220 healthy controls. The plasma ghrelin was detected by radioimmunoassay method collected from all subjects. Results There were three types of polymorphism of ghrelin gene at the base site Leu72Met.There were significant differences in the genotypes (CC,CA,AA)and alleles (C,A)between the EH patients and the controls (χ2 =6.054,P =0.048;χ2 =5.866,P =0.015).In EH group, the plasma ghrelin level in subjects who were homozygous CC without mutant was not only significantly lower than those who were heterozygous CA,but also lower than those who were nucleotide homozygous mutant AA (t =-8.738,P =0.000;t =-5.103,P =0.000).The patients with CC genotype had higher SBP (t =4.298,P =0.000;t =2.236,P =0.019)and lower HDL -C (t =-11.682,P =0.000;t =-7.872,P =0.000).The patients with A allele had lower plasma ghrelin (t =-16.264,P =0.000), HDL -C (t =-15.332,P =0.000)and higher SBP(t =3.800,P =0.000),DBP(t =11.895,P =0.000),and LDL -C (t =38.401,P =0.000).Conclusion The Leu72Met SNP of ghrelin gene is significantly related to the susceptibility of EH.Base mutation C to A reduced the incidence of EH.The Leu72Met polymorphism of ghrelin gene is related to the plasma ghrelin, blood pressure and blood lipid metabolism.Base mutation C to A elevated plasma ghrelin, and lowered blood pressure and blood lipid.
