Delirium is described as a manifestation of acute brain injury and recognized as one of the most common complications in intensive care unit (ICU) patients. Although the causes of delirium vary widely among patients, delirium increases the risk of longer ICU and hospital length of stay, death, cost of care, and post-ICU cognitive impairment. Prevention and early detection are therefore crucial. However, the clinical approach toward delirium is not sufficiently aggressive, despite the condition's high incidence and prevalence in the ICU setting. While the underlying pathophysiology of delirium is not fully understood, many risk factors have been suggested. As a way to improve delirium-related clinical outcome, high-risk patients can be identified. A valid and reliable bedside screening tool is also needed to detect the symptoms of delirium early. Delirium is commonly treated with medications, and haloperidol and atypical antipsychotics are commonly used as standard treatment options for ICU patients although their efficacy and safety have not been established. The approaches for the treatment of delirium should focus on identifying the underlying causes and reducing modifiable risk factors to promote early mobilization.

In this study, the effect of kanamycin sulfate was compared with fortified penicillin G in the treatment of uncomplicated penicillinase negative Neisseria gonorrhoeae(nonPPNG) urethritis in male. The subjects were 250 male patients with uncomplicated non-PPNG urethritis, at the Venereal Disease Clinic of Choong-ku Public Health Center in Seoul from May 1982 to August 1982. Among 103 patients treated with kanamycin sulfate, 2.0 gm, IM, 92 recovered and 11(10. 7%) failed. Among 100 patients treated with fortified penicillin G, 6 megaunits IM, plus probenecid, l.0 gm, 92 recovered and 8(8%) failed. No significant difference in the effect was found between these two regimens for non-PPNG urethritis. It is concluded that kanamycin sulfate as well as fortified penicillin G. have a good effect in the treatment of non-PPNG urethritis.

The authors present 2 cases of squamous cell carcinoma which occurred primarily in the eyelid. One is a 33-year-old Korean female who had the carcinoma in the right upper palpebral Conjunctiva, and the other is a 75 year-old Korean female who had the carcinoma at the margin of the left upper eyelid. These two cases were more likely to be confused with chalasion and were confirmed to be squamous cell carcinoma by the gistopathological examination.

Authors observed two cases in which the formation of glass-like hyaline membranes in the anterior chamber caused by birth trauma. Case 1 revealed a curtain-like glass membrane in the anterior chamber with a central nubecula corneae of right eye (Fig. 1). Case 2 revealed two paralled glass membranes and its stripes extended across the anterior chamber attached at each extremity to the posterior surface of the cornea (Fig. 2).

Distal foot defect is frequently encountered as an output site in high-tension electrical burn injuries, and proper treatment methods should be applied in order to achieve a properly-functioning foot. However, in distal foot defects around the metatarsal heads or metatarsophalangeal joints, conventional methods have not been ideal, replaced by such methods as secondary healing, amputation or skin graft because there was insufficient tissue in the distal foot and the general condition of the patient was unsuitable for distant or free flap surgeries. So, in order to establish convenient, promising methods for the reconstruction of distal foot defects in severe electrical burns, we classified the distal foot into four zones according to their blood supply and we defined or created ideal reverse flaps in each zone. From April 1996 to March 1999, we reconstructed 43 cases of distal foot defects in fourth-degree high voltage electrical burn wounds. In 19 cases of MP(medial plantar) zone injuries, we used reverse medial plantar island flap and in 5 cases of DP(dorsalis pedis) zone, we created a reverse dorsalis pedis island fascia flap, In 11 cases of P(peroneal) zone, we created reverse peroneal fascia flap and in 8 cases of I (intermediate) zone, we modified a reverse posterior tibial island flap, There was partial flap necrosis or grafted skin loss in five cases, but those were extremely minor problems and we successfully reconstructed distal foot defects in all 43 cases. In conclusion, proper reverse flaps according to the zone of the distal foot are ideal reconstructive methods because they provide good vascularization, sufficient tissue from the proximal parts and they require short operative time with relatively easy procedures.

Although penicillin still remains the drug of choice in the treatment of gonorrhoea in many countries, treatment failures are becoming more and more coramon. Recently, the prohlem cauaed, by antibiotic resistant strains of N. gnorrhoeae has been aggravated by appearance of penicillinase producing N. gonorrheeae. Good results were reported with the use of spectinomycin, cefuroxime and efotaxime. These drugs are, as yet, quite expensive. A lower cost regimen has to be sought. In this study, the effect of fortified penicillin G was compared with kanarnycin in the treatment of uncomplicated male gonorrhoeae. The subjects were 446 rnale patients with uncomplieated gonocoacal urethritis at VD clinic of ChoongKu Public Health Center in Seoul between June and November, 1981. The patients are assigned randomly to one of the regimens. Among 181 patients treated with kanamycin, 2. 0 gm, IM, 148 recovered with 27 (19%) postgonococcal urethritis (PGU) and 39(21,6%) failed.
