3.Bile Duct Stenosis & Intrahepatic Stones after a Transcather Hepatic Arterial Embolization: A case report .

Transcatheter hepatic arterial embolization (THAE) is one of the treatment modalities that can be applied to hepatocellular carcinomas (HCC) and metastatic tumors of the liver. Complications such as cholecystitis and gallbladder necrosis, are common with THAE, but liver and peripheral bile duct necrosis are rare, and intrahepatic stones with main bile duct necrosis have never been reported. To prevent intrahepatic spread during operative manupulation and to decrease the vascularity and size of the tumor, we performed a THAE on a huge-sized HCC five times before performing the hepatectomy. We succesfully undertook a right lobectomy after the THAE with lipiodol, gelform, and adriamycin. However, severe bile duct stricture and intrahepatic stones were confirmed during the operation.

We report a case of hepatoma with duodenal metastasis in a 53 year-old male patient. Hepatoma was confirmed by fine needle aspiration cytology technique, and duodenal metastasis by gastrofiberscopic biopsy. Duodenal metastasis of hepatoma is rare. We briefly review the role of fine needle aspiration cytology technique in diagnosis of hepatoma.

6.Effect of Ca Supplementation on Serum Ca Level and Psychological Conditions in Perimenopausal Women.

This study was conducted to investigate the relationship of serum calcium and magnesium level to depression and anxiety symptoms in 66 perimenopausal women. Daily nutrient intakes and dietary sources of calcium were analyzed by convenient method. General status was conducted by a questionnaire whereas the questionnaire of CED-S(the Center for Epidemiological studies-Depression Scale) was used for depression and Spielburger's STAI-S(state-Trait Anxiety Inventory-State) was used for anxiety. Fasting blood samples were collected, and serum calcium and magnesium concentrations were measured before and after calcium supplementation. The age distribution of the subjects was 49-55 years. Results indicated that serum calcium concentrations were significantly(P<0.05) increased to normal ranges after calcium supplementation. Depression and anxiety scores of the subjects with calcium supplementation were significantly(p<0.05) lower than those before calcium supplementation. There were significantly(P<0.05) decreased between serum magnesium concentration and depression and anxiety scores, but calcium concentration was not significantly decreased. These results suggest that psychological conditions of perimenopausal women are possibly effected by serum calcium and magnesium levels. More studies are needed to measure the long-term effects of calcium supplementation on psychological conditions in perimenopausal women.

8.Clinical Analysis of Cranial Nerve Injuries in Craniocerebral Trauma.

The clinical analysis of cranial nerve injuries was performed on 435 cases with cranoicrerbral trauma. This prospective study included the correlation between cranial nerve injuries and risk factors such as intracranial hematoma, initial Glasgow Coma Scale(GCS) score, pneumocephalus, and other combined injuries. The results were revealed as follows : 1) 133 cranial nerve injuries(on 97 patients) were noted among 435 craniocerebral trauma victims(97/435=22.2%). 2) The order of frequent cranial nerve injuries was facial nerve(7.3%), olfactory nerve(6.9%), oculomotor nerve(4.4%), abducens nerve(3.9%), optic nerve(3.2%), etc. 3) Bilateral involvment of cranial nerve injuries was noted in 16.5%(22/133). 4) The incidence of immediate onset of cranial nerve injuries was 66.9%(89/133). 5) The incidence of cranial nerve injuries was significantly high in patients with pneumocephalus and low initial GCS score. 6) The functional recovery of injured cranial nerve within 3 months was noted in 30.1%(40/133).

9.Clinical Analysis of Interhemispheric Subdural Hemorrhage and Tentorial Hemorrhage.

A propecive analysis of 50 patients with acute interhemisphric subdural hemorrhage and tentorial hemorrhage, an unusual pattern of acute subdural hematoma, who were managed in a uniform way was analyzed to related outcome to hemorrhagic site, initial Glasgow Coma Scale(GCS) and combined injuries. The incidence of acute interhemisphric subdural hemorrhage(ISH) and tentoria hemorrhage(TH) after head trauma was 3.83%, 50 cases among 1303 head injured cases. And 80% of the above hemorrhage disapperaed within two weeks after trauma. There was no significant relationship between feature of hemorrhage and intial GCS(P>0.05), but there was highly significant relationship between initial GCS and Glasgow Outcome Scale(GOS) (p<0.001). There noted significant relationship between initial combined injury and GOS(p<0.01), and also brainstem injury and GOS(p<0.001).

10.Emphysematous Pyelonephritis in Diabetic Nephropathy A report of two cases.

Diabetic nephropathy is characterized by one or a combination of the following lesions: (1) glomerular involvement with three distinctive patterns: diffuse glomerulosclerosis, nodular glomerulosclerosis, and exudative lesions; (2) arteriolo sclerosis; (3) urinary tract bacterial infection with pyelonephritis and sometimes emphysematous pyelonephritis. Emphysematous pyelonephritis is an uncommon life-threatening and acute suppurative infection of the kidney, and usually occurs in diabetic female patients. It is characterized by the production of intraparenchymal gas. Glucose fermentation has been considered the main cause of the gas formation. We presented two illustrative nephrectomy cases of emphysematous pyelonephritis in addition to the typical pathologic features of diabetic nephropathy.
