1.A Case of Norwegian Scabies in a Patient with Cushing Syndrome.

We describe here a case of Norwegian scabies in a patient with Cushing syndrorne, the first case in Korea. He had complained of intermittent mild itching for 5 month. Physical examinations revealed the characteristic features of Cushing syndrome and hyperkeratotic brownish papules or/and patches were distributed on the dorsa of hands, axillae, groins, interdigital webs, upper part of the chest and around the neck. We could find numerous burrows and many mites, eggs, lavas and scybala in the affected sites on direct microscopic examination with 25% KOH. Significant laboratory findings were as follows: 1. Total eosinophil count: 0 2. 17-hydroxycorticoids in 24 hr. urine: 10. 1 mg/900 cc 3. Bone X ray series: generalized osteoporosis 4. Mantoux test: negative About 10 days after topical application of 25% benzyl-benzoate lotion combined with 5% sulfur ointment, the skin lesions were cleared. We suspect that the lack of delayed hypersensitivity might attribute to the development of Norwegian scabies in this patient.

2.A Study of Anctiviy with CAstsnea Crenata Flos.

There are many herbs used in the treatment of dermatophytosis as home medicine and Castanea crenata Flos (C.C.F.) is one of that used in Kyung gi-do district. With that regard we tried to examine in vitro antifungal activity of the C.C.F. with its water extract and alcohol extract as the test materials. Until now many authors have studied the antifungal activity with several plant extracts, as like the alcohol extracts of Zanthoxylum piperatum Fructus and Berberis Korea palibin or Fennel oil, and reported that these plant extrscts or essential oiI have good antifungal activities in vitro and in vivo. On that they said that the principal fractions those have strong fungistatic activity were all alikely the fatty acids chemically similar with the well known undecylenic acid. At this in vitro experiment the extracts of C.C.F. were diluted. serially in the Sabourauds dextrose agar and certain portions of fungi (the strains of Trichophyton (Tr.) mentagrophytes and Tr. rubrum) were transplanted into each medium containing different concentrations of test materials then observed its growth for 2 weeks at room temperature. After that we measured the sizes of colony grown in various conditions and compired with that of the normal medium to determine the fungistatic effectiveness. As the result, in the view of the colony sizes, Tr. mentagrophytes and Tr. rubrum equally revealed marked inhibition of its growth only in the media with water extract over the concentration of 10mg/ml in the mediurn, and the critical fungistatic dilution was 20 mg/ml. We could not identified the principal fraction in the water extract of C.C.F.which exhibited antifungal activity. In the comparison of the critical fungistatic dilution with the alcohol extract of Zanthoxylum piperaturn Fructus and Fennel oil, water extract of C.C.F. required tenfold higher concentration for the same effect. However in regarding the utilizing habits of C.C.F. in horne medicine, as topical wet dressing with water boiled solution, we might expect its effect practically. iVow there are problems to be investigated about the chemical natures and physical properties of the C.C.F. and for the antifungal fractions of it, in addition further in vivo study we thought would be interesting.

4.Treatment of Complicated Femoral Neck Fractures in Children by Quadratus Femoris Muscle Pedicle Bone Graft: A Preliminary Report of 4 Cases

The femoral neck fractures in children remain an unsolved problem because of lack of the optimal treatment followed by unpredictable, serious complications. Since the first trial of muscle pedicle bone graft by Judet(1962), Meyers et al(1973) refined and promoted this method for nonunion and as a primary procedure, and Baksi(1983, 1986) expanded its indication to post-traumatic avascular necrosis of the femoral head. Two cases of avascular necrosis and Two nonunion cases treated by quadratus femoris muscle pedicle bone graft at the Chonbuk National University Hospital from Jan. 1986 to Jun. 1988 were reported. The age of the patients varied from 6 to 16 years ; Follow-up ranged 6 to 29 months ; all cases were displaced transcervical fracture. In according to Ratliffs assessment, three cases showed satisfsctory results and one poor.

8.Significance of Microscopic Examination of Bile Directly Collected during Endoscopic Retrograde Cholangiography.

The usefulness of microscopic examination of pure bile directly collected from the biliary tract during endoscopic retrograde cholangiography and without hormonal stimulation was evaluated in 86 patients. Cholesterol monohydrate crystal and/or calcium bilirubinate granule were observed in the bile of 64% of GB stones, 88% of CBD stones and l00% of IHD stones. But only 4% of non stone control group were positive crystals examinations. According to this results, microscopic examinations:of bile samples collected during endoscopic retrograde cholangiography exhibited a sensitivity and a specificity for cholelithiasis recognition of 82.3% and 95.8%, respectively, with a positive and negative predictive value of 98.1% and 67.6%, respectively. We conclude that microscopic bile examinations of bile samples collected during ERC is useful to investigate in patients with suspected cholelithiasis.

9.Two Cases of Weber-Christian Disease.

Two cases of Weber-Christian disease are reported. This very rare disease, we had two another reports of case sincc last ten years in The Korean Journal of Dermatology. In our cases both patients are seventeen years old girls and have the histories of remissions and exacerbations for two to three years. The skin lesions of subcutaneous nodules are scattered on the trunk, extremities and even to the hand dorsurns, with the atrophic depressions. Histologic features are typical with faormy histiocytes in the subcutaneous fat tissue. On tne treatments these patients are given oral corticosteroid, antihistaminics and salicylate with some improvments.
