3.The Effect of Intra-articular Steroid Injection for Adhesive Capsulitis in Spinal Cord Injured Patients.

OBJECTIVE: The purpose of this study was to investigate the risk factors of adhesive capsulitis and the effect of intra-articular steroid injection in spinal cord injured patients. METHOD: Fifty spinal cord injured patients participated in this study. The risk factors of adhesive capsulitis were compared between fourteen patients with adhesive capsulitis and thirty-six patients without one. Methylprednisolone acetate 40 mg mixed with 0.5% lidocaine 2 ml was given into glenohumeral joint space in adhesive capsulitis group and their pain and range of motion (ROM) were analyzed. RESULTS: 1) The incidence of adhesive capsulitis was higher in patients with higher injury level, older age and delayed start of rehabilitation therapy. 2) 10 cm visual analogue scale scores were significantly decreased after intra-articular steroid injection (p<0.01). 3) The shoulder ROM was increased after intra-articular steroid injection. The shoulder ROM at pre-injection was 126o in flexion, 113o in abduction, 64o in external rotation and 51o in internal rotation. The shoulder ROM at 4 weeks after injection was 138o in flexion, 131o in abduction, 74o in external rotation and 77o in internal rotation. CONCLUSION: There was a trend that the incidence of adhesive capsulitis was higher in patients with higher injury level, older age and delayed start of rehabilitation therapy. Further, it was suggested that intra-articular steroid injection was effective for reducing pain and improving ROM.

4.DIAGNOSTIC VALIDITY OF BONE SPECT IN ORAL MAXILLOFACIAL REGION.

Single photon emission computed tomographic(SPECT) imaging, performed at oral and maxillofacial surgery department, is valuable in assessing temporomandibular dysfunction, looking for metastases in patients with malignancies, diagnosing facial asymmetry. Conventional planar scintigraphy is limited by two-dimensional representation of three-dimensional structures. The SPECT exam, by sectioning through a distribution of tracer, eliminates the obscuring effects of tracer located outside the plane of interest. In our department, we used this image for four type cases retrospectively. 1. Those patients who have facial asymmetry 2. To find metastases in patients with malignacies 3. In combination with magnetic resonance imagine, to confirm the diagnosis of internal joint derangement 4. To evaluate the viability of grafted bone. Recently, this image is able to be used in those patients whose symptoms etiology cannot be explained by other diagnostic techniques.

6.A Case of Transient Acantholytic Dermatosis.

We present a case of transient acantholytic dermatosis which developed on the face of a 19-year-old Korean girl. Asyrnptomatic pinhead to miliary-grain sized brownish papules arranged in a somewhat band-like outbreak along the left side of the nose for about 3 months. Histopathologic findings were similar to Dariers disease. Immunoperoxidase staining showed that IgG was deposited on the intercellular area of keratinocytes around the suprabasal clefts. Skin lesions disappeared spontaneously in 5 weeks after a skin biopsy.

8.Radiological evaluation of tricuspid atresia: an analysis of cineangiography in 11 cases

Total 11 cases of tricuspid atresia were diagnosed radiographically at Seoul National University Hospital in recent two years since 1979. Some characteristic radiological findings were analyzed in chest P-A view and cineangiographies of right atrium and left ventricle. The results are as follows; 1. Among the 11 cases, 4 cases were male and 7 cases were female. Age distribution was from 2 months to 19 years and mean age was 3 1/2 years. 2. Anatomaic types of those cases according to Keith's classification were as follows; type Ia in 2 cases, type Ib in7 cases, type Ic 1 case and type IIb in 1 case. 3. The cineangiographic features of tricuspid atresia are sequential filling of contrast media in the order of right atrium, left atrium, and left ventricle, and triangular filling defect at the base of the heart in all cases, and right atrial dimple in two cases. 4. It is essential for the diagnosis of tricuspid atresia to undertake cineangiographies of right atrium and left ventricle, and the interrelationship of great arteries.

9.A Case with Isolated ACTH Deficiency

Isolated ACTH deficiency is a rare cause of secondary adrenocortical insufficiency. Adrenal crisis in isolated ACTH deficiency is less common compared to primary adrenal insufficiency, but isolated ACTH deficiency is an important cause of hypoglycemia. Recently we experienced a 41-year-old man admitted because of mental confusion. On admission, plasma glucose and sodium concentration were 1.7, 132 mmol/L, respectively. Basal plasma ACTH and cortisol levels were low and other pituitary hormone showed normal response to combined pituitary stimulation test except growth hormone. Plasma ACTH concentration remained low even after intravenous injection of ovine corticotropin releasing factor. It suggest that the defect of ACTH secretion was apparently due to intrinsic pituitary rather than hypothalamic disease. The sellar CT showed the fossa to be filled by cerebrospinal fluid. After treatment with glucocorticoid, he had no further evidence of hypoglycemia and hyponatremia. In conclusion, we report a case of isolated ACTH deficiency with empty sella.

10.Trigeminal Neuralgia due to Unusual Offending Vessels and Tic Convulsif.

Trigeminal neuralgia is a cranial rhizopathy characterized by paroxysms of hyperactivity strictly limited to the anatomical distributions of the fifth cranial nerve. One of a documented cause is vascular cross-compression at the root entry zone of the fifth cranial nerve near the brain stem. The offending vessels are the superior cerebellar artery, anterior inferior cerebellar artery, small arteriole, vein only, and vertebrobasilar artery in order of frequency. When trigeminal neuralgia and hemifacial spasm occur together in the same patient in rare occasions, they are termed "tic convulsif" We have recently experienced three cases of trigeminal neurlgia caused by unusual offending vessels and one case of tic convulsif. They were treated by microvascular decompression using Teflon felt. Post-operative courses were uneventful and marked symptomatic improvement had been achieved.
