As an optimal quality of the restorations, there should be a least amount of seating discrepancy between the casting and abutment teeth. However, high viscosity of the cementing medium and its resulting thickness ay prevent complete seating of the restoration. The use of die spacing material provides adequate internal relief for the cementing medium. The purpose of this study is to compare the thickness of three commonly used die spacing materials. Materials and Methods : Stone plates were fabricated and divided into 12 sections to be painted with die spacers. Tru-Fit, Whip-Mix and Belle de St. Claire die spacer which are commonly used in dental practice were tested in this study. Each die spacers were painted layer by layer according to the manufacturer's recommendation. The average thickness of each die spacers were measured with light microscope(x100) and compared between them. Results and Conclusions: A silver-colored Tru-Fit die spacer has the lowest value of thickness without statistical significance comparing with a gold-colored Tru-Fit die spacer and a gray layer of Whip-Mix die spacer has the highest value of thickness without any statistical significance comparing with Belle de St. Claire die spacer. Three and four layers of Tru-Fit die spacer and two layers of Whip-Mix and Belle de St. Claire die spacers seem to be in the acceptable range of thickness of 25 to 45micrometer for optimal seating of the restorations. The standard experimental design and method should be further evaluated for more consistent and objective results.

Many of problems which are faced to the edentulous patients are related to a minimal amount of available mandibular bone volume and height. Most of the patients with mild atrophy of mandible are treated using endosseous implant prosthodontics. TMI (Transmandibular Implant) can be used in case of severe mandibular atrophy of the mandible with exposed of inferior alveolar nerve, osteoporosis and the fracture of the atrophic mandible.Also it can eliminate the need for bone grafting and vestibuloplasty. The TMI is a rigid box frame structure which controls and distribute the masticatory force over the severely resorbed mandile. The box frame structure consist of a superstructure, baseplate, 4 transosseous posts, and 5cotical screws. This is a case report that also describes about the transmanibular implant reconstruction system.

An ossifying fibromyxoid tumor of soft parts is a rare, recently described, fibro-osseous neoplasm of uncertain histogenesis. It occurs most frequently within the subcutis or skeletal muscle of the extremities. Its biologic behavior is generally regarded as benign with at worst a locally aggressive clinical course. But, atypical and malignant variants have been recently reported. Herein we report a case of a benign ossifying fibromyxoid tumor which occurred in the left upper back of 41-year-old man. The tumor is composed of uniformly round or polygonal cells arranged in cords or nests which are separated by myxoid and hyalinzed fibrous matrix and associated with irregular bony trabeculae. The tumor cells are strong positive for vimentin. Ultrastructural findings and a review of literatures are added.

Intraductal variant of peripheral cholangiocarcinoma is extremely rare. This variant shows intraductal growth and intraluminal extension without any infiltrative growth. The mode of intraductal growth is not known. The prognosis of this variant is better than that of usual cholangiocarcinoma. We report three cases, one of which is associated with Clonorchis sinensis (CS) infection. The tumors were entirely confined within the dilated peripheral tributaries of the intrahepatic bile duct. Microscopically, the tumors were well to moderately well differentiated, with a papillary or a micropapillary growth pattern. Focal clear cytoplasmic change and mucin production were noted. The tumors showed intraductal spreading without any invasion to the liver parenchyme. Mucosal hyperplasia and dysplasia were noted in the adjacent ducts. The authors assume that intraductal cholangiocarcinoma is a distinct subtype, and persistent irritation, such as, CS infection may undergo a malignant transformation through mucosal dysplasia.

Background & Objective: Hiccups is a disabling condition of lateral medullary infarction (LMI). Unlike other symptoms of LMI, the anatomical lesions of hiccups are not well known. Few studies have evaluated the relationship between the lesional location of LMI and hiccups. We performed this study to correlate hiccups and magnetic resonance imaging (MRI)-based lesional location in pure LMI. Methods: Between January 1997 and February 2013, we identified 24 patients with pure LMI who presented with hiccups in addition to typical lateral medullary syndrome. Sixty six pure LMI patients without hiccups were included as a control group. Clinical and radiologic findings were compared between the two groups. MRI-identified lesions were classified rostrocaudally as rostral, middle and caudal, and horizontally as typical, ventral, large, lateral and dorsal. Results: The pure LMI patients with hiccups had significantly more frequent aspiration pneumonia (P = 0.001) and longer hospital stay (P = 0.03). The patients with hiccups significantly more often had dorsal rather than ventral lesion at horizontal levels (P = 0.012). But, there were no rostro-caudal differences at vertical levels (P = 0.162). Conclusions: We suggest that pure LMI associated with hiccups often locates in the dorsal medulla at horizontal correlation. This MRI-based comparative study has advanced the understanding of the neural substrate for hiccups in LMI, and indicates that hiccups become predictable when specific lesional locations in the lateral medulla are considered.
