3.8MHz RF Capacitive Heating on Rabbit Lung.

The usefulness of hyperthermia for cancer therapy has been established. The purpose of the present investigation was to access feasibility of heating normal lung and the temperature and power requirement were compared with that for liver as solid organ in rabbits by using radiofrequent heating machine. In this study, 20 rabbits were divided into 2 groups according to the heating site and the method of temperature measurement ; in group I : lung heating and temperature measuring in skin, esophagus and lung parenchyme ; in group ll : liver heating and temperature measuring in skin and liver parenchyme. The results were as follows ; 1) When the maximum temperature was almost same in lung heating group and liver heating group, the power for liver heating was lesser required than the power for lung heating (p<0.05). 2) The temperature of esophagus for the measurement of mediastinum temperature was 1.1+/-0.9 degrees C higher than the temperature of lung parenchyme (p<0.05). Therefore the above findings suggest lung, air containing organ, is well heated as same as liver, solid organ. So more active trials of lung heating in the lung cancer must be likely considered. But whets the lung is heated, the esophageal temperature is higher than lung parenchyme, so the mediastinum damage must be considered seriously.

4.A Case of palisaded, Encapsulated Neuroma.

Palisaded, encapsulated neuroma is characterized clinically by a solitary, slowly-growing and dome-shaped papule or nodule, usually accompanied by telangiectasia on the surface, and histopathologically by an encapsulated nodule in the dermis and the palisading arrangement of nuclei. A 57-year-old female patient presented with a 1cm-sized, solitary nodule on the right ala nasi which had been present for about 5 years. The nodule had a tendency to slow growth and it became a polypoid nodule. Telangiectasia was shown on the surface of the lesion. Histopathlogical findings showed a well-defined and encapsulated nodule in the dermis, composed of spindle cells with basophilic and plump nuclei in a palisading fashion. On immunohistochemical staining, the tumor cells of the nodule were positive for S-100 protein, while the capsule of the nodule was negative for S-100 protein. Epithelial membrane antigens were focally positive only on the capsule of the nodule. We report herein a case of palisaded, encapsulated neuroma, one case of which has been reported in Korea.

5.Serum Iron Concentration of Maternal and Umbilical Cord Blood during Pregnancy.

Anemia diagnosed early in pregnancy is associated with increased risks of low birth weight and preterm delivery. The purposes of this study were to assess the maternal iron status during pregnancy and to evaluate the relationships between the iron indices of maternal-umbilical cord serum iron and ferritin levels and pregnancy outcomes. Dietary intakes of the pregnant women were estimated by 24 hour-recall (3 times). Serum iron and ferritin levels in maternal blood and umbilical cord were measured at 1st-, 2nd-, 3rd- trimester and delivery, respectively. The mean of maternal serum iron levels of the trimester and delivery were 124.27microgram/dl, 97.03microgram/dl, 94.32microgram/dl, and 145.53microgram/dl. Those maternal levels were significantly lower than that of umbilical cord blood (222.59microgram/dl). Serum ferritin levels of maternal trimester and delivery were 22.68microgram/l, 11.09microgram/l, 14.18microgram/l and 24.54microgram/l, which were significantly lower than those of umbilical cord blood (184.35microgram/l)(p<0.0001). This prevalence of anemia of total subjects was 30.3% by WHO criteria (Hb<11.0 g/dl, Hct<33%). Iron levels of 2nd-trimester was significantly higher in the normal group than in the anemia group. And ferritin levels of 3rd-trimester and delivery was significantly higher in the normal group than in the anemia group. Therefore, we suggest for successful pregnancy outcome and delivery differential iron supplementation programs will be carried out with individual pregnant women on the basis of pre-pregnancy nutrition.

7.A case of bilateral primary adrenal lymphoma.

Primary adrenal lymphoma is extremely rare with only 65 cases reported in the worldwide literature until 1998. The presenting symptom may be related to the lymphoma itself or to adrenal insufficiency. Most of the lymphomas were of B cell diffuse large cell type with only two seemed to be T-cell in origin. The therapeutic modalities include surgery, combination chemotherapy, surgery followed by chemotherapy and/or radiation therapy, in addition to corticosteroid replacement. A 65 year old man with chronic hepatitis C and hemangioma of liver presented with general weakness. Abdominal ultrasonography showed bilateral adrenal masses. The adrenal masses were diagnosed as a primary adrenal non-Hodgkin's lymphoma by open biopsy. The histologic findings were consistent with B cell diffuse large cell lymphoma. After three cycles of CHOP chemotherapy, the adrenal mass disappeared on follow up abdominal CT scan, but the patient died 4 months after diagnosis due to sepsis.

8.Two cases of CD30+ anaplastic large cell lymphoma.

Two cases of CD30 positive ALCL is presented. A 27 year-old male patient presented with general lymphadenopathies, hepatosplenomegaly, splenic infarction, acute hepatitis, and hemopagocytic syndrome. The other 66 year-old female patient presented with general lymphadenopathies. Biopsy of the lymph node revealed proliferation of anaplastic pleomorphic tumor cells in the sinusoidal pattern. Immunohistochemical study revealed tumor cells were positive for LCA, T cell, and CD 30. All two cases were stage IIIA. Young patient was treat with 8 cycles of ProMACE-CytaBOM chemotherapy, and elderly patient was treated with 8 cycles of CHOP chemotherapy that resulted in complete remission with favorable clinical course.

9.Amylase-Producing IgG Kappa Type Multiple Myeloma.

Amylase-producing multiple myeloma is a rare disorder and has poor prognosis. Its characteristics are elevation of salivary type amylase, extensive extramedullary spread, extensive bone destruction, shorter survival time, and abnormal karyotype. Recently, we have experienced a case of amylase-producing IgG kappa type multiple myeloma in a 63-year-old woman. On admission, serum and urine amylase was 8,450U/L and 169,710U/L, 85% and 86% of which was determined to be the salivary type, respectively. The other cause of hyperamylasemia had not been detected. The presence of immunoglobulin and amylase in the myeloma cells was demonstrated immunohistochemically. Bone marrow aspiration smear revealed 59.1% plasma cells. The cytogenetic study of bone marrow cell showed normal karyotype. The patient died 3 months after chemotherapy with melphalan and prednisolone.

10.Evaluation of Scatter Reduction Effect of the Aft-Multiple-Slit (AMS) System Using MC Simulation.

PURPOSE: We designed the aft-multiple-slit (AMS) system to reduce scatter in cone-beam computed tomography (CBCT). As a preliminary study, we performed a Monte Carlo N-Particle Transport Code (MCNP) simulation to verify the effectiveness of this system. MATERIALS AND METHODS: The MCNPX code was used to build the AMS geometry. An AMS is an equi-angled arc to consider beam divergence. The scatter-reduced projection images were compared with the primary images only and the primary plus scatter radiation images with and without AMS to evaluate the effectiveness of scatter reduction. To obtain the full 2 dimensional (2D) projection image, the whole AMS system was moved to obtain closed septa of the AMS after the first image acquisition. RESULTS: The primary radiation with and without AMS is identical to all the slit widths, but the profiles of the primary plus scattered radiation varied according to the slit widths in the 2D projection image. The average scatter reduction factors were 29%, 15%, 9%, and 8% when the slit widths were 5 mm, 10 mm, 15 mm, and 20 mm, respectively. CONCLUSION: We have evaluated the scatter reduction effect of the AMS in CBCT imaging using the Monte Carlo (MC) simulations. A preliminary study based on the MCNP simulations showed a mount of scatter reduction with the proposed system.
