1.The determination of plasma IFN-γand IL-4 and fecal bifidobacteria in children with allergic asthma and analysis

Objective To explore the pathogenesis and clinical value of Th1/Th2 and normal intestinal flora from the angle of immunological and intestinal micro environment balance by detecting the plasma interferon-γ(IFN-γ)and interleukin-4(IL-4)and fecal bifidobacteria in Children with Allergic Asthma,so as to provide a theoretical basis through early intervention of probiotics to correct the Th1/Th2 balance.Methods The levels of blood plasma IFN-γ,IL-4 and fecal bifidobacteria amount of 165 cases of chil-dren with allergic asthma and 150 health students as control group were detected by ELISA and real time fluorescence PCR at the same time.Retesting these above indexes after applying bifidobacteria regulation of intestinal flora for 6 months.The clinical relative factors and experimental data were statistically analyzed.Results The level of IFN-γof asthmatic children were remarkably lower than those in the control group(P <0.05),while the level of IL-4 of asthmatic children were remarkably higher than those in the control group(P <0.05),and the amount of bifidobacteria of asthmatic children were remarkably lower than those in the control group(P <0.05).The level of IFN-γand bifidobacteria of asthmatic children improved remarkably and the level of IL-4 decreased significantly after applying bifidobacteria regulation of intestinal flora for 6 months.All of them had remarkable statistically signifi-cant.Conclusion The immune balance was affected by the intestinal flora balance of vice versa.This would provide an important theo-retical support to prevent the occurrence of allergic asthma by the early probiotics regulation of intestinal flora and immune balance.
2.The double-orifice technique: the treatment of anterior leaflet prolapse of mitral valve

Objective: To evaluate the clinical effect of the double-orifice technique in the treatment of the anterior leaflet prolapse of mitral valve. Methods: 35 patients with severe anterior leaflet prolapse of mitral valve underwent valve repair using the double-orifice technique. The condition of valve was assessed with echocardiography preoperatively, intraoperatively, and postoperatively. Results: The mean postoperative valve area was (4 1?1 8) cm 2 against the preoperative valve area of (8 3?1 6)cm 2. Mild regurgitation was found in 2 patients after operation. There were no hospital deaths. At the latest follow-up, the heart function was improved significantly in all patients (NYHA class I). Echo-Doppler assessment of valve showed stable valve function in all patients. There were no late stenosis of valve and no late death. Conclusion: The results of this study show that the double-orifice technique is a safe and effective method for treatment of anterior leaflet of mitral valve with low mortality and morbidity, and excellent early and mid-term results.

5. Exploratory factor analysis in analyzing construct validity of performance evaluation system of clinical centers

Objective: To analyze the construct validity of performance evaluation system of clinical centers by exploratory factor analysis strategy, so as to provide evidence for optimizing the parameters of the evaluation system. Methods: Using the exploratory factor analysis method, we evaluated the construct validity of the constructed performance index system. Results: Factor analysis showed that the factor components (listed in an increasing importance order) were: discipline superiority, cultivation of talented persons, medical service, scientific research capability, and infrastructure construction, etc. (the cumulative contribution of the first 6 factors was 90.17%). Most of index factor loadings matched with the meaning of their corresponding factors, indicating that the construct validity of the evaluation system was consistent with the expectation of its designers. Conclusion: The constructed clinical center performance evaluation system has a reasonable factor structure.

7.New development of analgesics brought about by studies on pain

OBJECTIVE:To review the recent advances in analgesics study and summarize the drugs for central sensitization and analgesia model by relating to the latest means and methods of research.DATA SOURCES:The literature on analgesics was retrieved in NCBI database for the related papers published between January 1996 and December 2001, with the key words of "analgesia", "analgesic", and "pain"and language restricted to English. At the same time, papers published in the past two years were retrieved in http:∥www.wanfangdata.com.cn, with the key words of "analgesia", "analgesic" and "ganglioplegic" in Chinese and the language restricted to Chinese.STUDY SELECTION:Totally more than 50 000 research references repeated contents.DATA EXTRACTION: Altogether 100 basic research articles were collected. We selected 15 articles relevant to the research development in analgesic drugs, and excluded 82 review papers with repeated content.expressed by proto-oncogene c-Fos located in the nucleus is a kind of DNA conjugated protein. It can regulate the expression of some pain-related genes. Some studies found that substance P synthesis is likely to play a research: Developing the central sensitization drugs is the orientation of sal root ganglion neurons and model of swallowing response were established.CONCLUSION:By investigating the effects of various new research methods and models on pain, we intended to look for more new drugs, to further study neurotransmitters and the clone of transmitter receptor at the cellular and molecular level so as to find the new effect target of the drugs, and to have a better understanding of the integrity of chronic pain in humans and achieve more breakthroughs in the development of neuroscience research.
8.Relationship between subclinical cervical human papillomavirus (HPV) infection and recurrence of condyloma acuminatum in patients

Objective To investigate the relationship between subclinical cervical HPV infection and recurrence of condyloma acuminatum (CA) in patients. Methods Cervical swabs were collected from 52 patients with frequently recurrent CA, 55 patients with infrequently recurrent CA, and 65 normal human controls. The Cenechip method was performed to detect the presence and type of HPV in cervical swabs followed by a statistical analysis. Results HPV was found in 67.3% (35/52) of swabs from patients with frequently recurrent CA, 5% (19/55) from patients with infrequently recurrent CA, and 4.6% (3/65) in the controls. There was a statistical difference among the three groups in the detection rate of HPV. Conclusion The subclinical cervical HPV infection may contribute to the recurrence of CA.
9.Treatment of posterior malleolar fractures by means of limited open reduction, poking reduction and QWIX cannulated screw internal fixation

Objective To evaluate the clinical effect of limited open reduction, poking reduction and QWIX cannulated screw internal fixation in treatment of posterior malleolar fracture. Methods A total of 14 patients with posterior malleolar fracture were treated by limited open reduction, poking reduction and QWIX cannulated screw internal fixation. Injury causes included traffic accidents in five patients, fall from height injury in four and inversion sprain in five. According to fracture type classification methods of Weber and Danis, there were two patients with type A3, seven with type B3 and five with type C3. Func-tional exercise was carried out based on postoperative exercise plan. Regular follow-up was performed to e-valuate the functional recovery of the ankle joint. Results All patients were followed up for 6-18 months (average 9.5 months). One patient with sural nerve injury returned to normal after six months. Twelve pa-tients showed excellent range of motion of the ankle joint and normal gait three months after operation. In 12 patients, dorsiflexion of the ankle joint was > 20° and plantar flexion > 40° six months after operation, which was approximately similar to contralateral normal ankle joint. The ankle joint function of all patients returned to normal six months after operation. According to Baird-Jackson ankle joint function evaluation, the result was excellent in 12 patients, good in one and fair in one, with excellence rate of 93%. Con-clusion The limited open reduction, poking reduction and internal fixation of QWIX caunulated screw are effective method for treatment of posterior malleolar fractures.
10.Investigation and analysis in the coping style and influencing factors of the recipients after relative kidney transplantation

Objective To investigate the coping style and influencing factors of the recipients after relative kidney transplantation. Methods Medical coping modes questionnaire (MCMQ) and observa-tion interview were performed in the sampling survey, 56 case of recipients after relative kidney transplanta-tion according to the adopted standard were analyzed. Results The confronting score of all the cases were close to that of normal model, but the avoiding score and yielding score significantly increased. The prospective health conditions of the relative donors, function and survival time of the graft kidney, rejection reaction and other complication after transplantation were the main factors affecting the coping style of the recipients. Conclusions Based on the accurate evaluation of the coping style, nursing intervention should be aimed at the recipients following relative riving donor kidney transplantation, in order to help the recipients create a correct coping style and improve the survival rate of patient/kidney.
