In clinical practice teaching of respiratory medicine, many factors have a major impact on the quality of training. This article focuses on chnical practice teaching in influence factors and puts forward some countermeasures.
2.The regulation of TLR3 on NF-κB signaling pathway and its significance in the pathogenesis of tumor

NF-κBs is a member of Rel protein family of transcription factors ,which is expressed in all cells and is involved in important physiological and pathological regulatory mechanisms ,including the regulation of TLR3 on NF-κB signaling pathway ,which has been attracted widespread attention .This paper reviews the mo-lecular mechanisms of the regulation .We summarizes the role of host immune responses in tumor development , which is mediated by the related signal molecules .It reveals that some key proteins in the regulation have a great significance to the treatment of diseases ,as used as the therapeutic targets ,and is conducive to the understanding of function and diversity in tumor tissue .
3.Efficacy of linezolid in the treatment of severe pneumonia and its psychological countermeasures

Objective To study and analyze the curative effect of linezolid in the treatment of severe pneumonia and the psychological countermeasures. Methods 100 cases of severe pneumonia patients treated in our hospital from March 2015 to August 2016 were selected and randomly divided into the control group and the experimental group. Two groups of patients according to the actual situation of patients with nutritional support and supplement electrolyte based treatment. The control group was treated with levofloxacin and ceftriaxone infusion treatment, the experimental group underwent intravenous linezolid therapy, and psychological care, pay attention to the psychological status of patients and patients, strengthen communication and exchanges, increase confidence in the treatment and the treatment compliance of patients. The therapeutic effects of the experimental group and the control group were compared and analyzed. Results After the corresponding treatment, the number of invalid cases in the experimental group was 4 cases, the number of effective cases was 31 cases, and the number of effective cases was 15 cases. The number of invalid cases in the control group was 10 cases, the effective number was 26 cases, and the effective cases were 14 cases. The effective rate of treatment in the experimental group was 92.0%, which was significantly higher than that in the control group, and the effective rate was 80.0%, with statistical difference (P<0.05). There were no obvious adverse reactions in the experimental group and the control group. The rate of adverse reactions in the control group was 14.0%, significantly higher than that in the experimental group (2.0%), and there was a statistical difference (P<0.05). Conclusion Linezolid is effective in the treatment of severe pneumonia, it can improve the therapeutic effect to a certain extent, and it has high security, it has the significance of clinical promotion.
6.Tumor necrosis factor-alpha and heart diseases

Tumor necrosis factor alpha (TNF ?) is a pro inflammatory cytokine produced primarily by mononuclear phagocytes in blood and different tissues. Current data indicate that adult myocytes can also produce TNF ? in the course of various heart diseases, such as congestive heart failure, myocarditis, ischemic heart disease, dilated cardiomyopathy, and septic cardiomyopathy. Elevated TNF ? is considered to be contributive to the pathophysiology of these diseases. Methods concerning anti TNF ? therapy and their clinical application are the on going investigations.
7.Study on the effects of two different routes of intravenous administration on mannitol induced phlebitis

Objective To study and analyze the effects of two different routes of intravenous administration on mannitol induced phlebitis. Methods 98 patients who received mannitol infusion in the first people's hospital in Wenling from December 2013 to December 2016 were selected as the study objects. The patients were divided into PICC group and peripheral group according to different intravenous administration modes. In group PICC, 49 cases were given intravenous administration by PICC. In peripheral group, 49 cases were received peripheral vein indwelling needle. The incidence of phlebitis in the two groups was compared. Results The data showed that the incidence of phlebitis in group PICC was 8.16%, and the incidence of phlebitis in peripheral group was 22.45%. The difference between the two groups was statistically significant (P<0.05). Conclusion Mannitol infusion by PICC can reduce the incidence of phlebitis.

9.Research in effectiveness evaluation indicator for thoracic close drainage system

Objective To establish a scientific, objective and practical evaluation scale for thoracic close drainage system. Methods Based on the evidence-based practice and related documents, the indicators of efficiency for thoracic close drainage system were identified tentatively. Then Delphi method was used and 23 experts were investigated and consulted by two rounds. Results The index of experts activity was 92% and 100% in the first and second round of Delphi consultation, respectively. The scale comprised 50 indicators in 3 dimensions, the experts' authority coefficient was 0.987. Conclusions The reliability and authority of the scale are acceptable and it can be used to evaluate the efficiency of thoracic close drainage system.
10.CLINICAL ANALYSIS OF CORONARY ARTERY BYPASS GRAFTING

To study the cause of postoperative complications of coronary artery bypass grafting(CABG) and their preventive measures. 210 cases were reviewed and analyzed from 1993 to 2000, including 194 males and 16 females with a mean age of 58 7 years (range 32~78 years). 150 patients had three coronary vessels disease 101 patients had the history of myocardial infarction 33 patients were in heart function Ⅲ~Ⅳ(NYHA) CABG under cardiopulmonary bypass(CPB) were performed in 200 cases, and off-pump CABG in 10 cases The average number of grafts were 3 1. Cardiac valve replacement and plastic operation were done in 25 patients at the same time, and left ventricular aneurysms were resected in 8 cases The hospital mortality was 1 9%(4/210), and 13 patients had perioperative myocardial infarction. The recurrence rate of angina was 5 2%(11/210). The incidence of postoperative arrhythmia was 16 2%(34/210). A rise of creatine kinase level was observed in 145 cases after CABG. Incidence of painless myocardial infarction was very high(11/13) in perioperative myocardial infarction, therefore the level of myocardial enzymes should be assessed after the operation and the changes in electrocardiogram should be monitored in order to avoid misdiagnosis. The great majority of postoperative arrhythmias were atrial flutter,atrial fibrillation, and supraventricular tachycardia. Normal sinus rhythm was readily restored by conventional treatment.
