2.Hemodynamic Evaluation of pre. and Postoperative Cardiac Imaging Scan in Congenital Heart Disease.

Recently, the radionuclide angiocardiographic study had been enhanced by rapid development of gamma scintillation camera and computer system, and utilized as noninvasive diagnostic measurement of heart disease. We obtained the following results by the radionuclide angiocardiography with human serum albumin tagged with 99m-Technetium from two groups. One of them was congenital heart disease group(n=50, ventricular septal defect 20, atrial septal defect 6, pulmonary stenosis 2, tetralogy of Fallot 10, truncus arteriousus 1, transposition of great artery 1, patent ductus arteriosus 10) operated from Oct. 1984 to Jun. 1985, and the other was the normal human group(n=10) as control. 1) In the cases of left to right shunt, Qp/Qs values were calculated by th gamma variet fit curve. The preoperative values ranged from 1.441-3.00, the postoperative values were normal among all the patients except one(atrial septal defect, postop. Qp/Qs 2.335). 2) In the cases of right to left shunt, Qp/Qs values showed no apparent change between pre. andpostoperative period, possibly due to still remained collateral circulation and hardly obtained lower value(less than QP/QS 1.0) by computer system, But the double peak curve was noticed in the left ventricular time activity curve, which indicate right to left shunt, disappeared after the surgery, suggesting shunt closed. 3) The ejection fraction of left ventricle was obtained from the congential cardiac patients, ranged 0.38-0.65 before the surgery and 0.41-0.67 after the surgery. 4) The circulation time calculated from the first-pass radionuclide angiocardiography, ranged normal in the most congenital cardiac patients, but prolonged in the severe pulmonary hypertension.

4.Computed Tomography(CT) in Head Trauma Patients with Alert Mental Status : How Important are the Clinical Symptoms.

STUDY OBJECTIVES: To identify the value of symptoms in head trauma patients with alert mental status requiring a head CT scan. METHODS: A retrospective study was performed at an emergency center over 8 months period. Patients(age > or = 7) were enrolled if they presented with a Glasgow Coma Scale score of 15 and underwent head CT after head trauma. The symptoms and signs were recorded in the entry forms prior to head CT The symptoms found in the patients with normal CT were compared with those in abnormal CT showing intracranial injuries. RESULTS: Among the 168 patients, traumatic intracranial abnormality was identified in 34(20.2%) by CT. Skull fracture was idendified in 8(4.8%) in normal CT, 17(10.1%) in abnormal CT group by simple X-ray and CT. In the normal CT group without skull fracture, 65(52.0%) had headache, 61(48.8%) had loss of consciousness( LOC), 43(34.4%) had vomiting, 24(19.2%) had nausea, 7(5.6%) had dizziness. In the abnormal CT group without skull fracture, 14(77.8%) had heacache, 9(50.0%) had LOC, 9(50.0%) had vomiting, 2(11.1%) had nausea, 2(11.1%) had dizziness. The patients with headache were significantly more common in the abnormal CT group. CONCLUSION: A head CT is required in alert head trauma patients with any symptoms and signs of intracranial injuries, especially with headache.

7.A Case of Patau Syndrome Diagnosed in Early Pregnancy.

Patau syndrome, or Trisomy 13 is one of the most common autosomal aberration associtated with multiple congenital abnormalities. We report a case with trisomy 13 mosacism which was found during an amniocentesis performed due to the age of the mother and abnormal nuchal translucency. The clinical features of fetus included cleft lip and palate, low set ears, polydactily, small ""micro"" penis, and Rocker-bottom feet. After termination of the pregnancy, the fetus was sent for an autopsy. The autopsy report was compatible with the gross findings and pulmonary hypoplasia, microophthalmia, hypoplasia of left ventricle of heart were found.

8.The Effects of PolyMem(R) on the Wound Healing.

Little objective information is avilable on the influence of occlusive dressings on the healing of cutaneous partial skin defect wounds. Our purpose was to examine the effects of occlusive dressing by using the synthetic dressing mateial, PolyMem in the management of 2nd degree burn wounds and donor sites of split thicknes skin graft and partial-thickness wounds in rabbits. New Zealand white rabbits, 12 to 14 weeks of age, were divided into 2 groups. Two partial thickness skin wounds measuring approximately 40x30 mm were induced using a scalpel on the back of each anesthetized animal. They were designated as group I (dressing with conventional method, n=15), group II (dressing with PolyMen, n=15). Each treated wound was individually covered with the assigned dressing immediately after wounding. Wound were examined and measured at 10 days to determine the extent of healing. By day 10, the PolyMem dressed wounds were approximately 67% healed, while all vaseline gauze dressed wounds were about 50% healed. Standardized 20 mm full-thickness biopsy wounds were treated for 10 days. Section of PolyMem group at POD 10 days showed complete epidermal regeneration above fibrotic dermis (H&E, x40). Section of conventional group at POD 10 days showed marginal epidermal regeneration (H&E, x40). 72 patients (44 patients with 2nd degree burn and 28 patients with skin graft donor sites) were divided into four groups. They were designated as group I (Burn patients with PolyMem, n=24), group II (Burn patients with conventional methods, n=24), group III (S.T.S.G. patients with PolyMem, n=14), group IV (S.T.S.G. patients with conventional methods, n=14). We investigated wound site pain, healing time, comfort and numbers of dressing change. As compared with the control group, the PolyMem dressed group had less pain, more rapid healing time, more comfort, less frequent dressing changes. From these results, we concluded that the occlusive dressing with PolyMem was an effective alternative to the conventional gauze dressig on the wound healing. Our results suggest tat PolyMem is one of the ideal dressing materials.

9.Medium Supplementation and Atmospheric Condition for Growth of Campylobacter pylori isolated from gastric biopsy tissue.

Experiments were conducted to define the optimal constituents of culture medium and atmospheric condition for growth of Campylobacter pylori. Two clinical isolates were streaked onto various media, incubated in two different atmospheric conditions (microaerophilic condition and carbon dioxide incubator), and growth was assessed semiquantitatively according to relative colony size and extent of growth through the streak. The growth obtained on Campy media, composed of GC agar base plus 1% hemoglobin, 0.2% activated charcoal, 1% IsoVitaleX, vancomycin 6mg /L nalidixic acid 20mg/L and amphotercin 2 mg/L, was used as reference. Our conclusions were as follows: Tryptic soy agar base was not acceptable for the growth of C. pylori. The organism grew in both atmospheric conditions, but generally showed a scantier growth in the carbon dioxide incubator than under the microaerophilic condition, however GC agar containing 1% hemoglobin and 0.2% activated charcoal supported well the growth of C. pylori in the carbon dioxide incubator. The authors have found that the GC agar base supplemented with 1% hemoglobin and 0.2% charcoal was the most satisfactory medium and a microaerophilic condition was optimal atmospheric condition for the growth of Campylobacter pylori in this study.
