2.Prostate specific antigen as a tumor marker for adenocarcinoma of the prostate.

Serum prostate specific antigen (PSA) and prostate acid phosphatase (PAP) were measured in 106 patients including 9 patients with prostate cancer, 63 patients with BPH. and 34 normal control group free of prostate disorders. The mean PSA value was 61.14+/-43.87 ng/ml for prostate cancer, 6.07+/-5.57 ng/ml for BPH and 1.42+/-0.98 ng/ml for normal control group, and the mean PAP value was 9.81+/-10.16 ng/ml for prostate cancer, 1.58+/-2.36ng/ml for BPH and 1.00+/-0.17 ng/ml for normal control group. The positive rates of PSA were 78% for prostate cancer, 22% for BPH and 0% for normal control group, and those of PAP were 67% for prostate cancer, 6% for BPH and 0% for normal control group. The sensitivity and specificity for PSA were 78%, respectively. We conclude that PSA may be a useful tool for detection and therapeutic monitoring of the prostatic cancer.

5.Successful Treatment of Accidental Hypothermia with Injury: A Case Report.

Accidental hypothermia is an uncommon health issue that can cause fatal problems. Mortality related to hypothermia has been reported to be up to 50%. Prognosis of hypothermia depends on the grade of hypothermia, interval of re-warming from the accident and the re-warming rate. Hypothermic patients with injury show worse prognosis. A 66 year-old man with history of cerebrovascular accident was rescued in the mountains. His Glasgow coma scale (GCS) was 3, core temperature was 25.2degrees C and he had a right lateral malleolar fracture. The second patient was a 45 year-old man whose GCS was 8 and core temperature 17.2degrees C. Blood pressure was unmeasurable with only palpable pulse and showed Osborn J wave on electrocardiography for both patients. Active re-warming was performed with forced warm air, warm saline infusion and bladder irrigation. Patient core temperature raised by 1.5degrees C/hour and 3.3degrees C/hour, respectively. The two patients were discharged without any other sequelae and disability.

6.The Emerging Crisis of Stakeholders in Implant-based Augmentation Mammaplasty in Korea

Background:
Korea is no longer safe from the risk of breast implant-associated anaplastic large cell lymphoma (BIA-ALCL); the first reported case was a Korean woman in her 40s who had a 7-year-history of receiving an implant-based augmentation mammaplasty using a textured implant. We conducted this study to discuss the emerging crisis of stakeholders in implant-based augmentation mammaplasty and to propose a multi-disciplinary approach to early detection of its complications.
Methods:
We analyzed medical examination data that was collected from patients who visited us between August 12 and September 27, 2019. We evaluated a total of 114 women (n = 114) in the current study. They were evaluated for whether they were in healthy condition. Moreover, their baseline characteristics were also examined; these included age, gender, height (cm), weight (kg), duration since surgery (years), possession of a breast implant card, the site of surgical incision, side of symptoms and reasons for outpatient visit. Furthermore, the patients were also evaluated for their subjective awareness of the manufacturer, surface and shape of the breast implant. Potential complications include malrotation, folding, seroma, capsule thickening, upside-down rotation, rupture, capsule mass and breast mass.
Results:
A majority of the patients had a past history of receiving textured implants. The corresponding percentage was 78.95% (90/114) and 85.09% (97/114) based on their subjective awareness of a breast implant and sonographic findings, respectively. That is, it was slightly increased with the use of a breast ultrasound.
Conclusion
Here, we propose the following approaches. First, patient data should be prospectively collected. By tracking outcomes and complications of an implant-based augmentation mammaplasty, both high-quality care and patient safety can be ensured. Second, stakeholders in implant-based augmentation mammaplasty should collaborate with customers and regulatory authorities. Third, surgeons should consider applying imaging modalities for early detection of postoperative complications.
7.A Case of Primary Ureteral Adenocarcinoma Associated with Long-standing Ureteral Stone.

Primary ureteral carcinoma is a rare disease comprising 1% of all urinary tract tumors. Primary adenocarcinoma of the ureter is extremely rare. We report a case of primary ureteral adenocarcinoma associated with long-standing ureteral stone in 40-year-old man, who was treated by nephroureterectomy with bladder cupping, with a brief review of the literatures.

8.Visualization of Coronary Arteries by Color-Coded Transesophageal Doppler Echocardiography.

The visualization of coronary arteries by transthoracic two-dimensional echocardiography has been used for over 10 years. In many cases, the imaging quality is too poor to allow an anatomic evaluation. During the last few years, transesophageal echocardiography has been shown to provide optimal imaging quality in virtually all patients and of all cardiac structures including the coronary arteries. The purpose of this study was to test the ability of transesophageal echocardiography in the visualiation of the coronary arteries and assessment of coronary blood flow by transesophageal two-dimensional pulsed Doppler echocardiography. We Studied 285 patients, 91 men and 194 women, aged 16 to 81 year(mean 50.6 year men, mean 54.2 year women). We have been used a 5-MHz phased array transducer with incorporated color-coded Doppler. The left main coronary artery was visualized 95.1%, left circumflex artery 27.4, left anterior descending artery 21.4% and the main stem of the proximal right coronary artery 45.1%. The time-sequential left anterior descending artery flow pattern generally consisted of a small late systolic component and a large diastolic component. The peak flow velocity in the proximal left anterior descending artery during diastole was 40.8+/-8.0cm/sec(integrity 7.6+/-0.9) and during late systole was 18.5+/-5.5cm/sec(integrity 2.9+/-0.9). There were no complications during and after examination. This study suggests that transesophageal color-coded Doppler two-dimensional echocardiography appers to be a feasible noninvasive technique for imaging the proximal left coronary artery and the left anterior descending artery flow is detectable from the transesophageal approach.

9.3D-reconstruction of right portal vein.

BACKGROUND/PURPOSE: Thorough knowledge of hepatic anatomy is essential in liver surgery. Recent development of computer technology has allowed three-dimensional (3D) reconstruction of radiological images. We evaluated the right portal veins by 3D-reconstruction and determined the numbers and directions of the segmental branches to provide a readily understandable portal branching pattern. METHODS: CT arterial portography(CTAP) data from 11 hepatoma patients were reconstructed threedimensionally in a computer workstation. The images were rotated from top(U90) to bottom(D90) and from right lateral(R90) to left lateral (L90), and the 3rd portal branches were matched and their course was determined. RESULTS: The direction of right portal trunk was right(R) 105(5/7 patients) - R 90(2/7 patients) and 0 - up(U) 15 in 5/7 patients(pt). The most common directions of R anterior trunk, VIII, V, R posterior trunk, VII, VI segmental portal branches were R60(6/10 pt) & U15(5/10), R 135(5/10) & U60(5/10), R30-75 & down(D)15-60(9/10), R105-120(6/10) & D15-30(8/10), R165(6/11) & U15(6/11), R105(6/11) & D30-45(8/11). The numbers of VIII & V branches visualized were 1(4),2(6),3(1) & 1(5),2(5),3(1). The numbers of VI branches were 1(5),2(4),4(1). The VII branch always branched into lateral and medial branches. Simple branching patterns were obtained on posterior segmental branches. The anterior segmental branches were somewhat variable. CONCLUSION: 3D-reconstruction of portal veins allowed accurate assessment of segmental branches and their spatial relationship was readily understandable. It would be helpful in planning and simulation of surgery.

10.Nocturanl Eating Behavior Associated with Zolpidem.

OBJECTIVE: The authors would like to fine the relationship between zolpidem and nocturnal eating episodes in diverse psychiatric patients. METHOD: We evaluated case series of 6 patients who showed nocturnal eating episodes after takine zolpidem. RESULTS: We have experienced 6 cases who showed nocturnal eating behavior after taking zolpidem. They included 3 patients with schizoprenia, a patient with major depressive disorder, a patient with PTSD and a patient with bipolra I disorder. With reducing or discontiuation of of zolpidem, their nocturnal eating resolved. CONCLUSION: This finding strongly suggests the relationship between zolpidem and the nocturnal eating episode. Physicians should be aware that zolpidem might induce nocturnal eating behaviors.
