The most common adverse effect of intravenous heparin is hemorrhage. Of the these, retroperitoneal hemorrhage and femoral neuropathy secondary to heparin anticoagulation has reported in 1966 for the first time by DeBolt and Jordan. We experienced a case of spontaneous retroperitoneal hemorrhage due to iliopsoas muscle hematoma in patient with myocardial infarction receiving intravenous heparin for a 3 days in therapeutic doses. The pathophysiology of iliopsoas muscle hematoma has not yet been cleared. But because of the possibility of large amount in volume, retroperitoneal hemorrhage has been known as a serious adverse effect that leads to the hypovolemic shock and death. In this article, we described the clinical manifestation, the importance of the early diagnosis and diagnostic clues and associated factors of the retroperitoneal hemorrhage in patient receiving intravenous heparin.

3.Treatment with Repeated Lumbar Epidural Blood Patches for a Patient with Orthostatic Headache after Suboccpital Craniectomy.

Orthostatic headache after suboccipital craniectomy has been rarely reported. A 32-year-old man underwent emergent suboccipital craniectomy due to acute bilateral cerebellar infarctions with massive brain edema. Although his neurological deterioration halted after the craniectomy, he still suffered from a severe disabling orthostatic headache. We performed two epidural blood patches at the levels of the lumbar and thoracic spine. He improved considerably a few days after the repetitive epidural blood patches. Epidural blood patches may be a useful treatment modality for post-craniectomy postural headaches.

4.Endovenous 980-nm Diode Laser Treatment of Incompetent Great Saphenous Vein.

BACKGROUND: Endovenous laser treatment has recently been introduced as a less invasive technique, with many good results reported. A 980-nm diode laser was used on the great saphenous vein (GSV) occlusions in patients with varicose veins, due to GSV reflux, and the therapeutic effects analyzed. MATERIAL AND METHOD: Between September 2003 and February 2006, 238 patients (332 cases) with GSV reflux underwent endovenous laser treatment with a 980-nm diode laser. Operative procedures were simultaneous endovenous laser treatment and ambulatory phlebectomy. The preoperative clinical findings, postoperative complications and postoperative duplex US follow up results at 1 and 3 months were reviewed. RESULT: Postoperative complications were ecchymosis in almost cases, paresthesia 68 cases (20.5%) and skin burn in 3 cases (0.9%). Follow up duplex US revealed 26 and 10 cases at 1 and 3 months of GSV reflux or partial recanalization, with therapeutic success rates of 91.3 and 87.9%, respectively. CONCLUSION: The treatment of GSV occlusion with an endovenous laser is less invasive than traditional GSV stripping, but relatively high recanalization was experienced, possibly due to inadequate laser power, laser fiber pullback speed or an extremely dilated GSV. However, continuous attempts will be required to reduce the recanalization after the procedure, with accurate follow up using duplex US.

7.Efficacy of Biometry Using Swept-source Optical Coherence Tomography for Posterior Chamber Phakic Intraocular Lens Implantation

Purpose:
The proper implantable collamer lens (ICL) size affects ICL stability. This study compared device efficacy using white-to-white diameter (WTW) measurements with Orbscan II and IOL Master 700.
Methods:
We retrospectively studied 90 eyes (45 patients) who underwent toric ICL implantation from January 2019 to February 2020 and were followed for 1 year. The correlation between WTW and anterior chamber depth (ACD) for each measuring device was analyzed.
Results:
The mean WTW measured by IOL Master 700 and Orbscan II was 12.2 ± 0.3 and 11.6 ± 0.3 mm, respectively, while the mean ACD was 3.28 ± 0.16 and 3.20 ± 0.15 mm. The WTW and ACD measured with IOL Master 700 averaged 0.57 ± 0.12 and 0.08 ± 0.04 mm larger than with Orbscan II. The differences were significant and the regression analysis had high correlations (R2 = 0.875 and R2 = 0.913, respectively; both p < 0.001).
Conclusions
WTW measured by the IOL Master 700 can be used as a reference either alone or together with the Orbscan II value to determine ICL size.
8.Refractive Power Outcomes with an Intraocular Lens with 0.25-diopter Intervals

Purpose:
The Eyelike K-flex Aspheric® (Koryoeyetech, Seoul, Korea) is manufactured in 0.25-diopter (D) intervals, which allows the target refractive error after surgery to be achieved. We here evaluate the refractive power outcomes.
Methods:
We retrospectively studied 95 eyes of 72 patients who underwent cataract surgery with implantation of the Eyelike K-flex Aspheric®. Refractive error was measured at 1 and 2 months postoperatively (33 eyes of 27 patients) and compared to that of patients fitted with 0.50-D-interval lenses (62 eyes of 49 patients).
Results:
At 1 month postoperatively, the mean absolute error between the spherical equivalent and planned value was 0.33 ± 0.28 and 0.41 ± 0.39 D in the 0.25- and 0.50 D-interval lens groups, respectively (p = 0.318). At 2 months postoperatively, the respective values were 0.21 ± 0.15 and 0.34 ± 0.29 D (p = 0.009).
Conclusions
The Eyelike K-flex Aspheric® shows excellent refractive predictability; use of 0.25-D-interval intraocular lenses close to the target refractive power allows the desired spherical equivalent to be achieved.

10.Angiotensin-Converting Enzyme Inhibitor Ramipril Improves Coronary Microvascular Remodeling in Type II Diabetic Rats.

BACKGROUND AND OBJECTIVES: Coronary microvascular remodeling is one of the complications associated with diabetic cardiovascular disease. We investigated the effect of long-term treatment with ramipril, an angiotensin converting enzyme inhibitor, on coronary microvascular remodeling on a type II diabetic model, the Otsuka Long-Evans Tokushima Fatty (OLETF) rat. MATERIALS AND METHODS: Twenty OLETF diabetic, and twenty LETO non-diabetic rats at twenty-four weeks old, were randomized into 4 equal groups for treatment with either an aqueous solution of ramipril (5 mg/kg, n=10 for two groups) or saline (n=10 for two groups) for 24 weeks on a daily basis. The blood glucose levels, body weights and blood pressures of the rats were checked on a regular basis. Masson's trichrome and picrosirius red stains were used for the morphometric analysis of the thickening of the coronary arterial wall and the degree of perivascular fibrosis. The myocardial collagen content was determined by measuring the levels of myocardial hydroxyproline. RESULTS: Marked thickening of the coronary microvascular wall and prominent perivascular fibrosis were detected in the hearts of OLETF rats to a greater extent than in the LETO rats (p<0.01). Ramipril significantly prevented coronary microvascular remodeling in OLETF rats (p<0.01), but there was no significant difference in the collagen content/dry heart weight ratio between the groups. CONCLUSION: Long-term treatment with ramipril has an antifibrotic effect, and improves the coronary microvascular remodeling in diabetic OLETF rats.
