1.Expression of c-erbB-2, c-myc, c-fos, bcl-2, p53, PCNA, and TGF-alpha in Transitional Cell Carcinoma of the Urinary Bladder.

Most of malignant tumors in the urinary bladder is transitional cell carcinoma (TCC) deriving from the urothelium. Clinical stage and histopathologic grading of the TCC of the urinary bladder is important in the determination of the patient's prognosis. To investigate the correlation between the prognostic factors and the expression of the various oncoproteins and growth factors in each grade of the TCC, immunohistochemical stains for c-erbB2, c-myc, c-fos, bcl-2, p53, proliferating cell nuclear antigen (PCNA), and transforming growth factor-alpha (TGF-alpha) were performed in the formalin fixed paraffin embedded tissues of the TCC (Grade I; 15 cases, Grade II; 20 cases, Grade III; 15 cases) of the urinary bladder. The immunoexpression rate of c-erbB2 was immunoexpression 78.0% in the grade I, 85.0% in the grade II, and 95.0% in the grade III TCC. The immunoexpression rate of c-myc, c-fos and bcl-2 was below 5% in each grades of TCC. The p53 immunoexpression was identified in 11.5%, 24.3% and 30.6% of the grade I, II, and III TCC, respectively. The PCNA and TGF-alpha expression was 53.0% and 27.6% in the grade I, 77.3% and 32.7% in the grade II, and 78.2% and 37.3% in the grade III TCC, respectively. These results suggest that the expressions of c-myc, c-fos, bcl-2, and TGF-alpha are similar in each grade of the TCC and the positivity of c-erbB2, p53, and PCNA shows an increasing tendency for the higher grade TCC of the urinary bladder. Therefore, c-erbB2, p53, and PCNA are clinically useful predictors of the patient's prognosis.

3.A Case of Leukemia Cutis in Erythroleukemia.

A 13 years old girl came to our department with complaints of multiple ulcerating and non-ulcerating nodules on her back and buttocks since 1 month prior to visit. Skin biopsy specimen showed diffuse monomorphous infiltration of leukemic cells in the dermis and subcutis. Bone marrow biopsy specimen showed changes of erythroleukemia. Treatment was initiated with combined chemotherapy which was a combination of cytosine arabinoside, adriamyein and 6 thioguanine. When combined chemotherapy was finished, she was in complete remission state and the nodules cured after 1 month of combined chemotherapy.

4.A Study on the Effect of Various Dosage of Khellin on Epidermal Melanocytes and a Comparison with Trimethylpsoralen in Systemic Photochemotherapy.

Ninety black C 57 BL mice were divided into control and photochemotherapy groups. Control group was subdivided into a UVA 1rradiation only subgroup and a khellin only administration subgroup, while the phototherapy group was subdivided into one TMP and two khellin administration subgroups with dosages of 1.5mg/kg, 5mg/kg and 20mg/kg, respectivly. UVA was administered three thimes a week in a dose of 4J/cm for 4 weeks. Skin biopsies were taken at 0, l, 2, 3 and 4 weeks and the split DOPA stain was employed to observe pigment production. The number, size and circumference of the melanocytes were assessed. In summary, our results inclicate that khellin, which is slightly less effective than TMP at the same effective dose, is quite effective for pigment production and the degree of the production is dose related.

This study was undertaken to investigate the effect of UVB radiation on epidermal melanoctes of C57BIL mouse, The results are summarized as follows : l) The numerical increase of DOPA positive melanocytes following UVB irradiation was due to both of active mitosis of epidermal melanocytes and the conversion of dormant. malanocytes to DOPA-positive melsnocytes. 2) Long term effects of UVB irradiatinn on the epidermal melanocytes suggest the persistent activation of typrosinase in UVB irradiated melanocytes.

Hematogenous contact dermatitis may be produced by the systemic administrationof a drug to an individeal who has been previously sensitized by the topical application of the drug or related chemicals. 37 year-old male patient had frequent rhus dermatitis. He took rhus with chicken for treatment of peptic ulcer, on next day generalized skin rashes with itching sensation were developed. the aptient was diagnosed as hematogenous contact dermatitis or systemic exzematous contact dermatitis according to the history, symptoms and the result of patch test.

7.Antioxidative Activity and Anti-melanogenic Effect of the Extract from the Leaves of Robinia Pseudo-acacia L.

BACKGROUND: Plant extracts have been widely used as important therapeutic drugs for many centuries all over the world. There have been many reports that natural products have various kinds of biological activities such as anti-allergy, anti-inflammatory and antimicrobial activities. Recently, the screening for the efficacy and safety of natural products has been extensively performed. OBJECTIVE: This study was carried out to find a beneficial plant extract possessing excellent antioxidative and anti-melanogenic activities. We have found that the leaf of Robinia pseudo-acacia L. has active substances which are involved in those activities. METHODS: To confirm the antioxidative activity of the extract obtained from the leaves of Robinia pseudo-acacia L., scavenging ability of the extract on DPPH free radicals and its inhibitory effects on lipid autoxidation and peroxidation were investigated. In addition, inhibitory effects of the extract on mushroom tyrosinase as well as melanin biosynthesis in cultured B16 melanoma cells were evaluated. RESULTS: The acacia extract showed not only powerful antioxidative activity but also antimelanogenic acitivity as strong as that of arbutin which is a well known inhibitor of melanogenesis. CONCLUSION: These resulis suggest that the extract from the leaves of Robinia pseudo-acacia L. could be used as a 4ghtening and antioxidative agent for the skin.

8.Nodular vasculitis Simulate to papulonecrotic Tuberculid.

The nodular vasculitis described by Montgomery on 1945 and is characterized by relatively chronic, persistent, or recurrent nodular lesions of nontuberculous origin chiefly on the legs. In the differential diagnosis the following must also be considered; erythema induratum, eythema nodosum, Weber-Christian disease, erythema nodosum migrans, recurrent thrombophrebitis and periarteritis nodosa. The authors observed one case of nodular vasculitis caused by sulfa drug. This patient was diagnosed to papulonecrotic tuberculid at first and treated by prednisolone 20 mg, INH 300 mg and streptomycin l.0 gm BIW. By the treatment, the patient, was cured completly within 3 months but visited again because of recurrence after 6 months. Therefore, same medication was given, but did not show improvment and added sulfa drug, Lederkyne. But, unfortunately the skin lesions was aggrevated after sulfa medication. It was suggestive that the cause of aggrevation was sulfa drug and discontinued the sulfa drug. The skin lesions were completely cured after the drug was discontinued.

9.A Clinical Evaluation of the Effects of Combination Photochemotherapy in the Treatment of Psoriasis with Etretinate and PUVA.

RePUVA therapy is the combination of an oral aromatic retinoid(etretinate) with oral photochemotherapy (PUVA) for the treatment of psoriasis. This combination therapy has the advantage of faster clearance with fewer side effects. Twenty three ambulatory psoriasis patients received RePUVA treatment. Etretinate was administered in a dosage of approxirnately 0.75 mg/kg body weight for 2 weeks and discontinued. One week after discontinuance of etretinate, PUVA therapy was begun. Complete clearance occured in 96% of the patients with an average of 9.5+4 irradiations and an accumulated ultraviolet A (UVA) dose of 67.5+42 J/cm. The primary side effects were dry rnucous membranes and mild cheilitis. Laboratory findings revealed no significant abnormalities before or after etretinate treatment.
