1.A Case of Cutaneous Cartilaginous Tumor.

A case of cutaneous cartilaginous tumor was diagnosed from solitary nadular skin lesion on a toe of 59-year old female. Lack of information about cartilage tumors in the dermatologic literature suggest that they rarely develop as supeficial lesions. General clinico-pathologic feature of the tumor is reviewed.

4.A Predictive Index of Left Ventricular Performance after Mitral Valve Replacement in Pure Mitral Regurgitation.

The index on myocardial contractility of pure mitral regurgitation(MR) after mitral valve replacement is believed to be useful in determining proper operation time before irreversible myocardial damage by volume overload. Thus the authors examined pre and post-operative echocardiographic results of 20 cases of pure MR patients who had been admitted to Pusan National University hospital and compared the usefulness of each index. Pre and post-operative echocardiographic results were as follows respectively ; 1) Left ventricular internal dimension in diastole(LVIDd) were 6.49+/-0.19cm and 5.51+/-0.17cm. 2) Left ventricular internal dimension in systole(LVIDs) were 4.26+/-0.12 and 3.79+/-0.18cm. 3) Ejection fraction were 71.28+/-1.57% and 59.24+/-3.05%. 4) Fractional shortening(FS) were 34.09+/-1.18% and 27.21+/-1.84%. 5) Mean Vcf were 1.53+/-0.08cm/sec and 1.18+/-0.09cm/sec. 6) Left ventricular end-diastolic volume index(EDVI) were 196.35+/-18.33cc/m2 and 98.46+/-9.96cc/m2. 7) Left ventricular end-systolic volume index(ESVI) were 55.28+/-5.12cc/m2 and 41.88+/-6.07cc/m2. 8) Left ventricular end-systolic wall stress/ESVI(ESS/ESVI) were 2.3+/-0.21 and 3.91+/-0.83. ESS/ESVI showed significantly(p<0.05) elevated postoperative change, LVIDd, LVIDs, EF, FS, EDVI, ESVI revealed significantly(p<0.05) reduced postoperative change whereas Mean Vcf had no postoperative change. After examining correlation between preoperative and postoperative results of each index, it is believed that LVIDd, LVIDs, and ESS/ESVI were useful prognostic indices but were unrelated to postoperative ejection phase indices such as EF, FS, and MEan Vcf. Especially in case of LVIDd>6cm, LVIDs>4cm, and ESS/ESVI<2, the prognosis was poor due to myocardial damage by mitral regurgitation.

5.Clinical Characteristics of Electric Cataract.

To evaluate the various factors related to the development of electric cataract in electric burn patients, we reviewed medical charts of 663 electric burn patients who were admitted to the department of General Surgery in Hanil General Hospital between 1981 and 1993. Eleven patients(1.7%) had electric cataract in both eyes. All of them were injured by contact with 22,900 voltage current, and developed third degree burns. Fifty-eight electric burn patients had their electric inputs through their head and eight (13.8%) among them developed cataracts. Only three(0.5%) among 567 electric burn patients who had their electric inputs through upper extremities developed cataracts. The interval between the electric injury and the diagnosis was 2 to 18 months. Anterior subcapsular opacity was the most common type of lenticular opacity. Other associated ocular complications included uveitis, macular edema, macular degeneration, and macular hole.

6.Study on the Nerve Regeneration in The Treansplanted Cornea-Grafts of the Rabbits.

The regeneration of corneal nerve in the graft after penetrating partial homo-keratoplasty had been observed histologically in the experimental rabbits and also the influence of thiamine hydrochloride on nerve regeneration had been studied. Cornea-grafts obtained from the central portion of the cornea by 6 mm trephine were transplanted each other among the rabbits and given 8 sutures in situ under deep sodium pentobarbital anesthesia. Thiamine hydrochloride were given daily does of 5 mg/kg body weight to the grouped rabbits intramuscularly. Enuculeations were done in 5 days, 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 14, 18, 20, 22, 24, 26, and 27 weeks after transplantation. Preparations were made with 10-15 micro paraffiserial sections, both of cross and flat, and modified Romanes' silver stain was applied. The results were as follows: 1) Control group: In 5 days after operation, fine nerve fibes were regenerated from the cutted nerve stumps of the host-cornea. After I weeks, nerve plexus were formed in the host-cornea around scar tissue and few fibers penetrated into the scar tissue. After 2 weeks, fine regenerating nerve fibers were found in the transplanted cornea-grafts. Thereafter, numbers and size of the regenerated nerve fibers increased gradually. In 9 weeks after transplantation, nerve innervation in the transplanted cornea-graft was similar to normal cornea. 2) Thiamine hydrochloride treated group: In 5 days after operation, fine regenerating nerve sprouts from the cutted nerves tumps in the host-cornea formed loose nerve plexus and few fibers penetrated into the scar tissue. After 1 week, regenerating nerve fibers were already found in the periphery of the transplanted cornea-grafts. In process of experimental time, nerve fibers in the cornea-grafts more actively regenerated compared with control group. In 7 weeks after homo-corneal transplatation, nerve innervation in the grafts was the same as normal cornea. 3) Thiamine hydrochloride remarkably accelerated nerve regeneration in the transplanted cornea-grafts after penetrating partial homo-keratoplasty.

7.A Case of Acanthoma Fissuratum Occurring on the Mucosa of the Upper Lip.

Acanthoma fissuratum is a localized thickening of the skin in response to pressure by an eyeglass frame and usually occurs on the retroauricular fold, superior auricular crease, or bridge of the nose. The definitive diagnosis can be confirmed by characteristic clinical and histopathological features in association with anamnestic data. Herein we report a case of acanthoma fissuratum developing on an unusual site-the mucosa of the upper lip and it was successfully treated with simple excision.

8.Effect of Excimer Laser Photorefractive Keratectomy on Goldmann Applanation Tonometry.

It can be speculated that the thinned and flattened central cornea after excimer laser photorefractive keratectomy (PRK) might give a falsely low Goldmann applanation tonometer(GAT) reading. We studied 353 eyes of 264 patients who underwent PRK to determine the effect of excimer laser PRK on the accuracy of Goldmann applanation tonometry. PRK was done with a VISX 20/20 excimer laser and topical fluorometholone was used after PRK. The intraocular pressure(IOP) was measured with the GAT before surgery and 1, 3, 6, 9, and 12 months postoperatively. The mean ablation depth of the cornea was 71.4 +/- 22.0 micrometer, and the eyes were divided into three groups according to their ablation depths. The mean postoperative IOP measured with the GAT decreased as compared with the mean preoperative IOP(P<0.01). The mean postoperative IOP at 9 or 12 months was lower than that at 1, 3, or 6 months(p<0.05). The mean IOP(11.9 +/- 2.5 mmHg) at 12 months was 2.8 mmHg lower than the mean preoperative IOP(14.7 +/- 2.7 mmHg). There was no statistically significant difference in the amount of postoperative IOP decrease among the three groups.

9.Difference between Goldmann Applanation and Topcon Noncontact Tonometer Measurements after Excimer Laser Photorefractive Keratectomy.

The authors' clinical experience has suggested that the noncontact tonometer (NCT) has a tendency to record a lower intraocular pressure(IOP) than the Goldmann applanation tonometer(GAT) after excimer laser photo refractive keratectomy(PRK). This study was done to verify this tendency. We performed PRK on 177 eyes of 122 patients using a VISX 20/20 excimer laser, and topical fluorometholone was used after PRK. The eyes were divided into three groups according to their ablation depths, and the IOP was measured with the GAT and the Topcon NCT before surgery and 1, 3, 6, 9, and 12 months postoperatively. The mean postoperative IOP measured with the GAT or the NCT decreased as compared with the mean preoperative IOP(GAT: 1.67 ~ 2.90 mmHg, NCT:5.27 ~ 6.54 mmHg). The mean NCT reading was 1.12 mmHg higher than the mean GAT reading preoperatively, but was 2.48 ~ 2.88 mmHg lower than the mean GAT reading postoperatively. Difference between GAT and NCT measurements postoperatively was more prominent in the group of deeper ablation. These results suggest that we should be cautious of evaluating the IOP measured with the GAT or the NCT after PRK.

10.Excimer laser photorefractive keratectomy for astigmatism.

The effectiveness of excimer laser photorefractive keratectomy (PRK) for astigmatism was evaluated. We treated 136 eyes of naturally occurring myopic astigmatism using the 193-nm excimer laser with an expanding slit and an iris diaphragm, and followed for a minimum of 6 months. At 6 months after operation, the mean refractive cylinder decreased from 1.62 +/- 0.88 D preoperatively to 0.48 +/- 0.48 D. For detailed analysis of the results, the surgically induced refractive change (SIRC) was determined from the preoperative and postoperative refractions by vector analysis of astigmatism. When we compared the cylinder of the SIRC with the preoperative refractive cylinder, the effect of the cylindrical ablation was 93.9 +/- 36.7% at 6 months. The axial error of the cylinder of the SIRC to the preoperative cylindrical axis was 5.9 +/- 10.2 degrees at 6 months. These results suggest that excimer laser PRK with an expanding slit appears to have a significant effect for the correction of astigmatism.
