BACKGROUND: To investigate the possibie difference, if any, between office blood pressure(BP) and home BP may be important in the diagnosis and treatment of hypersensive patients. This report deails the difference between the two BP's and the usefulness of digital electronic sphygmomanometer(DES) for self-monitoring of home BP. METHODS: The BP's of 14 patients with essential hypertension were measured with mercury sphygmomanometers at outpatient department by physician and with DES at home(twice a day) by the patients. Patients were followed up every 2 weeks for 4 weeks and previous 2 weeks' average home BP's were compared with the office BP's of each 2 weeks' end. RESULTS: There was a significant difference between office and home BP(both systolic and diastolic) ; office average BP(151/95mmHg) was higher than home average BP(136/86mmHg). CONCLUSION: Caution may be needed in the interpretation of office BP unless it is measured several times after adequate rest.

2.Symptomatic Epidural Pneumorrhachis : A Rare Entity.

Pneumorrhachis, which involves the entrapment of air or gas within the spinal canal, is a rare clinical entity, and the pathogenesis and etiologies of this uncommon entity are various and can present a diagnostic challenge. Usually, pneumorrhachis represents an asymptomatic epiphenomenon but it can produce symptoms associated with its underlying pathology. Here, we report a rare case of symptomatic epidural pneumorrhachis accompanying pneumothorax. Possible pathogenic mechanisms are discussed and a review of the literature is included.

3.Health related quality of life of the adult male smokers and nonsmokers in suwon city.

BACKGROUND: Smoking is the single most important factor for the prevention of diseases and the promotion of health. The objectives of this study are to compare the health related quality of life and other factors between the adult male smokers and nonsmokers. METHODS: Study subjects were selected from the parents of students of three schools; one elementary school, one middle school, and one high school in Suwon city, Kyungki Do. Among 2,308 respondents to the questionnaire, 1,127 were analyzed. The questionnaire consisted of questions of general socioeconomic status, health related quality of life questionnaire, Fagerstrom nicotine tolerance questionnaire, and the Why test. RESULTS: There were no significant socioeconomic differences between the smokers and nonsmokers except religion(p=0.000). The smoking rate of the subjects was 66.2%, the average amount of smoking was 18.28 cigarettes per day, and the mean nicotine dependence score was 3.99+/-2.33. Smokers ranked lower scores in all scales of the health related quality of life than nonsmokers. The health related quality of life of the group of high nicotine dependence was low in comparison with the group of low nicotine dependence. The most common reason of smoking was to relieve tension. CONCLUSION: Smokers ranked lower scores in all scales of the health related quality of life than nonsmokers.

4.A Case of Zoster Duplex Bilateralis.

Herpes zoster involving noncontiguous dual dermatomes is very rare in both immunocompetent and immunocompromised persons. This unique presentation has been referred to as zoster duplex unilateralis or bilateralis, depending whether one or both halves of the body are involved. A 22-year-old woman, who had been treated for acute leukemia, congestive heart failure and chronic disseminated candidiasis, was referred to our department for painful papulovesicular eruptions on the right side of the anterior chest and upper back for 2 days, and the left buttock for 1 day. Tzanck smear revealed multinucleated giant cells with intranuclear inclusion bodies. We report a rare case of zoster duplex bilateralis.

6.A Case of Unilateral Idiopathic Atrophoderma of Pasini and Pierini.

Idiopathic atrophoderma of Pasini and Pierini(IAPP) is an uncommon condition of unknown etiology and typically affects persons in the second and third decades of life. IAPP consists of asymptomatic grey to brown depressed lesions with "cliff drop" borders, which are usually bilateral and located on the trunk. A 40-year-old woman visited our department because of unilateral atrophic lesions on the abdomen, right arm, and posterior aspect of right thigh for 20 years. Laboratory evaluations were normal and histologic examination revealed significantly decreased thickness in dermis. We report an unusual case of idiopathic atrophoderma of Pasini and Pierini presenting unilateral distribution.

7.Comparison of the Efficacy and Vaginal Erosion Rate between Monofilament and Multifilament Polypropylene Tapes for Treating Urinary Incontinence.

PURPOSE: We compared the efficacy and vaginal erosion rates between a polypropylene multifilament tape(anterior intravaginal slingplasty; IVS) and a polypropylene monofilament tape(tension free vaginal tape; TVT) as surgical implants for suburethral, tension-free tape for the treatment of stress urinary incontinence(SUI). MATERIALS AND METHODS: We retrospectively reviewed the women(n=272) who underwent an anterior IVS(n=142) or a TVT (n=130) procedure for managing SUI, between January 2004 and December 2005 at Cheil General Hospital. Only 216 patients (the anterior IVS group: 112, the TVT group: 104) with a follow-up of at least 24 months were included in this study. Preoperatively, each patient received a history review, a physical examination, a voiding diary, a pad test, uroflowmetry and determination of the postvoid residual urine. Follow-up evaluations were performed at 1 month, 3 months and annually after the operation. RESULTS: The two groups were similar for their preoperative characteristics. There was no significant difference between the 2 groups in terms of the cure rate: cure(89.29% vs. 89.42%, respectively, p=0.974), improvement (6.25% vs. 7.69%, respectively, p=0.677), and failure(4.46% vs. 2.89%, respectively, p=0.539) for the anterior IVS and TVT groups, respectively. Nine women developed vaginal erosion, and this occurred significantly more in the anterior IVS group than the TVT group(7.14% vs. 0.96%, respectively, p=0.036). CONCLUSIONS: Anterior IVS and TVT appear to be equally effective for the surgical treatment of female SUI. However the number of women who were diagnosed with vaginal erosion was significantly higher in the anterior IVS group than in the TVT group. It is possible that the multifilamentous nature of the anterior IVS tape might contribute to vaginal erosion.

10.Isolated Spinal Neurocysticercosis : Unusual Ocular Presentation Mimicking Pseudotumor Cerebri.

Spinal intradural cysticercosis is a rare manifestation of neurocysticercosis. We report a unique patient who showed visual symptoms and normal imaging of the brain caused by isolated spinal neurocysticercosis. A 59-year-old male patient was admitted to the emergency unit with a history of severe headache and progressive blurred vision. Brain computed tomographic scanning and magnetic resonance imaging showed normal cerebral anatomy without hydrocephalus. The fundoscopic evaluation by an ophthalmologist showed bilateral papilledema. Perimetry studies revealed visual field defects in both eyes. With the diagnosis of pseudotumor cerebri, a lumbar tap was attempted; however, we could not drain the cerebrospinal fluid in spite of appropriate attempts. Lumbar magnetic resonance imaging revealed multilevel intraspinal lesions that were confirmed histologically to be neurocysticercosis. An intraoperative lumbar puncture revealed an increased opening pressure and cytochemical analysis showed elevated cerebrospinal fluid protein level. The headache resolved immediately after surgery. However, the visual symptoms remained and recovered only marginally despite antihelminthic medications after six months of operation.
