Proximal femoral nail anti-rotation (PFNA) with a lag screw that is shaped like a spiral blade shape is an orthopedic implant to fix trochanteric fractures of the proximal femur. In addition the reason of the biomechanical advantages, PFNA widely been used recently. We report an 83-year-old man with excessive sliding of the helical blade and a femoral neck fracture after AO/OTA type A2 intertrochanteric fracture, which was fixed with a PFNA.

4.The Results of Cementless Total Hip Arthroplasty for Primary Osteoarthritis Compared with Avascular Necrosis of the Femoral Head.

PURPOSE: Cementless total hip arthroplasty was performed to treat primary osteoarthritis of the hip and avascular necrosis of the femoral head. The clinical and radiological results of the acetabular cups and femoral stems were compared. MATERIALS AND METHODS: The subjects were 84 cases who underwent primary cementless total hip arthroplasty from March 1992 to May 2001. The 84 cases were followed up for at least eight years. The patients were divided into the following two groups: group (A), 38 cases of degenerative osteoarthritis of the hip; and group (B), 46 cases of avascular necrosis of the hip. The clinical evaluation was conducted based on the Harris Hip Score (HHS). A radiological assessment was performed to determine if osteolysis or acetabular loosening had occurred. The results were analyzed statistically. RESULTS: The postoperative HHS of groups A and B at the final follow-up were 92.1 and 91.2, respectively (p=0.483). The radiological results revealed 9 and 15 cases of acetabular osteolysis (p=0.671) and 1 and 3 cases of vertical migration of the acetabular cups in the group A and B, respectively. Eight and 7 complications (p=0.572) were encountered in group A and B, respectively. Among these, 4 and 5 cases of revisional total hip arthroplasty were performed in group A and B, respectively. The results of survival analysis were similar in the two groups (p=0.969). CONCLUSION: The clinical and radiological features of the acetabular cups and stems were similar in the two groups. Nevertheless, further studies will be needed due to the high rate of polyethylene liner wear and osteolysis around the acetabular cups or femoral stems.

The patient was a 11 years old boy, who was well after the radical removal of the tumor with concomitantly performed internal decompression. The propolis, honey bee hive product, is a folk medicine for treating various ailrnents and caffeic acid phenethyl ester (CAPE) is an extract of propolis. The purpose of this study was to examine the effects of ethanol extracted propolis (EEP) or CAPE on the tumorigenesis, pulmonary metastases, and proliferation and activity of splenocytes and macrophages in ICR mice. EEP at 0.2, 2 or 20mg/ml applied topically on the back of each mice 30 minutes before application of 7,12-dimethylbenz (a)anthracene and 12-0-tetradecanoylphorbol-13-acetate inhibited the number of tumors per mouse by 61, 75 or 100%, respectively. ...continue...

8.A Clinical Study of Benign Epilepsy of Childhood with Centrotemporal Spikes.

Benign epiepsy of childhood with centrotemporal spikes (BECCT) is an electroclinical syndrome characterized by noctural seizure that remit spontaneoully before adulthood, and belong to idiopathic age and location related epilepsies. We reviewed the medical records to analyse the seizure the seizure pattern, and also inspect the EEG recording to identify topography of the epileptiform discharge of 24 patient who met the following criteria: 1) presence of nocturnal seizure or partial seizure confined to the face, 2) normal intecual and development, 3) normal background EEG feature, 4) monomormhic spikes or sharp wave that increase in the frequency during sleep if those state were achieved. The result as follow: 1) The age of seizure onset were 3 to 11 years old (7.08 1.89) and male to female ratio was 1, 4:1. 2) Past history of birth trauma, CNS infection and other diseases involving CNS, were not found. There was past history of febrile convulsoin 33.3% of patient and epilepsy in 8.3%. One of paient's sister had childhood abscence epilepsy and never a nocturnal partial seizure, also and had centrotemporal spikes in EEG. 3) In 91.7% of Seizures occurred during sleep, especially short after sleep onset and the symptomatology of seizure was shown 66.7% of oropharyngeal sign, such as hypersalivation and gargle sound, and 54.2% of hemifacial involvement, 33.3% of speech arrest, 25.5% of upper limb involement, 18.3% of lower limb involvement, and 45.8% of general seizure. 4) In 91.5% of patients had on location of epileptiform discharge that located central (37.5%) midtemporal (33.3%), parietal and frontal. We could observe tangental dipole in 16.7% of patients.

9.The Exerimental Study on Possibility of Development of Lenticular Opacity after Argon Laser Retinal Photocoagulation.

Lenticular bum can occur as complication after laser iridotomy or retinal photocoagulation. Some authors have, indeed, reported the cases in which focal lenticular opacity developed after laser iridotomy or retinal photocoagulation. Fortunately, it was not progressive in short-term follow up study. When argon laser beam penetrates ocular media, a portion of it is absorbed by lens. So visible lenticular opacity is not developed after argon retinal photocoagulation if lens is clear. But it is not known whether subclinical lenticular opacity can occur after argon laser retinal photocoagulation. We focused argon laser beam onto anterior lens capsule in 35 black and brown rabbit and evaluated development of subclinical lenticular opacity with densitometer 1 and 8 weeks later. We also analyzed lens protein 1 week later. Argon laser energy used was below 20 Joules. The following results were obatined. 1. Lenticular opacity was detected by densitometer in all cases, but it was not detected by slit-lamp. 2. Amount of lenticular opacity was in direct proportion to argon laser energy(p<0.001). 3. Lenticular opacity detected on the 1st week by densitomer was not detectable on the 8th week. 4. The lens protein changes of senile cataract were not found in the lenticular opacity detected on the 1st week.

10.Fetal bone marrow stromal cells support the in vitro expansion of human hematopoietic bone marrow progenitor cells: requirement for cell-to-cell interactions and colony-stimulating factors

Objective: To elucidate the role of bone marrow stromal cells in cooperation with exogenous cytokines in hematopoiesis. Methods: Fetal bone marrow stromal cells (FBMSC) was combined with cytokines including SCF,IL-3,IL-6,GM-CSF in a 5-day liquid culture system of adult bone marrow mononuclear cells, then we cultured bone marrow derived CD34+-enriched cells with FBMSC+SCF+IL-3+IL-6+G-CSF+EPO for 2 weeks. Results:FBMSC were in good cooperation with above mentioned exogenous cytokines. When CD34+-enriched cells from adult bone marrow were cultured with combinations of FBMSC, SCF, IL-3, IL-6, G-CSF and EPO, total nucleated cells, CFU-GM, BFU-E and CD34+ cells were increased by 119.6±30.9, 54.6±17.4, 25.2±4.4, 11.1±4.2 folds, respectively. Conclusion:FBMSC in cooperation with exogenous cytokines support the in vitro expansion of human hematopoietic progenitor cells efficiently.
