2.Relation between Coronary Artery Cross Sectional Area and Left Ventricular Wall Mass.

The coronary artery cross sectional area (CSA) is proportional to LV mass. We have measured the cross sectional area of the left and right coronary arteries in patients with ischemic heart disease to see whether it is related to the change in the LV mass. The following results were obtained ; 1) There were no significant difference in mean CSA of coronary arteries and LV mass determined by echocardiography and cineangiography between control and ischemic heart disease. 2) There were significantly increased ratio of left ventricular mass by cineangiogram to CSA of left anterior descending coronary artery in patients with myocardial infarction as compared with control group. 3) A linear relation between LV mass by cineangiogram and CSA of left coronary artery was noted in control group (r=0.53, P<0.05) and ischemic heart disease group (r=0.51, P<0.05). 4) A linear relation between LV mass determined by echocardiography and CSA of left coronary artery was noted in control group (r=0.55, P<0.05).

3.MRI of Vertebral Compression Fractures: Differentiation between Benign and Malignant Causes.

PURPOSE: To evaluate the MR image in the differentiation of benign and malignant lesion in compression of the vertebral body. MATERIALS AND METHODS: MR images of 47 benign(acute traumatic within one month:19, chronic traumatic longer than one month or nontraumatic:28) and 21 metastatic compression fractures were respectively reviewed in terms of margin of lesions, signal intensity, paraspinal mass formation, soft tissue change, and involvement of posterior element of vertebra. MR images of TI-(T1WI) and T2*-weighted gradient echo (GE T2WI) sequences were obtained on 0.5T unit in sagittal and axial orientation with 5mm section thickness. RESULTS: The margin of benign compression fracture was usually indistinct (acute fracture:90% (17/19), chronic fracture:68% (19/28)), whereas it was sharply delineated in metastatic compression fracture (92%, (12/13) (p<0.001). Paraspinal mass was seen in both acute traumatic and metastatic compression fractures (acute fracture :26% (5/19), metastatic fracture: 52% (11/21). Soft tissue change was seen only in acute cornpression fractures (58%, 11/19). Involvement of posterior element of vertebra was noted in metastatic fracture (71%, 15/21), acute fracture (32%, 6/19) and chronic fracture (7%, 2/28) CONCLUSION: On MR imaging, involvement of entire portion of a given vertebral body, sharp margin between normal and abnormal areas in partially involved cases, paraspinal mass formation, and posterior element involvement are more frequently seen in metastatic compression fractures, which are considered to be useful in differentiation between benign and malignant causes of compression fracture.

4.Facial Cutaneous Mycosis by Paecilomyces lilacinus.

A 19-year-old man noticed a very slowly spreading erythematous macular lesion on his cheek when he was 5 years old. This asymptomatic and well circumscribed patch was covered with fine scales. He sought first medical attention at the age of 12 years and diagnosis was cutaneous tuberculosis was made by skin biopsy. Seven years after the second biopsy was made and it revealed a few spores within giant cells in the upper dermis. By mycologic study, it was identified as Paecilomyces lilacinus. The patient was put on the treatment of topical and oral antifungal agents. The skin lesion showed signs of improvement after 12 weeks of treatment.

5.A Case of Hair Follicle Nevus.

Hair follicle nevus is one of several nevi, in which hair and hair follicle are involved. Crowding of numerous tiny but mature hair follicles with thick fibrous root sheath are the characteristie histologic finding of the nevus. The patient was one month old male infant with pedunculated. mass l cm in size on the left preauricular area. Histologically the skin of the lesion is covered by acanthotic epidermis. In the corium are numerous small mature hair follicles with thick fibrous sheath and a few small Iobules of sebaceous gland embedded in a fibrovascular stroma. There are, however, no central plate of cartilage and adipose tissue.

6.A STRESS ANALYSIS OF THE IMPLANT: SUPPORTED OVERDENTURE USING STRAIN GAUGE.

Stress distribution on mandibular implants supporting overdentures were registered in vitro experimental model by means of 4 rosette gauges which were placed around the implant. The overdenture attachments used in this study were the Resilient Dolder bar, Rigid Dolder bar, Round bar, Hader bar&Dal-Ro attachment. An occlusal jig was placed on the overdenture and the loading sites were 3 points which mimicked working, balancing, and median relations. With 5 and 10kg loading, strains were measured by strain indicator(P-3500, Measurement group, Raleigh, USA), and using these data, maximum and minimum principal stresses and Von Mises stress were calculated and evaluated. The results were as follows: There was a tendency of high stress concentration in the lingual side of the implant, and in the buccal side low stress was developed regardless of the attachment systems. The resilient Dolder bar concentrated highest stress among the attachment systems, and the Round bar and the Dal-Ro attachment provided comparatively low stresses around the implant. The rigid Dolder bar concentrated high stress in the mesial side, and the Dal-Ro attachment developed tensile stress patterns in the lingual and distal sides of the implant at the balancing relation.

7.Two cases of External Carotid-Opthalmic Anastomosis in Takayasu's Arteritis.

Takayasu's arteritis is the chronic inflammatory panarteritis of unknown pathogenesis, affecting aortic arch and main arotic branches, pulmonary and renal arteries. It mainly affects young oriental women and cause chronic ischemic injury to tissues of the brian, orbits, upper limbs, myocardium, and kidneys. The ophthalmologic features are tend to be late the manifestations of Takayasu's arteritis and include ischemia of the retina, choroid, and anterior segment. We experienced two cases of Takayasu's arteritis whose ipsilateral ophthalmic artery was completely occluded. yet maintained proper visual acuity by external carotid-ophthalmic anastomosis.

Endophthalmitis is one of the most catastrophic complications in intraocular surgery and penetrating injuries of the eye and intraocular injection of gentamicin is frequently used for its treatment. But the use of gentamicn resulted in sight-limiting complication such as cataract and the increased calcium level in the lens is an important factor of cataractogenesls. In this experiment, Ca++ - achivated ATPase activity, active Ca++ efflux and calcium concentration in the lens were measured at day 1, day 3 and 7 days after injection of gentamicin into the anterior chamber or vitreous to compare the effect on calcium transport in the rabbit lenses according to the route of administration. Ca++ -activated ATPase activity was progressively inhibited, active efflux of Ca++ was significantly decreased and calcium concentration progressively increased. So, it may be speculated that the decreased active efflux of Ca++ from the lens is related to inhibiting Ca++ -activated ATPase activity and this makes the accumulation of calcium in the lens. These effects were more severe in the intravitreal injection than in the intracameral injection group.

10.Two Cases of Basal Cell Carcinoma Treated with Recombinant Alpha-2 Interferon.

Two patients with basal cell carcinoma (BCC) were treated with recombinant alpha-2 interferon. One patient was a 48-year-old man who had a 8 × 5 an sized ulcer with an elevated border on the face for 10 years, which revealed BCC of the solid type, histopathologically. He received intralesional injections of recombinant alpha-2 interferon (total dose: 51 × 10⁶ IU) for 11 weeks. The biopsy specimen after completion of injections showed a remaining small tumor cell nest in the lesion despite clinical remission. The other patient was a 75-year-old female, who had 1.2 × 1.2 cm-sized, darkly pigmented, nodular BCC of the solid type on the nose for 3 years. She also received intralesional injections of recombinant alpha-2 interferon (total dose: 18 × 10⁶ IU) for 8 weeks. The biopsy specimen after completion of injections did not show any tumor cells.
