1.A Case of Acrokeratosis Verruciformis of Hopf.

A case report of a 6p-year-old man with crokeratosis verruciformis on the dorsa of hands & forearms is presented. This case is unusual in that the patient had late onset of the lesions and lack of a family history of the condition. Literatures are reviewed and possible linkage with Darier's disease is discussed.

2.A Mycological and Immunologic Study of Patients with Concurrent Superficial Fungal Infections.

This study was to investigate the relationship between the causative strains and lesional sites and to detect the status of cell mediated immunity in the patients with concurrent fungal infections. The authors performed mycological and immunologic studies on 32 patients with concurrent fungal infections who had visited the dermatologic clinic of Soonchunhyang University HospitaI from July 1982 to June 1983. The results were summarized as follows: It was 18 cases that had the positive cultures in all the lesion sites. Among them 12 cases obtained the same causative strains in all lesions of each patierit. The cultured dermatophytes were Trichophyton rubrum, 10 stains, Trichophyton mentagrophytes, 2 strains. 2. Positive inrnediate hypersitivity reaction was observed only in patients with concurrent fungal infection by trichophytin (87. 5%). 3. For the delayed hypersensitivity reaction, the mean skin reaction scores to PPD, candidin, trichophtin were l. 58, 2. 78, 0. 66 respectively in 32 patients with concurrent fungal infection and 1. 89, 2. 47, l. 50 respectively in 17 controls, but the statistically significancies were only in trichophytin (p<0. 05). 4. The mean percentage of total and active T cell counts in peripheraJ blood were 55. 2%, 23. 9% respectively in 18patients, and 64,0%, 29. 8% in controls(p<0. 05). There was a significant decrease of T-cell count in patients with concurrent fungal infections.

3.Lichen Planopilaris.

A 26-year-old Korean woman had a 5 month history of pruritic, keratotic papules on the scalp, trunk, and pubic area, which were accompanied by diffuse hair loss from the scalp, eyebrows and pubic area. She also had flat-topped violaceous papules on the scalp and pubic area, and white lace-like striae on the oral mucous and hard palate. Histopathologic study of the progressive stages of the disease revealed dilated follicles filled with horny material, hyperkeratosis, hypergranulosis and hydropic degeneration of the basal cell layer. Throughout the entire subepidermal region, there was an intense bandlike infiltration of mononuclear cells, which were most prominent at the lower pole of the hair follicles. The late stage of the disorder showed atrophy of the epidermis and atrophy of the hair follicles with fibrotic tracts, and mild mononuclear infiltrate at the sites of the former hair follicles. This is the first recorded case, to the best of cur knowledge, in Korea.

4.A Case of angioimmunoblastic Lymphadenopathy with Dysproteinemia.

We present a case of angioimmunoblastic lymphadenopathy with dysproteinemia (AILD) in a 57-year-old male who had suffered from fever and general weakness of one month's duration. He showed generalized lymphadenopathy, hepatomegaly and purpuric maculopapules on the whole body surface. Lahoratory findings show anemia, polyclonal gammaglobulinemia and thrombocytopenia. The lymph node biopsy specimens show morphologic triad of AILD except lack of interstitial deposits of eosinophilic materials and the skin biopsy specimen shows lymphohistiocytic vasculitis. Bone marrow biopsy specimen shows similar findings in lymph node, suggesting bone marrow involvement. He have been treated with COPP chemotherapy.

5.A Case of Epidermodyspalsia Verruciformis Associated with Squamous Cell Carcinoma.

A case of epidermodysplasia verruciformis associated with squamous cell carcinoma occurred in 21 years old male. He had extensive flat-topped papules on the face, neck and the upper trunk, and verrucous lesions on the extremities, especially on the dorsa of hands and feet for 15 years. Several peanut sized ulcerative lesions on the forehead were noticed for 1 year and identified as squarnous cell carcinomi. The akin test with dinitrochlorobenzene, tuberculin, phytohemagglutinin, candidin and dermatophytin showed alI negative. T and B-cell counts were within norrnal limits, but IgG and IgD levels in serurn were incresed.

6.A Case of Nevus Sebaceus Syndrome Considered as a Neurocutaneous Syndrome.

Nevus sebaceus is an ozganoid nevus of sebaceous differentiation first described by Jydassohn in 1895 and commonly develops on scalp or face as a single lesion. But it may be multiple end extensive, and often associated with abnormallities-convulsion, megtal retardation, neurological defects, skeletal defects, or ocular abnormalities. We have experienced a case of 22-year-old male who has since birth had- multiple nevus sebaceus on she left side of face and trunk, epidermal nevi on parts of the face, and wide nevus spilus and numerous pigmenteri nevi on the left side of trunk and the left upper extremity. He has histories of convulsions and mental retardation. EEG showed minimal abnormality. Skull x-ray and other routine examinations were within normsl limits. We report a case of nevus sebaceus syndrome which may be considered as a neurocutaneous syndrome.

7.Effect of UV - B Radiation on Epidermal Melanocytes of C 57BL Mouse.

This study was undertaken to investigate the effect of UVB radiation on epidermal melanoctes of C57BIL mouse, The results are summarized as follows : l) The numerical increase of DOPA positive melanocytes following UVB irradiation was due to both of active mitosis of epidermal melanocytes and the conversion of dormant. malanocytes to DOPA-positive melsnocytes. 2) Long term effects of UVB irradiatinn on the epidermal melanocytes suggest the persistent activation of typrosinase in UVB irradiated melanocytes.

8.Five Cases of Cutaneous Leishmaniasis.

Cutaneous leishmaniasis is a self-limited infection of the skin caused by Leishmania tropica, a protozoan parasite transmitted by Phlebotomus sandfly. This disease is endemic in tropical and subtropical zone, but recently many cases of cutaneous leishmaniasis were reported in Korean among the peoples who had been worked in the Middle East. We experienced five cases of cutaneous leishmaniasis developed in Korean who had been worked in Saudi Arabia and Jordan. Skin lesions of various sized, central ulcerated and marginal elevated, with serosanguinous discharge and some crust were scattered on exposed area, especially both extremities. Histopathologic findings showed hyperkeratosis, acanthosis, pseudoepitheliomatous hyperplasia, dermal infiltration of histiocytes, lymphocytes, plasma cells and neutrophils, and numerous Leishman-Donovan bodies in and around histiocytes. About 1-4 months treatment with trimethoprim-sulfamethoxazole and metronidazole, two cases were healed with hyperpigmented scar and two cases were improved but one case was not responded.

9.Effects of pH Differences on the Skin Moisturizing Capacity of Natural Moisturzing Factors.

The flexibility of surface layer of skin is largely dependent upon the layer's water content. This, in turn, depends primarily on the quantity of water soluble hygroscopic and surface active msterials within the straturn corneum, often known as the natural moisturizing fsctors(NMF). It consist of amino acid, lactic acid, pyrrolidone carboxylic acid and inorganic salts etc.. Some of these components are greatly influenced by hydrogen ion content and ionic form of them were found to be highly hygroscopic. In this experiment, sunburn exfoliated human stratum corneum and pig epidermis separated by 2N-NaBr were used as in vitro test. These tissues were so treated to hsve each pH value (3, 5, 7, 9) and their water holding capacity and hygroscopicity were measured by gravimetric method. The results are summarized as follows : 1, Water holding capactiy of sunburn exfoliated human skin showed significant increase at pH 5 (p<0.1), 7 (p<0.01), 9 (p<0.01) compared to pH 3, and increase in pH 7 (p<0.1), 9 (p<0.05) compared to pH 5 in lactic acid. In pig epidermis, there was significant increase at pH 5 (p<0.05), 7 (p<0.01), 9 (p<0.05) cornpared to pH 3 in lactic acid. 2. Hygroscopicity of pig epidermis showed significant increase at pH 5 (p<0.05), 7 (p<0.1), 9 (p<0.05) in lactic acid and at pH 5

10.Accessory Axillary Breast Tissue.

We have experienced a case of the more unusual form of supernumerary breast tissue which is characterized by the presence of aberrant gland tissue alone. The case was a 30-year-old female patient in puerperium with bilateral axillary masses of seven years duration. Gentle pressure over the right-sided one caused milk to exude from a skin pore. Histopathologic findings of the tumors revealed the lobules of the breast forming islands of glandular tissue.
