2.A Case of Nevus Sebaceus Syndrome Considered as a Neurocutaneous Syndrome.

Nevus sebaceus is an ozganoid nevus of sebaceous differentiation first described by Jydassohn in 1895 and commonly develops on scalp or face as a single lesion. But it may be multiple end extensive, and often associated with abnormallities-convulsion, megtal retardation, neurological defects, skeletal defects, or ocular abnormalities. We have experienced a case of 22-year-old male who has since birth had- multiple nevus sebaceus on she left side of face and trunk, epidermal nevi on parts of the face, and wide nevus spilus and numerous pigmenteri nevi on the left side of trunk and the left upper extremity. He has histories of convulsions and mental retardation. EEG showed minimal abnormality. Skull x-ray and other routine examinations were within normsl limits. We report a case of nevus sebaceus syndrome which may be considered as a neurocutaneous syndrome.

3.Effect of Cyclosporine A on Plasma and Urine Levels of Endothelin-1 in Steroid Dependent Minimal Change Nephrotic Syndrome.

A fine needle aspiration biopsy specimen of a retroperitoneal mass in a 26-year-old man who had had an orchiectomy for seminoma was submitted for cytologic evaluation. Cytologic features of the specimen included uniform neoplastic cells found singly or in groups of several cells intermingled with lymphocytes in a characteristic foamy, lacelike background. These cells varied from 10 to 20 m in diameter. The nuclei were round to ovoid with fine or reticular chromatin and one or more prominent nucleoli. The poorly defined cytoplasm stained pale-blue or blue with cytoplasmic vacuoles. The cytologic appearance was consistent with seminoma. Documented reports of the cytological appearance of seminoma are rare. The diagnosis of primary gonadal seminoma by fine needle aspiration biopsy is probably not indicated since the treatment of a primary gonadal tumor, regardless of its histogenesis, requires surgical resection. However, fine needle aspiration biopsy is extremely valuable in the diagnosis of extragonadal as well as metastic and recurrent seminoma.

4.A Study on the Effect of Antihistamine in Patch Test Reaction.

The patch test reaction is artificially induced allergic contact dermatitis to prove the cause of allergic contact dermatitis. Allergic contact dermatitis is classically regarded as a delayed hypersensitivity composed of the simple cutanous infiltration of lymphocytes and macrophages. Recently several authors observed new morphologic findings in allergic contact dermatitis including infiltration and piecemeal degranulation of basophils, degranulation and replication of fixed tissue mast, cells, increased vascular permeability leading to dermal and epidermal edema, microvascular alterations affecting endothelial cells and pericytes, and activation of the clotting system, with deposition of fibrin in a characeristic inravascular pattern in the reticular dermis. And they proposed new hypotbesis that the vaoactive amines released by basophils and mast cells have the crucial role in eliciting allergic contact dermatitis. We studied the effect of antihistamine on allergic contact dermatitis caused by dinitrochlorobenzene(DNCB). with phenirarnine maleate,(Avil) 136.5mg/d in 15 young adult male persons. Clinically the degree of the skin reaction is decreased in 9 of 15 persons after tie ingestion of pheniramine maleate. and histopathologically the epidermal and dermal changes are ameliorated in accordance with the clinic':1 improvement. Our results indicate that the histamine also participates in the pathogenesis of allergic contact dermatitis.

5.Outpatient General Anesthesia for Mentally and Physically Handicapped Children Undergoing Extensive Dental Treatment.

BACKGROUND: Fear of dental treatment is a very real problem for many people. Very young patients or children that are mentally or physically handicapped have various problems that preclude routine dental treatment in the office and require general anesthesia for extensive dental restoration. In America, outpatient operations are performed in thousands of dental offices annually, but there is no report about outpatient general anesthesia in Korea. METHOD: A review of forty children treated under outpatient general anesthesia for extensive dental treatment between 1994 and 1996 inclusive was carried out to assess the patient selection, anesthetic method, recovery time and complication. RESULTS: The mean age was 8.4 years, and twenty-five percent of the patients were autism. The length of the postoperative observation period before discharge was 3.2 hours, and postoperative fever was major complication. CONCLUSION: For extensive dental treatment in handicapped children, we suggest that outpatient general anesthesia can provide reasonably safe treatment, while reducing its expense and requiring less hospital bed space.

7.Osteochondrodysplasia Pathologic study of 29 autopsy cases.

Osteochondrodysplasia is a heterogeneous group of disorders appearing short limbed dwarfism. Because many of these entities are lethal and hereditary, an accurate diagnosis is mandatory. The purpose of this study is to define the clinicopathologic features and radiologic findings of osteochondrodysplasia. We reviewed 29 autopsy cases of congenital short limbed dwarfism, consisting of thanatophoric dysplasia (TD) (12 cases), osteogenesis imperfecta (OI) (12 cases), asphyxiating thoracic dysplasia (ATD) (3 cases), short-rib-polydactyly syndrome (SRPS) (1 case) and hypochondrogenesis (1 case). The gestational age ranged from 16 to 41 weeks. Of 6 fetuses that were born alive, 3 were ATD, 2 were TD and 1 was hypochondrogenesis. TD was frequently complicated by hydramnios. Of 8 cases studied chromosomally, only 1 showed chromosomal abnormality -46XY, inv 9. Intrauterine growth retardation was frequently associated with OI. Pulmonary hypoplasia was present in 23 cases (79%), including all cases of ATD, SRPS and hypochondrogenesis, 11 in TD and 7 in OI. Other associated anomalies were present in 17 cases (59%).

10.Effects of Adenosine Triphosphate on Relaxation of Rabbit Cavernosal Smooth Muscle.

Externally applied acetylcholine(Ach) in corpus cavernosum has been shown to cause endothelium dependent smooth muscle relaxation. ATP is accepted as a relaxant of smooth muscle by both a direct action and more powerful indirect action via the endothelial cells. Endothelium-derived relaxing factor(EDRF) is released with stimulation of acetylcholine or other endothelium dependent substances raise cGMP level within the smooth muscle cell and cause relaxation of smooth muscle. EDRF is known as nitric oxide(NO) and its actions are abolished by specific inhibitor of nitric oxide synthesis, such as L-n-monomethyl arginine(L-NMMA) or inhibitors of cyclic GMP synthesis, such as methylene blue(MB). In this study, we evaluated the action of ATP related with NO and compared effect of ATP with acetylcholine and bethanechol chloride in rabbit corpus cavernosal smooth muscle under organ bath. Changes in isometric tension of corporal strips were monitored. With pretreatment L-NMMA or MB, relaxing effects of acetylcholine or bethanechol chloride in corporal strips were completely inhibited, but relaxing effects of ATP were not altered. These data suggested that nitric oxide plays a crucial role in cholinergically induced cavernosal smooth muscle relaxation. ATP mediated rabbit corporal smooth muscle relaxation was not affected by inhibitors of nitric oxide synthesis and independent of cyclic GMP accumulation.
