Objective To evaluate the effiicacy of nasotracheal intubation using blind tracheal intubation device and end-tidal carbon dioxide monitoring technique in patients with difficult airway.Methods Sixty ASA Ⅰ or Ⅱ patients of both sexes,aged 35-60 yr,weighing 55-75 kg,requiring nasotracheal intubation,undergoing selective oral and maxillofacial surgery,were involoved in this study.All of them were expected to have difficuh airway such as mouth opening ＜ 3 cm,neck upward degree ＜ 30° and Mallampati classification Ⅲ or Ⅳ,The patients were randomly divided into 2 groups(n =30):group Ⅰ received tracheal intubation with the blind tracheal intubation device and group H received tracheal intubation with the blind intubation device combined with end-tidal carbon dioxide monitoring technique.The number of successful intubation,the inlubation time and adverse effects during intubation(epistaxis,tachycardia,hypertension and hypoxemia)and after operation(sore throat and hoarseness)were recorded.Results The rate of successful intubation in the two groups was 100％.Tachycardia,hypertension,hypoxemia and hoarseness were not found in the two groups.Compared with group I,the rate of successful intubation at first attempt was significantly increased,the intubation time was significantly shortened,and the incidence of epistaxis and sore throat was decreased in group Ⅱ(P ＜ 0.05).Conclusion The blind intubation device combined with end-tidal carbon dioxide monitoring technique used in diffficult airway can shorten the intubation time,increase the success rate of tracheal intubation,and decrease the occurrence of adverse effects.

Objectives:To observe the effect of rhGH in the treatment of chronic wasting diseases. Methods:Twenty two cases of chronic wasting diseases needing nutrition support were chosen,including 10 cases of COPD,12 cases of cerebral infarction or cerebral hemorrhage with decubital ulcer in recovery phase.They were in the age of 60～75,in combination with morderate malnutrition.These patients were divided into control group and experimental group.In the experimental group,rhGH(4.5 U/d) was given hypodermically for 10 days.Serum albumin,prealbumin,transferin,24 h urea nitrogen in urine,the surface area of decubital ulcer and mental state were monitored before and after the treatment. Results:In the experimental group,many nutritional indices were increased markedly,24 h urea nitrogen excretion was decreased,the healing of decubitus was faster and mental state was better when compared with control group. Conclusions:The rhGH plays an important role in improving the nutritional status and mental state in patients with chronic wasting diseases. On the base of supplying enough nutrients, rhGH can promote anabolism,reduce the excretion of urea nitrogen in urine, accelerate the healing of decubital ulcer and improve the prognosis.

Objective To investigate the relationship of intraoperative cerebral oxygen saturation(rSO_2 )monitored with near-infrared cerebral oximeter (INNOS 5100) and postoperative cognitive dysfunction in patientsoperated upon under isoflurane or sevoflurane anesthesia and to determine the critical rSO_2 value below whichpostoperative cognitive dysfunction may occur.Methods Sixty ASA Ⅰ- Ⅱ patients of both sexes aged 62-80 yr,weighing58-77 kg schedules for elective abdominal surgery or surgery on the low limb were divided into threegroups according to their levels of education: group Ⅰthe illierate and uneducated (n = 20); group Ⅱ primaryschool education (6yr education (n = 20). Object:To find out college students' perceptions about counseling.Methods:By using a questionnaire about college students' view of counseling,875 college students of Zhejiang University from grade 1 to grade 4 were tested.Results:Most students acknowledged that counseling deals with mental issues and disorders. They were willing to help clients,and prefer counselors who are like their friends and family members.They agreed that counseling is helpful and,prefer one-hour sessions,which are free of charge.
