1.Changes of the autonomic nervous system during induction and intubation of children with cerebral palsy

ObjectiveTo investigate the changes of the autonomic nervous system during induction and intubation of children with cerebral palsy.MethodsThirty children with cerebral palsy (group A) and thirty children without cerebral palsy disease (group B) scheduled for abdominal operation were selected. Heart rate variability (HRV) was assessed before operation, after anesthesia induction and after trachea intubation using heart rate power spectrum analysis (HRPSA).ResultsAfter anesthesia induction, low frequency (LF), high frequency (HF), LF/HF and total pull (TP) decreased significantly in group A, and LF, LF/HF and TP decreased significantly in group B. After trachea intubation, LF, HF, LF/HF and TP increased significantly in both groups. During induction and intubation, HF and TP in group A was lower than that in group B significantly (P<0.05~0.01), but LF did not changed markedly in both groups.ConclusionParasympathetic nervous activity and the total power of autonomic nervous system are decreased significantly during induction and intubation in children with cerebral palsy compared with those without cerebral palsy, indicating that the effect of parasympathetic nervous activity is impaired.
2.Effect of etomidate and ketamine on the heart rate variability of children with cerebral palsy

ObjectiveTo observe effect of etomidate and ketamine on the cardiovascular function of children with cerebral palsy.Methods84 cerebral palsy cases were divided randomly into the etomidate group and ketamine group with 42 cases in each group, when they were treated with lower extremity orthopedic operation and anaesthetized by extradural continuous block anesthesia. The etomidate group was injected with etomidate and ketamine group was injected with ketamine. The heart rate variability (HRV) was recorded during anesthesia course.ResultsHeart rate and HRV of the etomidate group decreased significantly (P<0.05-0.01), but that of the ketamine group had no significantly differences (P>0.05).ConclusionEtomidate has a significant influence on cerebral palsy children's cardiovascular function in anesthesia course.

By means of qualitative and quantitative methods, it made validity and reliability measurement on the measurement tools in use. In addition, it extracted the structure validity of the factor regulating scale via a factor analysis, and developed the scale for clinical information system measurement These efforts provided the hospitals with the useful measurement tool to improve their information system, promoting positive and practical hospital informationziation.

7.Clinical Observation of Vaginal Administration of Lactobacillus Combined with Chlorquinaldol-promestriene for Senile Patients with Atrophic Vaginitis

OBJECTIVE:To investigate therapeutic efficacy and recurrence rate of vaginal administration of lactobacillus com-bined with chlorquinaldol-promestriene for senile patients with atrophic vaginitis. METHODS:150 senile patients with atrophic vag-initis were selected and divided into group A,B,C according to random number table,with 50 cases in each group. Group A was given Lactobacillus vaginal capsules,2 capsules,qd (at bedtime);group B given Chlorquinaldol-promestriene vaginal tablets,1 tablet,qd(at bedtime);group C given Chlorquinaldol-promestriene vaginal tablets,1 tablet,qd(at bedtime),and Lactobacillus vaginal capsules,2 capsules,qd,next morning,implanting into vagina. 3 groups received treatment for 18 d. Clinical efficacy, vaginal health score,vaginal symptom score and vaginal pH value were observed in 3 groups,and the occurrence of ADR and re-currence rate during follow-up period (12 months) were recorded. RESULTS:The total effective rate of group C was 94.00%, which was significantly higher than group A(72.00%)and B(74.00%),with statistical significance(P<0.05). Before treatment, there was no statistical significance in vaginal health score,vaginal symptom score and vaginal pH value among 3 groups (P>0.05). After treatment,vaginal health score of 3 groups were increased significantly,while vaginal symptom score and vaginal pH value were decreased significantly;and the improvement of above indexes in group C were significantly better than in group A and B,with statistical significance(P<0.05). No obvious ADR was found in 3 groups during treatment. The recurrence rate of group C during follow-up period(4.00%)was significantly lower than that of group A(18.00%)and group B(20.00%),with statistical significance(P<0.05). CONCLUSIONS:Vaginal administration of lactobacillus combined with chlorquinaldol-promestriene for se-nile patients with atrophic vaginitis can effectively relieve clinical symptoms and signs,improve vaginal cleanliness,regulate the lo-cal pH value and reduce the risk of recurrence,with good safety.
8.Clinical value of central venous Oxygen saturation in predicting fluid responsiveness in elderly patients with septic shock

Objective To determine the predictive value of central venous oxygen saturation (ScvO2) in volume response by comparing the relationship between variation of ScvO2 and cardiac output (CO) variability before and after volume expansion in elderly patients with septic shock.Methods Forty-five patients who diagnosed septic shock were enrolled in the study,inserted CVC and were carried out continuous cardiac output monitoring(PICCO) before volume expansion.Every patient was treated by rapid intravenous infusion of 250 mL physiological saline,then take blood gas analysis before and after treated which the blood samples were collected from central venous catheter.The value of ScvO2 and the value of cardiac output was recorded.Degree of variation between two sets of data were tested by Student t-test.The correlation was determined by using Pearson's correlation test.Operating characteristics curve analysis was used to test their ability to distinguish R and NR.Results The mean arterial pressure (MAP)、heart rate(HR)、CO、ScvO2 were demonstrated and have statistical significance (P< 0.05) compared with before treated.CO and ScvO2 indexes were positively correlated (P<0.01) before and after volume expansion.ScvO2 variations after VE were significantly correlated with CO changes after VE (r=0.781,P<0.01).Conclusion ScvO2 dynamic changes can be used as a criterion for determining the fluid responsiveness.

10.Help-seeking Behavior of Depressive Patients Presented with Somatic Complaints

Objective:to study help seeking behavior of depressive patients presented as somatic complaints.Method:62 patients with depression presented with somatic complaints in general hospital and 33 presented with depressive mood in psychiatric clinics were collected. All subjects received assessment with HAMA, HAMD and a research interview.Result:depressive patients in general hospital mainly complained as poor appetite (68%), difficult in falling in sleep (66%), chest discomfort (58%), palpitation (55%), and fatigue (45%). They were different significantly to depressive patients to psychiatric clinics in level of education, help seeking behavior, illness attribution. 55 of them complained loss half of their work capacity.Conclusion:depressive patients presented as somatic complaints in general hospital consist a separate subgroup with depressive disorders.
