1.A Case of Poikilodermatomyositis.

Poikilodermatomyositis is a variant of dermatomyositis which is characterized by polymyositis associated with mottled dark brownish pigmentation, seattered atrophic patch and telangiectasia of the skin. Poikiloderma usually occurs in patient with long standing dermatomyositis. The authors observed a case of typical poikilodermatomyositis in a 40-year-old male. Skin examination revealed dark brownish pigmentation, telangiectasia and whitish atrophic patches on entire body. Muscles of shoulder girdle were markedly wasted. Muscle weakness was so severe that he could not elevate his arm above shoulder level. He also had difficulty in going up stairs. Histopathologic findings of the skin were similar to those of poikiloderma vasculare atrophicans. Light microscopic and eIectronmicroscopic examination of muscle reveaIed myositis.

2.A Case of Incontinentia Pigmenti.

A case of infantile acropustulosis(I.A.). ariginally reported by Kahn, was seen at the Department of Dermatology, Catholic Medical College in September 1979. An 11 month old male patient had been suffering from 1~2mm sized severe pruritic pustules on both hands and feet from 3 months of age. The lesions had the course of exacerbations and remissions from the onset and aggravated since last summer. The eruptions did not respond to topical and oral steroids and antiboitics. There was no family history of atopy, psoriasis and scabies. Histopathological findings revealed large, well circumscribed, intraepidermal pustules which were filled with neutrophilic polymorphonuclear leukocytes. The scraping tests for S. scabiei and dermatophytic fungi were negative. Bacterial culture from the pustular lesions was negative. Treatment was done with 4,4-diaminodiphenylsulfone(Dapsone), 2mg/kg/day in devided doses. Dramatic effect was noted within 2 days after treatment.

3.A Case of Autoinoculated Herpes Simplex.

A case of herpes simplex autoinoculated on the 1st interdigital web of right hand was reported, Herpes labialis characterized by confluent rice sized vesicular eruptions had developed on the midportion of upper lip. After 3 days, typical herpetic lesion occurred on the 1st interdigital web of right hand. We reviewed briefly the recent literatures.

4.Two Cases of Xeroderma Pigmentosum.

Xeroderma pigmentosum is a hereditary disorder characterized by the early development of pigmentary chnges, atropht, keratoses, and carcinoma, predominantly light exposed area skin.Two cases of xeroderma pigmentosum complicated by basal cell Ca. were presented. The one was 15 years old male who had been suffered from pea to walnut sized multiple tumors and ulcers on the face with scattered brownish macuIes on the eun exposed area since about 9 years old of his age. The other was 22 years old maIe who had been suffered from the same skin lesions as the former on the face, neck and dorsum of hands. In both cases skin lesions aggravated during each summer seasons and there were on history of convulsions and other neurological symptoms Histopathological findings of tumors on both cases revealed typical findings of basal cell Ca.

5.Distraction Epiphysiolysis as a Method of Limb Lengthening

We, at Department of Orthopedic Surgery, College of Medicine, Pusan National University, applied epiphyseal distraction as a method of limb lengthening, using unilateral one-plane pin fixator(ORTHOFIX) and bilateral multi-plane ring fixator(SEQUOIA) to three children with limb length discrepancies and angulation deformity due to injury of epiphyseal plate, and obtained following results. l. At first case, on whose distal femoral physis, bone bridge occupied about 20% of total epiphyseal plate and distributed mainly on the lateral side. At 10 days of distraction, epiphysiolysis occurred from the lateral side at first and angulation deformity was corrected. Epiphysiolysis on the medial side of physis appeared apparently after 60mm distraction. Bone lengthening of 4.2cm and correction of 25 valgus deformity were achieved after 72mm distraction for about 10 weeks. The percentage of increase in the initial length achieved was 11.67% and healing index was 27.86. 7 months later, loss of lengthening of 2.7cm was observed and it was considered that this was due to relatively short corticalization phase. 2. At second case, on whose distal tibial physis, bone bridge occupied about 40% of total epiphyseal plate and distributed on the medial side. At 10 days of distraction, epiphysiolysis occurred on the medial side of the epiphyseal plate, but not on the lateral side and instead of further lengthening, valgus deformity of the ankle appeared. It was thought that asymmetric distraction was attributed to the difference between the force applied on the medial side and that applied on the lateral side of the epiphyseal plate by unilateral one-plane pin fixation. 3. At third case, on whose distal tibial physis, bone bridge occupied about 75% of total epiphyseal plate. No epiphysiolysis occurred. there was a difficulty in observation of epiphysiolysis because ring fixator was overlapped with the epiphyseal plate on radiographs.
