2.Study of Calcifying Epithelioma in 9 Cases.

The diagnosis of calcifying epithelioma is more easily made on the basis of the histopathological findings than the clinical manifestations. Calcifying epidermal cyst is only one that must be differentiated from calcifying epitheIioma because of its similarity in histopathological findings. 9 cases of calcifying epithelioma which have been collected at Seoul National University Hospital for the past ten years, frorn October 1960 to September 1970, are reported. Summary of the clinical and histopathological findings are as follows; 1) Range of age: one to sixty years old. 2) Ratio of sex: nearly equal in both sexes (5 male, 4 female.) 3) The final diagnosis of all of the cases was made not on the clinical basis but on the histopathological findings. 4) Only one case out of nine showed multiple skin lesions.

3.The Effect of Room Temperature on the VDRL Test Result.

The Venereal Disease Research Laboratory microflocculation (VDRL) test, especially the quantitative test is well standardized, simple, rapid and, at present, the most widely used non-treponernal test in serodiagnosis and follow up sturly of syphilis. However, the UDRL test result could be influenced by room ternperature, especially in summer and winter in Korea in the abscence of proper air-conditioning and heating. This study was undertaken to evaluate the effect of room temperature on the VDRL test result and to determine the critical temperatures which alter the test result. Eighty six VDRL-reactive serum samples collected at the Soonchunhyang Medical College Hospital and the V.D. Clinic of the Chung-Ku Public Health Center in Seoul during the 7 months periods from September 1982 to March 1983 were used. The results obtained were as follows: The change in the VDRL titer was not obscrved at room temperature of between 18 C and 32 C 2. The VDRL titer became weakly reactive on the same titer or lowered to half titer at room temperature of below 18C. R. The VDRL titer increased two fold at room temperature of above 32C.(countinue..)

4.Is Evaluation with Voiding Cystourethrography Necessary for Children with a Febrile Urinary Tract Infection?.

PURPOSE: In the diagnosis of pediatric urinary tract infection(UTI), the use of invasive methods, such as voiding cystourethrography(VCUG), is controversial. A retrospective study was carried out to evaluate the necessity for VCUG in assessing pediatric UTI. MATERIALS AND METHODS: Between April 2000 and March 2002, 70 children (60 boys, 10 girls) with symptomatic UTI were evaluated. All patients were evaluated with renal ultrasonography(RUS), 99mTc-DMSA renal scintigraphy(DMSA renal scan) and VCUG. The results of the VCUG were compared with RUS and similar comparisons made between the results of the VCUG and DMSA renal scans. Patients with both normal a RUS and DMSA renal scan were then evaluated for the frequency and grade of vesicoureteral reflux(VUR) on the VCUG. RESULTS: The mean age at the initial evaluation was 7.95 months, ranging from 1 to 52 months. RUS and DMSA scan abnormalities were found in 38(54.3%) and 26(37.1%) children, respectively. Of the 22 patients with VUR confirmed by VCUG, 11(50.0%) had an abnormal RUS, and 12 (54.5%) patients had abnormal DMSA renal scans. Of the 19 patients with both a normal RUS and DMSA scan, 4(21.2%, 6 kidney-ureter unit) were found to have VUR on VCUG. CONCLUSIONS: This study illustrated that a considerable number of patients may have significant VUR, despite normal renal ultrasonography and DMSA scans. Therefore, VCUG remains important in assessing pediatric UTI.

5.Three Cases of Mobius Syndrome in a Family.

Mobius syndrome is generally considered to be a static disorder of congenital origin, and is manifested as unilateral or bilateral facial weakness and lateral gaze limitation. In most instances the syndrome occurs sporadically, but rarely familial cases have been reported. We report a family of three members with Mobius syndrome; a 7-year-old girl, a 6-year-old boy, and their 29-year-old mother. Each patient revealed facial diplegia, and unilateral or bilateral lateral rectus palsy. Brain MRI scans showed normal and there were no definite brainstem dysfunctions on electrophysiologic studies.

7.Cepstral Analysis of Voice in Patients With Thyroidectomy.

OBJECTIVES: The vocal changes after a thyroidectomy are temporary and nonsevere, therefore, obtaining accurate analytical results on the pathological vocal characteristics following such a procedure is difficult. For a more objective acoustic analysis, this study used the cepstral analysis method to examine changes in the patients' voices during the perioperative period regarding sustained vowel phonation. METHODS: The sustained phonation of the five vowels (i.e., /a/, /e/, /i/, /o/, and /u/) by 35 patients with thyroidectomy were recorded by using a Multi-Speech program. Of the 35 patients, 10 were men and 25 were women, with an average age of 51.5 years. Voice data were collected a total of 3 times (preoperatively, 5-7 days after the operation, and 6 weeks after the operation) and were edited according to each fragment (on-set, mid, and off-set) for cepstral analysis. RESULTS: The cepstral analysis on the patients' voices revealed no significant differences between the examination periods of all vowel phonations. However, analysis of the on-set fragment of the vowel /i/ revealed pathological characteristics in which the cepstral measurements of the voice were significantly lower after the operation than before the operation, with the cepstral measurements of the voice increasing further 6 weeks following surgery. CONCLUSION: The results of the acoustic analysis on the on-set fragment of the vowel /i/ will be important data for characterizing the vocal changes during the perioperative period. This study contributes to future research on the mechanisms underlying changes in the voice of patients with a history of thyroid or neck surgery.

9.A case of Werner Syndrome Complicated by Bone Metastasis of Rhabdomyosarcoma.

Werner syndrome (WRN), or adult progeria, is a very rare, autosomal recessive disorder characterized by the appearance of accelerated aging, including cataracts, gray hair, skin atrophy, and atherosclerosis. This syndrome is caused by mutations in the WRN gene and had a high risk of a spectrum of rare neoplasms including: i) non-epithelial malignant or pre-malignant tumors/conditions, osteosarcomas and soft tissue sarcomas, malignant melanomas, myeloid leukemia and myelodysplastic syndrome; ii) an epithelial neoplasm, thyroid carcinoma, and iii) meningiomas. Recently, authors experienced a case of Werner syndrome complicated by bone metastasis of rhabdomyosarcoma in a 20-year old Korean man. The patient revealed a painful mass on his right knee and progeroid features, short stature, scalp alopecia, abnormal dentition, craniofacial disproportion, hypothyroidsm, cataracts and osteoporosis. The onset of symptoms of Werner syndrome generally precedes any later symptoms of associated conditions, such as malignant tumor. Therefore, early recognition of Werner syndrome is important to assist identification of malignant tumors at an early stage in this patient group.
