1.Interference test in bacterial endotoxins detection of Shenmai Freeze-dried Powder Injection

Object To prove the feasibility of bacterial endotoxin test (BET) in Shenmai Freeze-dried Powder Injection by interference test. Methods Interference primary screening test and interference test were conducted in BET of Shenmai Freeze-dried Powder Injection to detect whether the interference existed or not and to explore the method of removing the interference. Results There was inhibition on BET with 0.5 EU/mL tachypleus amebocyte lysate (TAL). Interference could be excluded by using 0.25 EU/mL TAL and diluting the samples. Conclusion It is feasible to use 0.25 EU/mL TAL or TAL with more sensitivity in BET of Shenmai Freeze-dried Powder Injection.
2.Comparative study of genetic polymorphism from Nanjiang yellow goats by AFLP and RAPD

Objective To study the genetic polymorphism of Nanjiang yellow goat and to investigate the feasibility of this research by AFLP. Methods The genomic DNAs of 15 goats were analyzed with RAPD and AFLP molecule marking techniques. The effects of the two methods were compared. Results The rates of the polymorphic loci and the genetic similar index of population by RAPD marker were 26.98% and 0.942 respectively, and that by 31.4% AFLP and 0.874 respectively. Conclusion AFLP and RAPD technique are effective methods in genetic polymorphism monitoring of Nanjiang yellow goats, and supply useful parameter with appraise genetic stability.
3.Orthogonal optimization of plastic beads microwave processing of Colla Corii Asini,Colla Carapacis et Plastri Testudinis,and Colla Cornus Cervi

Objective Colla Corii Asini,Colla Carapacis et Plastri Testudinis,Colla Cornus Cervi,three types of plastic bead microwave process parameters were optimized in order to standardize the production of the types of plastic beads and to enhance the stability of the quality of plastic beads.Methods Using a number of quality indicators to evaluate the quality of plastic beads and orthogonal test to optimize the plastic bead process parameters.Results The microwave process parameters of Colla Corii Asini,Colla Carapacis et Plastri Testudinis,Colla Cornus Cervi,three types of plastic beads were obtained through orthogonal optimization.Conclusion Different types of plastic type drugs,different production lot, and different manufacturers of similar plastic herbs have their own optimum process parameters.L4(23) orthogonal optimization can be used to screen out the plastic type drugs rapidly.
4.The Effects of Nimodipine and Magnesium Sulfate on Plasma Endotheline and Serum Nitric Oxide in Patients with Severe Pregnancy Induced Hypertension

Objective To explore the therapeutic effects of nimodipine and magnesium sulfate on severe pregnancy induced hypertension (PIH) as well as the effect on plasma endotheline (ET) and serum nitric oxide (NO), in order to provide theoretical foundation for clinical treatment. Methods Forty two patients with severe PIH, being divided into 2 groups at random, were treated with nimodipine or magnesium sulfate. The changes of blood pressure, arteriole of eyeground, urine protein, ET, NO and clinical symptoms were observed respectively before and after the treatment, and the pregnancy outcomes were compared. Results (1) After the treatment of nimodipine, the serum NO of patients increased significantly from (51.72?14.64)?mol/L to (67.56?14.77)?mol/L ( P 0.05). (2) The nimodipine group took shorter time (0.5 h) to lower the blood pressure notably than the magnesium sulfate group (2 h).(3) As for the disappearing of subjective symptoms , nimodipine group (1.7? 1.3) h was quicker than magnesium sulfate group (3.4?1.7) h. Conclusions Compared with magnesium sulfate, nimodipine was better in improving and protecting the function of endotheline cells, dilating cerebral blood vessels, depressing blood pressure strongly and persistently, reducing subjective symptoms of patients and had no other side effects except increasing heart rate.

6.Cognition Investigation of Adverse Drug Reaction among Medical Staff with Different Occupations and Dif-ferent Professional Titles in Our Hospital

OBJECTIVE:To provide reference for improving the cognition of medical staff for adverse drug reactions(ADR). METHODS:A questionnaire survey was conducted on the spot among the physicians,nurses and pharmacists with different profes-sional titles in our hospital to investigate their cognition about ADR concept,their judgment for ADR and ADR reporting,and the results were analyzed by statistical methods. RESULTS:Totally 452 questionnaires were sent out,and 439 were effectively re-ceived,with effective rate of 97.12%. In terms of correct cognition rate about ADR basic concept and the main reasons,the sur-veyed pharmacists were higher than physicians and nurses,the senior professional titles of surveyed pharmacists were higher than the intermediate and junior,the pharmacists with senior professional titles were the highest and the junior were the lowest,the dif-ferences were statistically significant(P<0.05);in terms of the proportion of certain for ADR judgment,the surveyed nurses were lower than pharmacists and physicians,the surveyed physicians with senior and intermediate professional titles were higher the ju-nior,the surveyed nurses with intermediate titles were higher than the junior,the surveyed pharmacists with senior professional ti-tles were higher than the intermediate and the junior,the differences were statistically significant(P<0.05);in terms of cognition about ADR reporting,the surveyed pharmacists were the highest and the nurses were the lowest,the differences were statistically significant (P<0.05),while in terms of the proportion of knowing ADR reporting timing limit,reporting procedures and depart-ments,the surveyed pharmacists and physicians with junior professional titles were lower than the senior and intermediate,the dif-ferences were statistically significant(P<0.05);in terms of reasons for missed ADR reporting,the proportion of consideration of the risk of causing medical disputes for doctors of the surveyed physicians was higher than pharmacists and nurses,while the sur-veyed nurses showed higher proportion for doing not understand reporting procedures,doing not reporting,thinking ADR reporting was dispensable and uncertain for ADR identification,the differences were statistically significant (P<0.05). CONCLUSIONS:Medical staff with different professional titles and occupations have different cognition about ADR basic concept,judgment and re-porting,the pharmacist are relatively good,followed by the physician and nurse,especially the nurse,and the medical staff with junior professional titles are relatively poor. It is necessary to strengthen ADR education and training for them,play professional ad-vantages from the pharmacist and optimize ADR reporting procedures to improve the reporting rate and quality of ADR.
7.Application of Microwave Technology in the Preparation of Plastic Beads

OBJECTIVE:To study the advantage of microwave technology in the preparation of plastic beads. METHODS:Three kinds of representative processing methods were applied to prepare Equus asinus,Colla Cornus Cervi and Colla Carapax et Plastri Testudinis. The processing methods and the quality of plastic beads were compared. RESULTS:Study results showed mic-rowave technology was superior to other processing methods in the preparation of plastic beads and quality. CONCLUSION:The microwave technology has its specific advantages in the preparation of plastic beads.

Objective To evaluate the short-term outcomes and postoperative inflammatory cytokine changes in patients with lung cancer treated with robot-assisted thoracoscopic surgery (RATS) versus video-assisted thoracoscopic surgery (VATS). Methods A total of 270 patients with lung cancer treated by minimally invasive surgery were selected for the study, and the surgical procedures were selected according to the patients' economic conditions and preferences. Among them, 132 patients completed the operation through RATS, and 138 patients completed the operation through VATS. The clinical data of the two groups were compared. Results All patients successfully completed radical lung cancer surgery, and no perioperative deaths were reported. Intraoperative bleeding, postoperative drainage time, postoperative hospital stay, number of lymph nodes dissected, and number of lymph nodes dissected groups were more advantageous in the RATS group compared with the VATS group (

Background As one of the most common microvascular complication of diabetes in eyes,diabetic retinopathy (DR) is one of the most important cause of blindness.Nuclear factor-kappa B (NF-κB) is involved in the occurrence and development of the disease through the activation of a series of inflammatory cytokines.Objective The present study was to investigate the effects of peroxisome proliferator-activated receptor-gamma (PPAR-γ) excitomotor,rosiglitazone,on NF-κB expression and apoptosis of the retinal ganglion cells (RGCs) in the retina with diabetes mellitus. Methods Ninety SPF male Wistar rats were randomized into normal control group,diabetic control group and rosiglitazone group.Diabetes mellitus was induced by intraperitoneal injection of 50 mg/kg streptozotocin(STZ).Then 3 mg/kg rosiglitazone was intragastricly administered once per day in the rosiglitazonegroup,and the same volume of saline solution was used at the same way in the normal control group and diabetic control group from 3 days after modeling.The rats were sacrificed and the eye cups specimen was made at 4,8 and 12 weeks after usage of drugs.Retinal histopathological examination was performed by hematine-eosin staining,and expression of NF-κB p65 protein in retina and apoptotic index(AI) of RGCs were detected by immunohistochemistry and TUNEL assay,respectively in different time points mentioned above.The use of the animals complied with the Regulations for the Administration of Affairs Concerning Experimental Animals by State and Technology Commission.Results The blood glucose level was significantly elevated at various time points in the diabetic control group and rosiglitazone group compared with normal control group (P<0.01 ),and that of the rosiglitazone group was significantly declined in comparison to the diabetic control group (q =0.81,0.82,1.23,P> 0.05 ).Normal retinal structure was seen in the normal control group,and edema retinal cell and disorder of retinal layers were exhibited in the diabetic control group.Retinal structure was almost normal in the rosiglitazone group.The NF-κB p65 was expressed weakly in the retina of normal control group,but the expression of NF-κB p65 was significantly elevated in the diabetic control group and rosiglitazone group compared with the normal control group(P<0.01 ).However,the expression of NF-κB p65(A value)was significantly decreased in the rosiglitazone group compared with diabetic control group at 8 weeks and 12 weeks( q=17.77,15.30,P<0.01 ).There were a few apoptotic cells in rat retina of the normal control group.Compared with the normal control group,the AI of the diabetic control group and rosiglitazone group was significantly reduced(P<0.01 ).However,the AI of RGCs in the rosiglitazone group was significantly lower than that of diabetic control group in various time points (q =19.28,27.39,49.92,P<0.01 ). Conclusions As one of the PPAR-γexcitomotors,rosiglitazone can inhibit apoptosis of RGCs through downregulating the expression of NF-κB in rat retina with diabetes mellitus,indicating a protective effect of rosiglitazone on retina in diabetic rat.
10.Application of transanal specimen extraction via prolapsing approach in laparoscopic anterior resection for ultra-low rectal cancer

Objective To explore the application value and clinical efficacy of the transanal specimen extraction via prolapsing approach in laparoscopic anterior resection for ultra-low rectal cancer.Methods The clinical data of 27 patients with ultra-low rectal cancer who underwent transanal specimen extraction via prolapsing approach in laparoscopic anterior resection for ultra-low rectal cancer at the Shengjing Hospital of China Medical University from July 2010 to July 2013 were analyzed retrospectively.The average operation time, average volume of intraoperative blood loss, average number of lymph nodes dissection, average distance to resection margin, average length of resected specimen, results of postoperative pathological examination, time for postoperative outoff-bed activity, time to anal exsufflation, gastric tube removal time and postoperative complications were recorded.The visual analogue scale (VSA) pain score and Wexner score for evaluating fecal incontinence were performed at postoperative week 1 and at postoperative month 1, respectively.The anal function was tested at postoperative month 3 and 12.The follow-up including tumor metastasis and recurrence, Wexner score and anorectal anometry was performed by outpatient examination and telephone interview once every 3 months for 2 years after operation and then once every year up to October 2014.Measurement data with normal distribution was presented as-x ± s and average (range).Repeated measures data were analyzed by the repeated measures ANOVA.Results All the patients received successful operations without procedure change or intraoperative accident.The average operating time, average volume of intraoperative blood loss, average number of lymph nodes dissection, average distance to distal resection margin and average length of resected specimen were 140 minutes (range, 115-173 mintues), 27 mL(range, 15-55 mL), 17(range, 14-20), 1.7 cm(range, 1.3-2.2 cm) and 18.5 cm(range, 14.7-22.4 cm), respectively.Postoperative TNM stages: T2N0M0 was detected in 19 patients, T2N1M0 in 3 patients,T3N0M0 in 4 patients and T3N1M0 in 1 patients.The time for postoperative out-off-bed activity and time to anal exsufflation were 8.8 hours (range, 7.0-13.0 hours) and 51 hours (range, 30-79 hours).Twenty-seven patients had the gastric tube removal after operation with fluid diet intake at postoperative hour 24 and semi-fluid diet intake at postoperative hour 48.One male patient was complicated with urinary retention at postoperative day 3 and 1 with anastomotic leakage at postoperative day 9, they were cured by symptomatic treatment.VSA pain scores in all patients from 1 day to 6 days postoperatively were 5.6, 4.6, 4.0, 3.2, 2.2 and 1.3.The average duration of hospital stay was 11.1 days (range, 7.0-19.0 days).Patients had good healing of abdominal incision at postoperative week 2.All the patients were followed up for a average time of 24.8 months (range, 15.0-32.0 months) without tumor metastasis and recurrence.Wexner score was 2.6 (range, 1.0-4.0) at postoperative month 1.The results of anorectal anometry: maximum anorectal resting pressure (MARP) and maximum anorectal systolic pressure were (267 ±23)mmHg (1 mmHg =0.133 kPa) and (305 ± 23)mmHg before operation, (266 ± 40)mmHg and (300 ± 38)mmHg at postoperative month 3, (267 ± 33)mmHg and (315 ± 30)mmHg at postoperative month 12, respectively, with no significant difference in the changing trend between pre-and post-operation (F =0.510, 1.390, P > 0.05).Anorectal resting vector volume and anorectal systolic vector volume were (45 594 ± 1 981) cm (mmHg) 2 and (98 480 ± 8 165) cm (mmHg) 2 before operation, (40 310 ±3 465)cm(mmHg)2 and (78 461 ±6 777)cm(mmHg)2 at postoperative month 3, (40 385 ± 3 379) cm(mmHg) 2 and (82 082 ± 10 383) cm(mmHg) 2 at postoperative month 12, respectively, with significant differences in the changing trend between pre-and post-operation (F =26.845, 48.090, P < 0.05).Conclusion Transanal specimen extraction via prolapsing approach in laparoscopic anterior resection for ultra-low rectal cancer is safe, aesthetic and minimally invasive compared with the traditional laparoscopic surgery, with the advantages of radical cure of tumor and protection of anal function.
