4.The Concentrations of Ascorbic Acid in Vitreous and Subretinal Fluid in Retinal Detachment Patients.

Ascorbic acid in aqueous humor, the concentration is 15-69 times higher than in plasma, protects ocular tissues from free radicals as an antioxidant and absorbs ultraviolet rays. It is regarded that the concentration of ascorbic acid in the vitreous body is greater than in the plasma, but there is no knowledge about it's role and relationships to retinal disease. We measured the concentrations of ascorbic acid in plasma, aqueous humor, vitreous and subretinal fluid in 10 patients with rhegmatogenous retinal detachment who were treated with scleral encircling by high performance liquid chromatography. The average concentrations of ascorbic acid in plasma, anterior chamber aqueous humor, vitreous, and subretinal fluid were 30.8 +/- 7.5 micro gram/ml, 157.4 +/- 2.5 micro gram/ml, 195.3 +/- 7.5 micro gram/ml, and 178.5 +/- 2.5 micro gram/ml respectively. The ascorbic acid concentrations in intraocular fluids were 5-6 times higher than in plasma.

5.The Effects and Complications of Mitomycin-C for Prevention of Recurrence after Pterygium Operation.

The most common problem in pterygium operation is the recurrence. Therefore radiation therapy or instillation of mitomycin-C after pterygium operation have used to prevent the recurrence of pterygium. We evaluated recurrence rate and complication rate with mitomycin-C and without mitomycin-C after the operation of pterygium in 113 eyes. The 112 patients(113 eyes) underwent simple surgical excision, whose mean age was 50.7 tears, and whose follow-up period was 6 to 12 months. 0.04% solution of mitomycin-C was instilled to 61 eyes and not to 52 eyes. Recurrence was noticed in 12 out of 61 eyes(19.6%) among the group not using mitomycin-D and in 2 out of 52 eyes(3.8%) among the group using mitomycin-C after pterygium operation. One eye in patients using mitomycin-C showed severe corneal edema due to endothelial decompensation by the toxicity of mitomycin-C. It is concluded that mitomycin-C ws effective to prevent recurrence after pterygium operation, but we should be aware of the severe complications by the toxicity of mitomycin-C.

6.A Case of Neonatal Alloimmune Thrombodytopenia Due to Anti - HLA Antibody.

Anti-HLA antibody related neonatal thrombocytopenia is an uncommon disorder caused by platelet antigen incompatibility between mother and fetus in Korea. Mothers who lack the specific platelet antigen produce the IgG against the platelet antigen which the fetus inherits from the father. These IgG antibodies are then transported across the placenta into the fetal circulation where they lead to the destruction of fetal platelets. We report a case of neonatal alloimmune thrombocytopenia related with anti-HLA antibody in second baby of dizygotic twin who had petechia on trunk and platelet count 43,000/mm. Initially, mother and twin showed the positives in antiplatelet antibodies. In microlym- phocytotoxic test at 6 months of age, anti-HLA antibodies was negative in twins but anti-HLA A2, A24 was positive in their mother. The patient was treated with intravenous immunoglobulin and clinically improved and her platelet count was norrnalized.

9.Ultrastructure of the Experimentally Induced Cataract in Porcine Crystalline Lens.

One of the purposes of this experiment is to observe the structure of crystalline lens with cataract, which is formed artificially, using the light microscope and electron microscope. The other is to observe the differences of structural variations in the cataract developed inside body. Twelve eyes of six pigs were used for this experiment. Two of them are normal crystalline lens, five are cystalline lens in distilled water, and the rest are in balanced salt solution through intact or ruptured capsule. We examined the time of cataract formation and compared the ultrastructural changes. Ruptured capsule and high osmotic pressure difference induced more rapid opacity. In case of distilled water, the capsule is maintained but it is very difficult to distinguish between epithelium and lens fibers. Also, there is a severe crack in the lens fibers. In electron microscopic, as the cataract progresses, the osmotic swelling becomes more prominent. One of the striking changes was a marked intercellular cyst formation. Lens cells became irregular in size and density and were extensively vacuolated. The swelling of the lens cells continuously induced large intracellular vacuoles and liquefied the cytoplasmic protein. In conclusion, the structural change of cataract, which was seen by an electron microscope, resulted from change in osmolarity from previously announced in vivo experiment and structural change resulted from this experiment are similar.

Cerebral gigantism(Sotos syndrome) is a growth disorder that consists of large size at birth, rapid early growth rate with accompanying advanced bone age, acromegalic features, and developmental delay. Clumsiness in the absence of other abnormal neurologic findings is common. The cause is unknown. We report here a case of 238/12-year-old Sotos syndrome with final adult height above 97 percentile, abnormal brain MRI findings(large ventricles, prominent trigone, prominent occipital horn & thining of corpus callosum), clumsiness, and some behavioral problems.
