1.The Effect of CO2 Insufflation and Trendelenburg-lithotomy Position on Intraocular Pressure during Laparoscopy.

BACKGROUND: The laparoscopy requires carbon dioxide (CO2) insufflation and Trendelenburg position for operational convenience. However, the above circumstances affect the cardiopulmonary systems significantly and intraocular pressure (IOP) may be also influenced. METHODS: In 27 non-glaucoma patients right and left intraocular pressure (RIOP, LIOP) were measured 5 minutes after induction of general anesthesia (control value), 15 and 30 minutes after CO2 insufflation and endelenburg-lithotomy position. RESULTS: The control values of RIOP and LIOP were 11.3 4.7 mmHg and 11.5 4.7 mmHg respectively. At 15 minutes after CO2 insufflation and Trendelenburg-lithotomy position, they increased to 16.6 5.3 mmHg and 17.0 5.9 mmHg (p<0.05). At 30 minutes, 18.4 3.5 mmHg and 18.2 4.1 mmHg (p<0.05). CONCLUSION: CO2 insufflation and Trendelenburg-lithotomy position increase IOP significantly in non-glaucoma patients during laparoscopy.

3.Effect of Resveratrol on Oral Cancer Cell Invasion Induced by Lysophosphatidic Acid.

The aim of the current study was to demonstrate the potential therapeutic efficacy of resveratrol in oral cancer patients. Lysophosphatidic acid (LPA) intensifies cancer cell invasion and metastasis, whereas resveratrol, a natural polyphenolic compound, possesses antitumor activity, suppressing cell proliferation and progression in various cancer cell lines (ovarian, gastric, oral, pancreatic, colon, and prostate cancer cells). In addition, resveratrol has been identified as an inhibitor of LPA-induced proteolytic enzyme expression and ovarian cancer invasion. Furthermore, resveratrol was shown to inhibit oral cancer cell invasion by downregulating hypoxia-inducible factor 1α and vascular endothelial growth factor expression. Recently, we demonstrated that LPA is important for the expression of transcription factors TWIST and SLUG during epithelial-mesenchymal transition (EMT) in oral squamous carcinoma cells. In this study, we treated serum-starved cultures of oral squamous carcinoma cell line YD-10B with resveratrol for 24 hours prior to stimulation with LPA. To identify an optimal resveratrol concentration that does not induce apoptosis in oral squamous carcinoma cells, we determined the toxicity of resveratrol in YD-10B cells by assessing their viability using the MTT assay. Another assay was performed using Matrigel-coated cell culture inserts to detect oral cancer cell invasion activity. Immunoblotting was applied for analyzing protein expression of SLUG, TWIST1, E-cadherin, and GAPDH. We demonstrated that resveratrol efficiently inhibited LPA-induced oral cancer cell EMT and invasion by downregulating SLUG and TWIST1 expression. Therefore, resveratrol may potentially reduce oral squamous carcinoma cell invasion and metastasis in oral cancer patients, improving their survival outcomes. In summary, we identified new targets for the development of therapies against oral cancer progression and characterized the therapeutic potential of resveratrol for the treatment of oral cancer patients.

5.Susceptibility of cockroaches (Blattella germanica Linneaus) to various insecticides.

During the period from May to October 1969, the authors carried out a series of tests with the aim to ascertain the present status of susceptibility of adult cockraoches to various insecticides. The tests on the susceptibility of cockroaches to dieldrin, D.D.V.P., Diazinon; Dibrom, Malathion and lindane were performed with the topical application and the residual film methods under the conditions of 26 degrees C and 80% of relative humidity. The insects tested were collected from Seoul City and were reared at the laboratory. Some of the important results so far obtained can be summarized as follows: The median lethal doses (LD(50)) of the five insecticides against female adult cockroaches were: 0.4 ug/roach or 4.02 ug/body weight (gm) by Dieldrin, 0.29/roach or 2.88 ug/body weight (gm) by D.D.V.P., 0.29 ug/roach or 2.96 ug/body weight (gm) by Diazinon, 0.54 ug/roach or 5.37 ug/body weight (gm) by Dibrom, and 2.96 ug/roach or 29.60 ug/body weight (gm) by Malathion. And those against male adult cockroaches were: 0.18 ug/roach or 3.29 ug/body weight (gm) by Dieldrin, 0.27 ug/roach or 4.89 ug/body weight (gm) by D.D.V.P., 0.16 ug/roach or 2.91 ug/body weight (gm) by Diazinon, 0.49 ug/roach or 8.97 ug/body weight (gm) by Dibrom, and 1.32 ug/roach or 24.19 ug/body weight (gm) by Malathion. The level of susceptibility of adult cockroaches to the insecticides tested was 1.0-1.6 times higher as compared with the results obtained on the Japanese Takashi strain. The median knockdown times (LT(50)) of the female adult cockroaches by insectcides at 1% concentration of 3 mg per square centimetre were 1.82 hours by Malathion, 1.36 hours by Diazinon, 0.56 hours by Lindane, 0.50 hours by Dibrom, and 2.56 hours by D.D.V.P. A marked difference was seen to exist between the males and the females. The female cockroaches were more resistant than the male to Dieldrin, Diazinon and Malathion.

6.Influence of modification in core building procedure on fracture strength and failure patterns of premolars restored with fiber post and composite core.

PURPOSE: The influence of the modified process in the fiber-reinforced post and resin core foundation treatment on the fracture resistance and failure pattern of premolar was tested in this study. MATERIALS AND METHODS: Thirty-six human mandibular premolars were divided into 4 groups (n = 9). In group DCT, the quartz fibre post (D.T. Light-post) was cemented with resin cement (DUO-LINK) and a core foundation was formed with composite resin (LIGHT-CORE). In group DMO and DMT, resin cement (DUO-LINK) was used for post (D.T. Light-post) cementation and core foundation; in group DMO, these procedures were performed simultaneously in one step, while DMT group was accomplished in separated two steps. In group LCT, the glass fiber post (LuxaPost) cementation and core foundation was accomplished with composite resin (LuxaCore-Dual) in separated procedures. Tooth were prepared with 2 mm ferrule and restored with nickel-chromium crowns. A static loading test was carried out and loads were applied to the buccal surface of the buccal cusp at a 45 degree inclination to the long axis of the tooth until failure occurred. The data were analyzed with MANOVA (alpha = .05). The failure pattern was observed and classified as either favorable (allowing repair) or unfavorable (not allowing repair). RESULTS: The mean fracture strength was highest in group DCT followed in descending order by groups DMO, DMT, and LCT. However, there were no significant differences in fracture strength between the groups. A higher prevalence of favorable fractures was detected in group DMT but there were no significant differences between the groups. CONCLUSION: The change of post or core foundation method does not appear to influence the fracture strength and failure patterns.

8.Replantation of Hand Amputated Through the Palmar Arch Level

The value of replantation of parts of the upper limb is well established and replantation of a digit has now be come a standard procedure. However, there are few reports of replantation of hand amputated through the palmar arch level. We have reviewed 14 cases of 14 patients replantation of hand amputated through the arterial palmar arch level from Mar. 1976 through Mar. 1982. The success rate in our series is 64 percent. This is slightly lower than the other levels due to its anatomical complexity. According to the anatomical complexity and distribution of nerves and vessels, it is divided to three levels. In level A, the superficial palmar arch and/or the deep palmar arch should be repaired. In level B, the princeps pollicis artery should be repaired after anastomosis of the superficial and deep palmar arch. In level C, each common palmar digital artery should be repaired but if it is impossible to repair of all fingers, replantation of thumb and index should be performed as possible. Meticulous microsurgical technique and precise anatomical knowledge is mandatory for replantation of the palmar arch level.

9.The Depressive Effects of Osteoarthritis Symptoms in the Knee Joint Patients.

BACKGROUND: Osteoarthritis(OA) of the knee represents a common chronic disease, especially in the elderly, leading to functional impairment and disability. And OA patients have depressive symptoms frequently. Several factors contribute to the level of depressive symptoms. Authors tried to assess these factors and show effect of these factors to depressive symptoms. Through this study in the future physicians must consider psychiatric problems besides the pain itself in the treatment of OA patients. METHODS: Subjects included in this study were 64 knee joint OA patients. We obtained depressive symptoms from the CES-D(The Korean version of the Center for Epidemiological Studies-Depression Scale), level of pain, stiffness, functional disability from KWOMAC(WOMAC Korean Version 2, The Korean version of Western Ontario and Macmaster Osteoarthritis Index Score) and other factors able to contribute to depressive symptoms. RESULT: We divided the patients into two groups using CES-D cut-off score of 21. Each group consists of 46(CES-D<21) and 18(CES-D> or =21) patients. Frequency of sleep disturbance and pain consistency and level of pain, stiffness and functional disability were different significantly between two groups(p<0.0l). Through the correlation analysis level of pain, stiffness and functional disability were associated with CES-D score significantly(p<0.0l). And through the stepwise multiple regression analysis we demonstratred that the CES-D score influenced by level of functional disability, BMI, disease duration and, pain but only level of functional disability and BML were statistically significant(p<0.0l). Level of functional disability explained 46% of CES-D score(partial R2=0.46) so influenced on depressive symptoms most effectively. CONCLUSION: This study suggested that functional disability was most important factors to explain depressive symptoms of knee joint OA patients.

10.Propranolol Hcl in Patients with Angina Pectoris.

A double-blind trial of the efficacy of propranolol hydrochloride a new beta adrenergic blocking agent, in the prophylactic treatment of angina pectoris was conducted in 9 patients. Through the trial period of 8 weeks to more than 26 weeks, the patients received propranolol and placebo for equal period. Following are the results. 1. The effective dose of propranolol hydrochloride in the prophylactic treatment of angina pectoris was 80 mg to 120 mg per day. 2. The effectiveness of propranolol hydrochloride was manifested by a reduction in frequency of pain to 40.7%, in nitroglycerin comsumption to 38.8% and by an increase in exercise tolerance. 3. Obseved side effects were transient nausea, vomiting and bradycardia, which were improved with reduction in drug dosage. 4. Further clinical and experimental information may be necessary in order to assess the long-term effects of propranolol hydrochloride.
