1.A Case of Choroidal Malignant Melanoma.

The author has experienced a case of malignant melanoma of the choroid in a 57-years old Korean male. The patient visited our hospital's emergency room in August 1981, with severe right ocular paiq and headache. The eyeball was slightly protruded and enlarged. And the cornea wea edematous. The vision of right eye was complete blindness. The intraocular tumor was suspected by patient's history and ocular examinations. So enucleation was performed and histopathologically the tumor was confirmed to choroidal malignant melanoma composed of spindle cells and epitheloid cells. And the metastasis to lung was suspected on chest X-ray.

6.Screening of Urine Culture Specimens by Gram Stain, Urinalysis and Urine Microscopic Examinations.

BACKGROUND: The purpose of this study was to discover ways to screen urine culture specimens through Gram stains, urine stick analyses and microscopic examinations for the laboratory cost saving. METHODS: One hundred and fifty-eight urine specimens for culture were included. Fifty uL of urine were inoculated onto one well each of 10-well slide, dried on the hot plate, and Gram-stained. The results combined with routine urinalyses including urine nitrite and leukocyte esterase, and pyuria, were compared with the routine culture results. RESULTS: The screening of bacteriuria by Gram stains, urinalyses and microscopic examinations revealed the high sensitivity (91.9%) and negative predictive value(95.5%) with cost saving of 41.8% of inoculating media. Not considering the Gram stains, the screening revealed 83.8% sensitivity and 92.5% negative predictive value, even if the cost saving of inoculating media were as high as 50.1%. CONCLUSION: It was demonstrated that it was sensitive and economic and produced rapid preliminary results to screen bacteriuria by the Gram stains combined with urinalyses and microscopic examinations.

Malignant fibrous histiocytoma of the bone is relatively rare than that affecting the soft tissue. The authors have first experienced a case of malignant fibrous histiocytoma affecting the distal tibia of thirty-two years old woman in Korea. We obtained good results from free vascularized fibular graft after radical resection in stead of amputation. A case of malignant fibrous histiocytoma of the bone is reported with brief review of literature.

9.Replantation of Hand Amputated Through the Palmar Arch Level

The value of replantation of parts of the upper limb is well established and replantation of a digit has now be come a standard procedure. However, there are few reports of replantation of hand amputated through the palmar arch level. We have reviewed 14 cases of 14 patients replantation of hand amputated through the arterial palmar arch level from Mar. 1976 through Mar. 1982. The success rate in our series is 64 percent. This is slightly lower than the other levels due to its anatomical complexity. According to the anatomical complexity and distribution of nerves and vessels, it is divided to three levels. In level A, the superficial palmar arch and/or the deep palmar arch should be repaired. In level B, the princeps pollicis artery should be repaired after anastomosis of the superficial and deep palmar arch. In level C, each common palmar digital artery should be repaired but if it is impossible to repair of all fingers, replantation of thumb and index should be performed as possible. Meticulous microsurgical technique and precise anatomical knowledge is mandatory for replantation of the palmar arch level.

10.An Analysis of 300 of Arthroscopies of Knee Joint

Arthroscopy of the knee joint is an essential diagnostic procedure of the knee joint lesions because of its high diagnostic accuracy and simple procedure and high safety. We analysed in 300 cases of knee joints arthroscopy which performed in Kyung Hee University Hospital from July 1978 to June 1982. The results were as follows: 1. The diagnostic accuracy of clinical diagnosis of knee joint lesions based on arthroscopic findings was 51.0%. 2. The diagnostic accuracy of clinical diagnosis in surgically confirmed cases of knee joint lesions was 56.1% and diagnostic accuracy of arthroscopy was 90.9%. 3. In meniscal lesions, the diagnostic accuracy of arthroscopic diagnosis was 94.5%, arthrographic diagnostic accuracy was 84.9%, clinical diagnostic accuracy was 48.9% and combined accuracy of three diagnostic methods was 97.3%. 4. In cases which diagnosed as I.D.K. (Internal Derangement of Knee) in initial stage of knee joint lesions, meniscal lesions (32.8%) and ligament injuries (22.4%) were predominant lessions based on arthroscopic findings. 5. The frequency of meniscal tear were no significant difference in lateral (33.3%) or medial (36.0%) meniscus except discoid meniscus rupture. 6. 68 Cases of 300 arthroscopies (22.7%) were avoided unncessary arthrotomy through preoperative arthroscopic examination of knee joint. 7. Arthroscopy is very useful and essential not only to confirm the diagnosis but also to treatment of knee joint lesions.
