Transplants

Although the primary repair has been recognized so far, the tendon graft is reported as a primary treatment method for tendon injury in no man's land of Bunnel, judging from the state of injured hand, the ability of operator and the environment of medical operation. The 27 cases of tendon graft that were operated for 11 years from June, 1973 to June, 1984 in the Department of Orthopedic Surgery, Hanyang University College of Medicine have been followed up for clinical study. The results were as follows: 1. The Material of this study was composed of 13 males (21 cases) and 5 females (6 cases) whose age were from 8 to 39 years old. 2. The injured fingers were classified with 3 thumbs, 6 index fingers, 7 middle fingers, 8 ring fingers and and 3 little fingers out of 27 cases and all injured parts of fingers were Zone II except that of thumb which was Zone IV of Bunnel. 6 patients were injured more than 2 fingers. 3. The duration from tendon injury to tendon graft which scattered from 1 month to 9 years and averaged out to 28.2 months is supposed to have little relation to post-operative results. 4. The post-operative results showed up 9 excellent, 7 good and 1 fair out of 17 cases in good gmup, 3 good and 6 fair out of 9 cases in cicatrix group, 1 fair out of 1 case in nerve damage group at pre-operative state. 5. The post-operative results of thumb which show up 2 excellent, 1 good out of 3 cases were most successful among those of all fingers. 6. The pre-operative state is supposed to have great relation to post-operative state judging from 94% satisfactory results of good group at pre-operative state. 7. As multiple injury makes little difference from single injury, the injured state of each hand is supposed to an important effect on post-operative results. 8. As all electric bumed injury show up fair except that of thumb, they are supposed to have unsatisfactory results.

Colonna capsular arthroplasty is a two-stage procedure for congenital dislocation of the hip, but is not suitable for congenital subluxation. The bone architecture of the femoral head should approach the nearly normal and the child preferably should be more than three years old and less than eight if the best functional result is to be obtained. These brief case reports of five patients who could be followed for from one year and one month to more than seventeen years document the fact that the procedure may produce a hip with an excellent roentgenographic appearance and enduring satisfactory function.

CD99 plays an critical role in the diapedesis of monocytes, T cell differentiation, and the transport of MHC molecules. Engagement of CD99 by agonistic monoclonal antibodies has been reported to trigger multifactorial events including T cell activation as well as cell-cell adhesion during hematopoietic cell differentiation. In this study, to identify the functional domains participating in the cellular events, we mapped the epitopes of CD99, which are recognized by two agonistic CD99 monoclonal antibodies, DN16 and YG32. Using recombinant fusion proteins of GST with whole or parts of CD99, we found that both antibodies interact with CD99 molecules independently of sugar moieties. DN16 mAb detected a linear epitope located in the amino terminal region of CD99 while YG32 mAb bound another linear epitope in the center of the extracellular domain. To confirm that the identified epitopes of CD99 are actually recognized by the two mAbs, we showed the presence of physical interaction between the mAbs and the fusion proteins or synthetic peptides containing the corresponding epitopes using surface plasmon resonance analyses. The dissociation constants of DN16 and YG32 mAbs for the antigen were calculated as 1.27 X 10(-7) and 7.08 X 10(-9) M, respectively. These studies will help understand the functional domains and the subsequent signaling mechanism of CD99.

BACKGROUND AND PURPOSE: Thrombolytics-induced recanalization fails in a significant portion of patients with ischemic stroke, which is partly due to the resistance of clots to lysis by thrombolytic agents. The pretreatment level of endogenous fibrinolysis inhibitors may affect such thrombolysis failure. METHODS: We studied 43 stroke patients whose arterial recanalization had been evaluated by angiography, and whose blood had been obtained prior to the administration of thrombolytic agents. Plasma samples from 34 healthy volunteers were used as normal controls. Plasminogen activator inhibitor type 1 (PAI-1) and thrombin-activatable fibrinolysis inhibitor (TAFI) levels were quantified using an enzyme-linked immunosorbent assay. RESULTS: Arteries were recanalized [Thrombolysis in Myocardial Infarction (TIMI) grade 2 or 3] in 30 patients, but not (TIMI grade 0 or 1) in the other 13. The plasma PAI-1 level was significantly higher in patients without recanalization (nonrecanalization) than in those with recanalization and in normal controls. The TAFI levels did not differ among the groups. CONCLUSIONS: The pretreatment PAI-1 levels are increased in acute stroke patients with thrombolysis failure.

Infiltrating lipoma is a rare mesenchymal neoplasm that, in spite of benign nature, characteristically infiltrates adjacent tissues and tends to recur after surgery. It has a predilection for the extremities and the trunk and is extremely rare in the head and neck region. We present a case of congenital infiltrating lipoma of the face, describing the intrabony invasion and osseous dystrophy as well as the soft tissue changes seen on plain radiographs and magnetic resonance imaging.

BACKGROUND: Intra-arterial cerebral angiography, which is a prerequisite for carotid endarterectomy and angioplasty, carries some risks but provides the best visualization of the cerebral vasculatures. We attempted to examine the inci-dence of complications associated with cerebral angiography in patients with ischemic stroke. METHODS: We retrospec-tively reviewed the medical records of patients with ischemic stroke or transient ischemic attack (TIA) who underwent the digital subtraction cerebral angiography. Four hundred nineteen procedures were performed between October 1994 and August 1999. The systemic, local, and neurologic complications were evaluated. The neurologic complications were defined as occurrences of any new focal neurologic deficits or progressions of the preexisting neurologic deficits during or within 24 hours after the procedure. RESULTS: There were 5 systemic (1.2%), 17 local (4.1%), and 10 neuro-logic (2.4%) complications. The neurologic complications were reversible within 7 days in 6 (1.4%) and were persistent after 7 days in 4 (1.0%). Six out of 10 patients with neurologic complications had previous stroke or TIA. The angio-graphic studies revealed the stenosis or obstruction of the relevant arteries in 7 patients. CONCLUSIONS: Cerebral angiog-raphy in patients with ischemic stroke was associated with 1.4 % reversible and 1.0% persistent neurologic complica-tions, all of which developed after the angiographic procedure. The history of previous stroke or TIA and the presence of severe stenosis or occlusion of the symptomatic arteries may carry a high risk of neurologic complications.
