3.How Aware Elderly Subjects are of Medical Device Clinical Trials and Their Adverse Events - A Survery.

BACKGROUND: Along with the recent greater number of medical devices for the elderly in the market, there has been a parallel increase in the number of clinical trials for these devices. It is uncertain, though, whether the target population has the awareness and understanding of these clinical trials and the possible adverse events of the devices. METHODS: A total of 147 elderly subjects (mean age, 65.8+/-8.3 years; 39 males) responded to the questionnaire. A pilot study during the development of the questionnaire was conducted in 2009 with the actual survey taking place from June to November 2011. The questionnaire included 22 items-10 items on awareness and understanding of the clinical trial; 4 items on the understanding of adverse events; and 8 items on demographical characteristics. RESULTS: Of the responders, 62.5% had participated in at least one clinical trial and had heard about the clinical trials through a doctor, newspaper, or television. And 63.3% of the responders understood that the purpose of the trial was to show efficacy and safety for the medical device. There were significant differences on the awareness of the trial by gender and life level. However, there was no statistically significant difference in adverse events awareness. CONCLUSION: Although elderly subjects participate in clinical trials, it seems they do not receive enough information when involved in clinical trials of medical devices. In the future, before these trials are carried out, medical device companies need to ensure that they provide their elderly subjects with better education and information.

4.Skin Metastasis of Adenoid Cystic Carcinoma of Parotid Gland.

Adenoid cystic carcinoma is known to be a neoplasm of the major and minor salivary glands but it also occurs in the lacrimal gland, external auditory canal, easphagus, breast and skin. Adenoid cystic carcinoma of salivary gland is a slow-growin nalignant tumor. Although metastasis of this tumor clevelnps in about half the cases, its meta taes to the overlying skin is not common, moreover the skin metastasis to the remote site is raely found. Primary cutaneous adenoid cystic carcinoma is rare and histologically indistingisl able from skin metastasis of adenoid cystic carcinoma of the salivary gland. A 53 year-old man presented a solitary, well defined, erythen at us, 1.3 x 1.3cm sized nodule on the vertex of the scalp, which revealed the typical histopatholog ndings of adenoid cystic carcinoma. One and half years ago, a tumor of the parotid gland va removed and proved to be a cribriform pattern of adenoid cystic carcinoma. The skin lesion was diaghosed as a skin metastasis of the alenoid cystic carcinoma of parotid gland rather than a primary cutaneous adenoid cystic carcinona because a similar pattern of adenoid cystic carcinorom had been discovered on the parotidg an l although it developed on the scalp, the usual site of primary cutaneous adenoid cystic carcinama.

5.A Case of Multiple Intravascular Papillary Endothelial Hyperplasia.

Intravascular papillary endotholial hyperplasia is a relatively rare (istose which is characterized by the development of endothelialdined papillary projections in a vascula humen. It is commonly found to be associated with thromboticrraterial and is now considered a reative process of the endothelium rather than a neoplastic one. Although this disease usually presents petri dish or bluish nodule on the head, neck or upper extremiti it can develop anywhere on the bod, . There are many reports about lesions in unusal locations, but t.hey are almost always solitary. A 61-year-old woman complained of multiple, tender, 1 x 1 to 4 x 5 creasized nodules on both hands, antecubital fossa, chest, abdomen, left shoulder and right calf. These the nodules showed reddish to bluish colors and had long hisbiries from 1 year to 7 years. Four our 13 lesions were excised for histopathologic diagnosis intravascular papillary endothelial hyperplaian 3 lesions and cavernous hemangioma in 1 lesion.(

7.Evaluation of the Contaminant Bacteria Isolated from Humidifiers Used in the Intensive Care Unit.

BACKGROUND: This study aims to obtain basic data regarding the proper maintenance and potential role of humidifiers in the development of nosocomial pneumonia. We investigated into a spectrum of bacteria that grew after disinfection in humidifiers that were used in intensive care units (ICUs). The bacteria were isolated at different time points after disinfection. METHODS: We randomly selected 6 humidifiers that were used in an ICU and conducted a series of bacterial culture studies at 24 hr, 48 hr, 72 hr and 168 hr after disinfecting the six humidifiers. The fluid from the humidifider was cultured in the same manner as the dialysate. RESULTS: No bacteria were isolated in most of the 6 humidifiers 24 hr after the disinfection of the instruments. Gram-negative bacilli such as Acinetobacter baumannii, Burkholderia cepacia, and Stenotrophomonas maltophilia were cultured at 48 hr after disinfection. CONCLUSION: Regular and thorough disinfection of humidifiers is necessary to prevent possible spread of infection via humidifiers in the ICU.

9.Tratment of Pathological Fractures in Metastatic Bone Tumors

Better palliative management of patients with disseminated cancer has prolonged their lives but has raised the incidence of pathologic fractures in their variable lives. The essence of treatment is to prolong life, maintain comfort, and stabilize the patient with metastatic disease. The treatment of pathologic fractures with internal fixation has long been accepted by orthopedic surgeons and the benefits afforded to the patient are well known, so adequate fixation is essential. A combination of methylmethacrylate and internal fixation devices can greatly enhance the stability of fixation, because diseased bone is a poor base on which to insert an internal fixation device. The authors have treated 19 cases of the pathologic fracture of spine, humerus and femur by use of bone cement with and without internal fixation devices. The results are as follwed. 1. All cases utilized methylmethacrylate and there was no complication in methylmethacrylate in itself. 2. All cases had received various type of ancillary treatment: radiation in 9 cases, chemotherapy in 4 cases and in 8 cases, conservative pain control. 3. In spinal pathologic fracture with paraplegia or neurologic deficiency, the patients could walk with walker at an average of two weeks after operation. 4. Among the 19 cases, the pain was relieved markedly and ambulation was possible easily and early after operation in 16 cases. 5. Among the 19 cases, two survived less than 2 months, 7 surrived from 2 month to 3 month, five survived from 3 months to 6 months, four survived from 6 months to a year and one survived more than a year.
