3.Clinical Effects of Venitol(R) on Complications after Hemorrhoidectomy Prospective Randomized and Placebo-controlled Trial.

Fifty four patients who received closed hemorrhoidectomy were randomized into two parallel groups and treated with Venitol(R)(a micronized flavonoidic fraction containing diosmin 450 mg and hesperidin 50 mg) (group 1) or placebo (group 2). Venitol(R) was administered at the dosage of three toblets b.i.d. the first four days and two tablets b.i.d following three days. Postoperative analgesia and laxative prescription as well as hospital stay were same in two groups. Though there is no difference of symptoms at D1, improvement of symptoms of complications was greater in group 1 than in group 2 at D18. The clinical severity of postoperative spot-bleeding, pain and anal discharge diminished in both groups but to a greater extent in group 1 (P<0.005). There was no side-effects in using Venitol(R). In summary, Venitol(R) is effective in reducing complications after hemorrhoidectomy.

5.The Effect of Zinc Sulfate in Acnd Vulgaris.

In order to evaluate the effect of zinc sulfate on acne vulgaris, this study was performed during 3 months' period from October, 1979 to December 31, 1979. Total number of acne patients visited at the Department of Dermatology, Han Gang Sung Shim Hospital were 130, but among them 58 patients had completed this atudy. The patients were classified in 3 groups, A, B, C, as follows. Group A (23 patients) who were treated with zinc sulfate (600mg/day) and 2. 5% benzoyl peroxide Group B (20 patients) who were treated with tetracycline (initial: 750mg/day, after 2weeks: 500rng/day) and 2. 5% benzoyl peroxide. Group C (15 patients) who were treated with only 2. 5% benzoyl peroxide. To evaluate the efficacy of treatment in these 3 groups, the number of open comedone, closed comedone, papule and pustule were counted every other week for 8 weeks. (countiuned..)

7.A Case of Penile Cutaneous Horn.

Cutaneous horn is a clinical term for a circumscribed concial hyperkeratotic mass amounts to at least half of its largest diameter. The number of lesion may be single or multiple. We report herein a cases of penile cutaneous horn in a 22-year-old male. He had dark brownish colored conical shaped protruded mass on the penile shaft for about 2 months. We performed excisional biopsy and curettage. Histopathologic findings showed verruca vulgaris.

9.Percutaneous biliary drainage

Percutaneous biliary drainage is an effective new nonsurgical method for combined external/internal catheterdecompression of obstructed extrahepatic bile ducts. Molnar and Stockum, and more recently others, have reportedsucdess utilizing this approach to establish biliary drainage in patients with obstructive jaundice. Percutaneousbiliary drainage was performed in 9 patients with obstructive jaundice. Internal drainage was accomplished in 4patients (44.4%), and external drainage in 5 patients (55.6%). Major complications occurred in 2 patients; bileleakage, one; sepsis, one. Minor complications occurred in 7 patients; mild or high fever, 5 cases; transienthemobilia, 2 cases. The catheter management problems encoutered including pain, catheter dislodgement ormigration, lumen occulusion, and external bile leakage.

10.Clinical Study of Post - tetanic Count ( PTC ) during Vecuronium - induced Intense Neuromuscular Blockade.

Vecuronium induced intense neuromuscular blockade was evaluated in 40 USA class l, ll adult patients using the post tetanic count (PTC) and train-of-four(TOF) methods. All patients were anesthetized with thiopental sodium, nitrous oxide(50%), and enflurane(1~2%). Neuromuscular monitoring commended immediately after administration of thiopental sodium. The ulnar nerve was stimulated using surface electrodes and the mechanical response of the adductor policis muscle recorded using the Acclograph, neuromuscular transmission monitor. TOF nerve stimulation was used every 15s. After supramaximal stimulation was achieved a bolus of vecuronium 0.1mg/kg was injected intravenously. The traches was intubated when TOF response was depressed to 95% or above to control twitch height and the lungs ventilated with a tidal volume of 10ml/kg and a rate of 14 b.p.m. The patients were allocated randomly to two groups of 20 patients each. Patients in control group were allowed to recover TOF response spontaneously, and in PTC group were applied tetanic stimulation(50Hz for 5s) at 7 minutes intervals during no TOF response. In PTC group, the tetanic stimulation was preceded by a 30s period of 1Hz stimulation on each occasion, which was continued after the 3s pause. Duration of no TOF response in each group, and relationship between first post tetanic twitch height of PTC and time to first reaction to TOF nerve stimulation in PTC group were measured. In the results, duration of no TOF response was shortened by tetanic stimulation(p<0.01). There was a close inverse correlation between first post tetanic twitch height or PTC and time to first reaction to TOF nerve stimulation (p<0.01). It was concluded that PTC method appeared to be a valuable supplement to TOF nerve stimulation in assessment and trend for vecuronium induced intense neuromusclular blockade.
