2.A Case of Acrodermatitis Enteropathica Localized on the Hands and Feet with a Normal Serum Zinc Level.

Acrodermatitis enteropathica is classified as a congenital autosomal recessive type and an acquired transient type. This disease manifests as acral and periorificial dermatitis, alopecia, intractable diarrhea, and failure to thrive. Whereas the autosomal hereditary type is caused by malabsorption of zinc in the intestine, the acquired type is caused by low nutritional support or decreased peripheral release of zinc from blood. We experienced a case of a 5-month old, breast feeding, full-term female presenting with only acral bullous dermatitis without diarrhea, periorificial dermatitis and an abnormal serum zinc level.

3.Expression of Eotaxin and RANTES mRNA in Nasal Polyps and Its Effect on Eosinophils.

BACKGROUND AND OBJECTIVES: Nasal polyps usually develop in the ethmoidal and middle turbinate area, often in relation to inflammatory conditions. But the exact etiology and pathogenesis are still under debate. Histologically, the polyps are infiltrated by a number of inflammatory cells with eosinophil predominating in most specimens, and it is suggested that the nasal polyp is an inflammatory growth that is controlled by the local microenvironment. Eotaxin and RANTES have been postulated to be involved in the recruitment and activation of eosinophils to certain inflamed tissues. The purpose of this study is to investigate eotaxin and RANTES mRNA expression in nasal polyps and its effect on tissue and nasal eosinophils. MATERIALS AND METHODS: Seventeen allergic and 18 non-allergic nasal polyps were obtained from an endoscopic sinus surgery and 8 normal inferior turbinates were also taken. lmmunohistochemical staining for eosinophil and RANTES and quantitative RT- ICRs for eotaxin mRNA expression were performed. RESULTS: The eotaxin mRNA levels in allergic nasal polyps were 11.4 fold higher and non-allergic polyps were 6.4 fold higher than the normal inferior turbinates. But the RANTES didn't show any differences among the three groups. Tissue eosinophilia and its activation were significantly correlated with the eotaxin mRNA level but not with the RANTES level. CONCLUSION: In nasal polyp, eosinophilic infiltration and activation is mainly correlated with the increased eotaxin gene expression and, to some extent, with the case of RANTES.

4.The three dimensional finite element analysis of the stress distribution in the three treatment options of implants restorations for the posterior partial edentulism.

In this study, three treatment options to replace two posterior missing teeth were investigated using three dimensional finite element analysis: two wide(.5.0mm) implants(the experimental model I), two standard(.3.75mm) implants(the experimental model II), and three standard(.3.75mm) implants(the experimental model III). Two kinds of load case were applied ; 1) perpendicular on occlusal surface(axial load), parallel on occlusal surface(lateral load). 2) perpendicular on occlusal surface(3mm lateral to central point). The results obtained from this study were as follows; value of on-mises stress (equivalent stress) was smallest in the two wide implant among the three experimental models. It was reported that the diameter is the efficient factor than osseointegrated surface area.

5.Clinical Usefulness of Long-term Application of Fentanyl Matrix in Chronic Non-Cancer Pain: Improvement of Pain and Physical and Emotional Functions.

BACKGROUND: Opioids are recently recommended for those who do not gain adequate pain relief from the use of acetaminophen or nonsteroidal anti-inflammatory drugs. Medical opioids are administered in various routes, and transdermal opioid products that can make up for the weaknesses of the oral or intravenous products have been developed. This study is to evaluate the clinical usefulness of fentanyl matrix in terms of the long-term improvement in pain and physical and mental functions. METHODS: This was a multicenter, open, prospective, observational study that was conducted in 54 institutions in Korea. Patients with non-cancerous chronic pain completed questionnaires, and investigators also completed questionnaires. A total of 1,355 subjects participated in this study, and 639 subjects completed the study. Subjects received transdermal fentanyl matrix (12 µg/hr, 25 µg/hr, or 50 µg/hr depending on the patient's response and demand). Subjects visited at 29 ± 7 days, 85 ± 14 days, and 169 ± 14 days after administration, respectively, to receive drug titration and fill out the questionnaires. The results were analyzed using the intention-to-treat (ITT) analysis, full analysis set (FAS), and per-protocol (PP) analysis. The FAS analysis included only 451 participants; the PP analysis, 160 participants; and the ITT analysis, 1,355 participants. RESULTS: The intensity of pain measured by the Numeric Rating Scale decreased from 7.07 ± 1.78 to 4.93 ± 2.42. The physical assessment score and mental assessment score of the Short-Form Health Survey 12 improved from 28.94 ± 7.23 to 35.90 ± 10.25 and from 35.80 ± 11.76 to 42.52 ± 10.58, respectively. These differences were significant, and all the other indicators also showed improvement. Adverse events with an incidence of ≥ 1% were nausea, dizziness, vomiting, and pruritus. CONCLUSIONS: The long-term administration of fentanyl matrix in patients with non-cancerous pain can reduce the intensity of pain and significantly improves activities of daily living and physical and mental capabilities.
